4) <strong>Palliative</strong> care should be part of a comprehensive package of care that includes ART, OI<br />

prevention, <strong>and</strong> treatment <strong>and</strong> prevention <strong>for</strong> positives.<br />

5) <strong>Palliative</strong> care should be family-centered.<br />

6) <strong>Palliative</strong> care services should be tailored to the specific needs of different populations.<br />

Continuum oF <strong>Care</strong><br />

Because life-threatening <strong>diseases</strong> are often chronic, the needs of patients <strong>and</strong> their families<br />

will change over time. Thus, a range of palliative care services must be available at each<br />

point of disease. A continuum of care is needed in order to best meet changing needs<br />

over time. The continuum of care (see figure 2) is a set of comprehensive <strong>and</strong> linked care,<br />

treatment, <strong>and</strong> support services provided at all levels, from health facility to community<br />

to home. Services are generally provided by a combination of providers, including government,<br />

NGOs, community- <strong>and</strong> faith-based organizations, PL<strong>HIV</strong>, <strong>and</strong> family members.<br />

hubs oF <strong>Care</strong><br />

Given the importance of coordinated care <strong>for</strong> PL<strong>HIV</strong>, palliative care should be provided in<br />

t<strong>and</strong>em with <strong>other</strong> essential care at the same site wherever possible. <strong>HIV</strong> care is often provided<br />

through <strong>HIV</strong> outpatient clinics or ART clinics. Providers at these sites can be trained<br />

in palliative care <strong>and</strong> supported with clinical assessment tools <strong>and</strong> medicines as needed to<br />

provide basic palliative care on-site.<br />
