accompanied by a valid, unexpired foreign passport.<br />

not expired, (I-551, I-688, and I-766).<br />

• Re-entry Permit form (I-327).<br />

• Refugee Travel Document form (I-571).<br />

Pro<strong>of</strong> <strong>of</strong> Current Full Legal Name<br />

You are required to present pro<strong>of</strong> <strong>of</strong> your full legal name. Your full<br />

legal name is the name recorded on the pro<strong>of</strong> <strong>of</strong> legal presence document.<br />

If your current full legal name is different than the name shown on your<br />

pro<strong>of</strong> <strong>of</strong> legal presence, you will be required to present additional pro<strong>of</strong> <strong>of</strong><br />

identity or legal name change documents.<br />

Examples <strong>of</strong> acceptable identity documents that can be used to<br />

prove your current full legal name include: current driver license,<br />

instruction permit or identifi cation card; military identifi cation or<br />

common access card; or an out <strong>of</strong> state driver license, instruction<br />

permit or identifi cation card. For a complete list <strong>of</strong> acceptable identity<br />

documents visit www.<strong>Oregon</strong>DMV.com.<br />

Acceptable pro<strong>of</strong> <strong>of</strong> legal name change includes: an <strong>of</strong>fi cial<br />

government-issued marriage certifi cate/license; certifi cate <strong>of</strong> registered<br />

domestic partnership issued by vital statistics; U.S. court issued divorce<br />

decree; judgment <strong>of</strong> dissolution or annulment <strong>of</strong> marriage or domestic<br />

partnership; adoption decree; or court ordered name change. All<br />

documents must be signed by a government <strong>of</strong>fi cial <strong>of</strong> the issuing agency<br />

and include a stamp or seal signifying that the document has been fi led<br />

with the state and/or county.<br />

Social Security Number<br />

You are required to provide your Social Security Number (SSN) issued<br />

by the U.S. Social Security Administration on the application. The SSN<br />

you provide will be verifi ed with the Social Security Administration.<br />

If you are not eligible for a SSN, you are required to provide pro<strong>of</strong><br />

<strong>of</strong> ineligibility and sign a Statement <strong>of</strong> Not Eligible for Social Security<br />

Number form. If you knowingly supply a false statement, you may be<br />

fi ned, imprisoned or both and DMV will suspend your driver license,<br />

permit or identifi cation card.<br />
