Key Executives<br />
Michael L. Cotton – CEO, <strong>Alere</strong> Health<br />
Mr. Cotton became CEO of <strong>Alere</strong> Health in January 2012. Previously, he joined<br />
<strong>Alere</strong> in September 2010 as president of <strong>Alere</strong>’s Health Improvement Division.<br />
With more than 22 years of managed care and healthcare consulting<br />
experience, Mr. Cotton came to <strong>Alere</strong> from WellCare Health Plans, Inc., where<br />
he served as President of National Health Plans, overseeing the company’s<br />
health plan markets, operations, national provider contracting and business<br />
development.<br />
Prior to WellCare, Mr. Cotton served as a worldwide partner and market leader<br />
for Mercer Human Resources Consulting from October 2001 to December<br />
2005. Before Mercer, he held the positions of president and chief executive<br />
officer of Mid-Valley CareNet, a physician hospital organization (PHO) with<br />
more than 600 physician members. Earlier in his career, Mr. Cotton was a<br />
regional president and chief executive officer for AMERIGROUP Corporation<br />
and senior vice president and chief operating officer of QualChoice Health<br />
Plans, Inc., a managed care plan of the University Hospital’s Health System.<br />
Mr. Cotton earned his undergraduate degree from Franklin University and his<br />
master’s degree in business administration from Cleveland State University.<br />
Gregg E. Raybuck – President, <strong>Alere</strong> Health<br />
Mr. Raybuck became President of <strong>Alere</strong> Health in February 2012. He is<br />
responsible for overall company strategy, performance and growth, providing<br />
leadership and direction to operations, sales, marketing, program development<br />
and reimbursement. He has total problem-solving responsibility for <strong>Alere</strong><br />
Health’s operations and expansion strategies. Previously, he served as<br />
President of the Women's and Children's Health Division beginning in
November 2006. Prior thereto, Mr. Raybuck served as Senior Vice President of<br />
Sales for Women's and Children's Health beginning in August 2006. Mr.<br />
Raybuck was associated with Matria Healthcare from 1992 to January 2001,<br />
last serving as President of the Cardiology business unit of the company. Prior<br />
to his appointment as President of the Cardiology business, Mr. Raybuck held<br />
the position of Vice President of Sales and Marketing for Women’s Health.<br />
Prior to his return to Matria before it was acquired, Mr. Raybuck served as Vice<br />
President of Sales and Marketing for CardioNet, Inc., a technology and service<br />
provider of remote wireless monitoring technology for cardiac patients from<br />
2004 to 2006. Prior thereto, he served as President and CEO of North<br />
American operations for Card Guard Inc., a global provider of medical devices<br />
and cardiac monitoring technology.<br />
Mr. Raybuck earned a bachelor’s degree in business from Thomas Edison<br />
State College.<br />
Sanford Becker – President, <strong>Alere</strong> Wellbeing<br />
Mr. Becker joined <strong>Alere</strong> Wellbeing (formerly Free & Clear ® ) in February 2005<br />
and has served as CFO and COO prior to becoming President in June 2011.<br />
His professional experience includes more than 35 years in management,<br />
operations, accounting and finance.<br />
Prior to <strong>Alere</strong>, Mr. Becker held senior management positions at several<br />
privately-held, venture-backed companies, including Radiant Research (a<br />
leading clinical research organization); Applied Discovery (a leader in providing<br />
Web-enabled business services to the legal industry, sold to LexisNexis); and<br />
Pointshare Corporation (a provider of online services to the healthcare industry,<br />
sold to Siemens Medical Solutions). He was a managing member of Compass<br />
Capital Advisors, a private merchant-banking firm that specialized in financing<br />
high-growth, privately held companies. Before that, Mr. Becker was a partner at<br />
Moss Adams CPAs.<br />
Mr. Becker’s particular areas of expertise include operations, financial<br />
management, strategic business growth and development, mergers and<br />
acquisitions and structuring debt and equity financings. During his career, Mr.<br />
Becker has arranged more than $1.4 billion in debt and equity transactions.<br />
He earned a bachelor’s degree in business administration from the University of<br />
Washington, and he is a Certified Public Accountant.<br />

Craig Apolinsky – Executive Vice President & General Counsel<br />
Mr. Apolinsky has overall management responsibility for <strong>Alere</strong>’s Legal<br />
Department. Previously, Mr. Apolinsky served as Vice President, General<br />
Counsel and Corporate Secretary for Merge Healthcare, Inc., a publicly-traded<br />
company that develops, licenses, implements and supports medical imaging<br />
and information software. He led Merge’s worldwide legal affairs for their U.S.,<br />
Canadian, Indian and European operations.<br />
Prior to Merge, he served as Deputy General Counsel and Assistant Secretary<br />
for Gold Kist, Inc. He was responsible for securities compliance, corporate<br />
governance, contracting and licensing matters for this publicly-traded company<br />
with annual revenues in excess of $2 billion. He began his legal career with<br />
Alston & Bird LLP, a prominent national legal firm, and practiced law with the<br />
firm until joining Gold Kist. Craig was a partner with Alston & Bird in its<br />
Corporate Health Care and Technology Practice Groups.<br />
Mr. Apolinsky earned his law degree from the University of Texas at Austin,<br />
School of Law and earned his bachelor’s degree from the University of Virginia.<br />
Joseph Blankenship – Executive Vice President & Chief<br />
Financial Officer<br />
Mr. Blankenship became Chief Financial Officer of <strong>Alere</strong> in March 2010.<br />
Previously, he served as Vice President and Corporate Controller, first for<br />
Matria Healthcare (acquired by Inverness Medical Innovations, Inc. in May<br />
2008) beginning in December 2004, and then for <strong>Alere</strong> beginning in May 2008.<br />
Prior to joining <strong>Alere</strong>, Mr. Blankenship held various senior executive financial<br />
positions for both private and public companies. He served as Controller for Air<br />
Serv Corporation from 2002 to 2004 and held the positions of Senior Vice<br />
President, Chief Financial Officer, Treasurer and Assistant Secretary while at<br />
Healthcare.com from 1999 to 2001. Mr. Blankenship served as Corporate<br />
Controller of Decorative Home Accents, Inc., a textile manufacturer, from<br />
October 1996 to December 1998, and as Director of Accounting of Allegiant<br />
Physician Services, Inc., a healthcare services company, from April 1994 to<br />

October 1996. A Certified Public Accountant, Mr. Blankenship earned a<br />
bachelor’s degree from West Virginia University.<br />
Pete Desai – Executive Vice President, Health Management<br />
Strategy<br />
Mr. Desai has more than 10 years of health industry experience with an<br />
extensive background in developing and implementing innovative healthcare<br />
programs and services. In his current role, Mr. Desai is responsible for driving<br />
strategy and business development, while supporting the global health<br />
management function, working with leadership to grow services and revenue in<br />
targeted geographies. Previously, Mr. Desai served in the roles of Senior Vice<br />
President of Health Strategy & Global Solutions beginning in 2011, Director of<br />
Strategy & Business Development beginning in 2008, and Director of Strategic<br />
Projects beginning in 2007.<br />
Previously, Mr. Desai served in various roles in Accenture’s Health & Life<br />
Science’s practice from 1999 to 2007, where he developed a broad healthcare<br />
background including experience working with health insurance payers,<br />
providers and behavioral health vendors. While at Accenture, Mr. Desai’s<br />
responsibilities ranged from defining corporate strategy and growth plans for<br />
clients to managing large-scale multi-million dollar implementation projects, all<br />
of which helped him build a core foundation in managing complex initiatives.<br />
Mr. Desai is a graduate of Vanderbilt University, where he earned a Bachelor of<br />
Science degree in biomedical engineering.<br />
Julie Griffin – Executive Vice President, Culture & Performance<br />
As Executive Vice President of Culture & Performance, Ms. Griffin is<br />
responsible for organizational development and human resource activities<br />
across <strong>Alere</strong>’s business units. She is accountable for creating a culture built on<br />
<strong>Alere</strong>’s six core values of teamwork, excellence, health, accountability, trust<br />
and innovation – a culture in which associates can develop and succeed. Ms.<br />
Griffin also works closely with the C&P Business Partners to build a<br />
performance strategy consistent with <strong>Alere</strong>’s leadership.<br />
Ms. Griffin first joined <strong>Alere</strong> as Vice President of Human Resources when it was<br />
still Matria Healthcare in September 2006. She was promoted to her current<br />

ole in September 2009. Ms. Griffin offers 15 years of progressive management<br />
experience in human resources with a wide range of experience in the areas of<br />
accounting, administration and operations. She has served in a variety of<br />
leadership roles for Loyaltyworks, Inc., Benecorp, Quovadx, Inc. (a spin-off of<br />
Matria Healthcare), Scottish Rite Children’s Medical Center and Allegiant<br />
Physician Services, Inc.<br />
Ms. Griffin attended Georgia State University and is certified by the Society for<br />
Human Resource Management.<br />
Garrett Hale – Executive Vice President, Clinical Operations<br />
Mr. Hale has served as Executive Vice President of Clinical Operations for<br />
<strong>Alere</strong> Health since March 2012. Previously, he served in executive leadership<br />
roles for the Women’s and Children’s Health Division, starting with Vice<br />
President when he joined the company in 2005, followed by Senior Vice<br />
President, and later Executive Vice President of Operations beginning in 2008.<br />
His diversified experience in process improvement, operations management,<br />
change leadership, organizational design, Six Sigma process reengineering<br />
(Black Belt), complex project management, and call center leadership provides<br />
him with the insight and experience necessary to work with and understand the<br />
needs of clients. Mr. Hale works both externally with clients and internally with<br />
the product development team to ensure the products and services <strong>Alere</strong><br />
delivers are not only in line with current trends, but also anticipate the future of<br />
the marketplace.<br />
Prior to <strong>Alere</strong>, Mr. Hale served as Senior Manager with The North Highland<br />
Company, where he consulted with Fortune 500 companies specializing in<br />
organizational change management, process improvement, organizational<br />
design and complex program management. Mr. Hale also has tenure with The<br />
Coca-Cola Company and with Arthur Andersen, where he worked with<br />
accounts and clients representing a variety of industries, including healthcare,<br />
food services, financial services, manufacturing and telecommunications.<br />
Mr. Hale earned his bachelor’s degree in accounting from MidAmerica<br />
Nazarene University.<br />

Steven Kelley – Executive Vice President, Technology<br />
Solutions<br />
Steven Kelley has served as Executive Vice President of Technology Solutions<br />
for <strong>Alere</strong> Health since February 2012. He is responsible for all aspects of IT,<br />
including application development and support, IT operations and<br />
infrastructure, and network and telecom. Steven joined <strong>Alere</strong> in February<br />
2011. Previously, Mr. Kelley was the Owner and Principal of Standard Door, a<br />
local-Atlanta independent manufacturing company. Prior to Standard Door,<br />
Steven was an Associate Partner with Accenture, where he spent 20 years<br />
delivering IT consulting services to Fortune 500 clients.<br />
Mr. Kelley brings more than 25 years of leadership experience delivering<br />
operational results both as a small business owner and in high-performance<br />
corporate environments. He has a strong background in business operations<br />
as well as extensive experience delivering large-scale strategic change<br />
programs and IT projects across a broad range of industries, including<br />
healthcare, financial services, telecommunications, and manufacturing.<br />
Steven earned a Bachelor of Science degree from Vanderbilt University and a<br />
master’s degree in business administration from the University of North<br />
Carolina at Chapel Hill.<br />
Craig Keyes, MD, MBA – Executive Vice President & Chief<br />
Medical Officer<br />
Dr. Keyes joined <strong>Alere</strong> Health in March 2011 as Executive Vice President and<br />
Chief Medical Officer. In this role, he is responsible for the direct oversight of<br />
our enterprise Quality and Accreditation programs, the clinical solutions that we<br />
deliver through our various programs, and the measurement of the<br />
effectiveness of the programs we deliver to our clients.<br />
A lifelong advocate of health and well-being, Dr. Keyes most recently founded<br />
SportXcel, a youth sports performance and adult fitness enterprise in Metro<br />
Denver, which provides both commercial and charitable research-based fitness,<br />
conditioning and injury-prevention programs for youth, adults, seniors and elite<br />
athletes. Prior to SportXcel, Dr. Keyes held senior executive positions over a<br />
10-year span with United Healthcare from 1998-2008, including CEO-New<br />
York, CEO-Colorado/New Mexico, National SVP of Mid-Market Sales, and<br />
Chief Medical Officer of the 16 markets in the North Division.<br />

Dr. Keyes has also held prior positions as a Physician Surveyor for the National<br />
Committee for Quality Assurance and as Medical Director for IPRO, a nationally<br />
recognized healthcare assessment and improvement organization. Dr. Keyes<br />
began his career in the practice of medicine serving under- and un-insured<br />
people with HIV/AIDS in New York City.<br />
Dr. Keyes earned his undergraduate and medical degrees, both with Honors, at<br />
the University of Wisconsin, and completed his internship and residency at<br />
Columbia University and Lenox Hill Hospital in New York City. He is Board<br />
Certified in both Internal Medicine and Geriatrics. He also earned a master’s<br />
degree in business administration from Columbia University School of<br />
Business.<br />
Scott McClintock – Executive Vice President & Chief Marketing<br />
Officer<br />
Mr. McClintock has led Marketing for <strong>Alere</strong> since January 2011. Previously, he<br />
served as Senior Vice President of Marketing for <strong>Alere</strong> beginning in 2008. Prior<br />
to this role, he served as Chief Marketing Officer of Matria Healthcare (acquired<br />
by Inverness Medical Innovations, Inc. in May 2008) beginning in December<br />
2006. Prior to joining Matria, Mr. McClintock served as Senior Vice President,<br />
Head of Global Marketing for Premiere Global Services, a $500 million<br />
international leader in teleconferencing services. He was responsible for all<br />
aspects of marketing for PGI. Prior thereto, he headed up branding, marketing<br />
and communications for Equant (now Orange Business Services), a $3 billion<br />
international leader in providing data communication solutions to Global 500<br />
companies. He joined Equant at its inception and was instrumental in growing<br />
the company from startup, through its IPO and finally to its sale to France<br />
Telecom.<br />
Prior to Equant, Mr. McClintock served as Vice President, Management<br />
Supervisor for several advertising agencies including BBDO, Ogilvy & Mather,<br />
Rubin Postaer, and WayneWest, where he worked on the Shell Oil, Compaq,<br />
MediaOne (now Comcast), Pizza Hut and BellSouth accounts. He began his<br />
career as a systems engineer with Raytheon and brings more than 20 years of<br />
marketing and communications experience to <strong>Alere</strong>.<br />
Mr. McClintock earned his bachelor’s degree from the University of Texas,<br />
Austin, where he double-majored in computer science and marketing.<br />

Gordon K. Norman, MD, MBA – Executive Vice President &<br />
Chief Innovation Officer<br />
Dr. Norman joined <strong>Alere</strong> in 2005 when <strong>Alere</strong> purchased PacifiCare's disease<br />
management business and became their exclusive disease management<br />
vendor. In 2004, Dr. Norman was named among the Top 25 Most Influential<br />
People in Disease Management by Managed Healthcare Executive magazine.<br />
In that year, he also joined the Disease Management Association of America’s<br />
(DMAA) Board of Directors. Prior to joining <strong>Alere</strong>, Dr. Norman served as<br />
Executive Director of PacifiCare's dedicated disease management unit. He also<br />
served as Vice President of Health Care Quality for PacifiCare, as well as<br />
Regional Medical Director and Senior Plan Medical Director for PacifiCare<br />
of California.<br />
Before joining PacifiCare, he earned a master’s degree in business<br />
administration from Stanford University, where he previously earned degrees in<br />
psychology and medicine. Following family medicine training in Maine and New<br />
York, Dr. Norman entered private practice in upstate New York and later New<br />
Hampshire, where he helped launch a physician-led HMO and served as a<br />
hospital medical director and lead physicians in a "group without walls" primary<br />
care practice.<br />
Scott Schell, MD, PhD, MBA – Executive Vice President & Chief<br />
Clinical Officer<br />
Dr. Schell assumed the role of Chief Clinical Officer in March 2012. He is<br />
responsible for clinical compliance, quality management, medical affairs,<br />
analytics and clinical operations. Dr. Schell joined <strong>Alere</strong> in 2009 as a clinical<br />
consultant and member of <strong>Alere</strong>’s Clinical Advisory Board. Previously, he was<br />
Chief Executive Officer of Innovatim, LLC, a clinical consulting company, which<br />
he launched in 2002. Throughout his career, Dr. Schell also launched three<br />
companies in the healthcare technology and electronic medical records spaces,<br />
and spent extensive time consulting pharmaceutical, biotech and medical<br />
device companies.<br />
After earning his Society of Surgical Oncology Certification in 1998, Dr. Schell<br />
served in various faculty and staff appointments from 1998 to 2008 for Johns<br />
Hopkins University, the University of Florida, the University of Medicine &<br />
Dentistry, a hospital system in Michigan and the Cleveland Clinic.<br />

Dr. Schell earned his bachelor’s degree, doctorate and medical degree from the<br />
University of Chicago. He completed his internship and residency at Johns<br />
Hopkins University and a fellowship in surgical oncology at Oxford University in<br />
England. He is a Board Certified Oncologist and earned his master’s degree in<br />
business administration from the University of Michigan.<br />
Dr. Schell has authored more than 70 research studies and given more than<br />
100 lectures and presentations at both national and international institutions.<br />
