SDMS Leaders Participating in ACR’s
Haitian Relief Effort

Ted Whitten, BA, RDMS, RVT, was one of two
SDMS Board Members who traveled to Haiti
to provide didactic and hands-on training in
ultrasound to health-care providers as they
attempt to rebuild from the earthquake that
further devastated the poorest country in the
Western Hemisphere.
“I have to say that it truly was a life-changing
experience,” says Whitten, an ultrasound
practitioner at Elliot Hospital in Manchester, N.H.
The two days Whitten, and fellow sonographer,
Tammy Stearns, MS, RT(R), RDMS, RVT, of
Missouri, an adjunct professor for Florida
Hospital College of Health Sciences, spent
teaching sonographic techniques in Haiti’s
capital and largest city was part of the American
College of Radiology Foundation’s ongoing
Haitian Radiology Relief Program.
When the earthquake hit Haiti in January 2010,
the devastation was so massive and its needs
so great that the country became a priority for

About a year after the earthquake, a delegation

Phelps, MD, a long-term volunteer at Grace Children’s
Hospital, had gone on a fact-finding mission to
determine the state of the country’s imaging services.
They also looked for the best ways for ACR could assist
in improving radiology there.
“When they came back, they discussed a longerterm
plan,” says Brad Short, senior director of the
ACR Member Services Department and staff for the
international outreach program, who accompanied
them.
Sonography training a priority
One of the big needs they saw on their fact-finding
mission was the need for training in sonography.
“Anything they can treat there that needs crosssectional
imaging can be accomplished with
sonography,” says Borgstede. “If you need a CT scanner
or MR, for the most part, they couldn’t treat it anyway.”
Also, ultrasound equipment is portable and lightweight
and relatively inexpensive.
So the organizers planned two “Radiation Education
Days” for early June of this year that included
instruction in plain film X-ray and sonography. They
invited health-care practitioners from across the
country to attend. (Haiti only has 19 radiologists for
a population of about 10 million people, Borgstede
notes.)
The ACR also invited SDMS to participate. The SDMS
and SDMS Foundation boards were both eager to
support the relief effort. SDMS and the Foundation
not only contributed financially but also identified
sonographers – Whitten and Stearns – who were
willing and able to travel to Haiti on relatively short
notice to help teach.
Stearns was scheduled to give a lecture on
sonography. “But I didn’t know the level of experience
or education those attending would have,” she says.
So she prepared two lectures – one on the very basics
and one for advance scanners. Turns out she didn’t
need the second. “When we got there, we found only
seven of those who attended had any experience
scanning,” she says.
Grace Children’s Hospital has one ultrasound machine.
Sonosite provided three portable ultrasound units
and GE Healthcare one for the delegation to use in
teaching. The units and multiple transducers were on
loan as part of the hands-on training program.
Response was overwhelming

The Association of Private Hospitals had sent a mass
mailing about the education days to its members,
which included more than 100 Haitian physicians.
Interest was greater than the organizers anticipated.
“We were planning for 60 but we had 90 attendees,”
Whitten says.
Physicians, technicians and sonographers from
throughout Haiti including Port-au-Prince, Cap Haitian,
St. Marc, Fond des Blancs and the Central Plateau
attended. The event was held in a small conference
room in the administrative office building that housed
International Child Care (ICC) and was adjacent to a lot
that once contained Grace Children’s Hospital. Eighty
to 90 percent of the hospital had been destroyed in
the earthquake, Short says.
A Canadian, John Yates, MD, former medical director
at Grace, has been assisting the rebuilding efforts of
the hospital. Yates’ children, who had grown up in Haiti
and returned to work there after earning their degrees
in Canada, helped the members of the delegation
navigate customs and provided logistical support for
the meeting. Their support was very helpful, Whitten
says.
Ernst Garcon, MD, another radiologist delegate who
practices at Columbia University in New York City, was
born and raised in northern Haiti and was eager to
return to his homeland. “He was very helpful to us as
well because he speaks French and Creole,” Whitten
says.
Some of the Haitian attendees spoke English, but the
ACR had to arrange for translators. Although French

is the official language, many people in Haiti speak
Creole.
The weather was far from ideal. It was like a sauna
– hot and humid and quite oppressive, Whitten
says. Some of the hands-on classes were held in a
temporary facility across the street from the hospital
that consisted of container pod buildings. “Two of the
scanning rooms had AC, but Tammy and I were not so
fortunate,” Whitten says with a laugh.
Whitten, Stearns impressed with interest
Whitten and Stearns were impressed with how quickly
the participants caught on to what they showed them.
“Hands-on scanning takes time to learn, but they
grasped the concepts fairly quickly,” Whitten says.
“They couldn’t possibly learn everything they needed
to know in that first session, but every time we go
back, they’ll have an opportunity to improve their
scanning skills and learn more.”
Because it was hard to cover all that was necessary
in such a short time, they shared emails for future
contact. “That way if they have questions they
can contact us,” he says. “We expect ongoing
communications with them.”
Stearns, too, was impressed at how eager the
participants were to learn. “At breaks, we would be
met by participants clutching the one book they
had that was completely worn through that they
had highlighted with questions,” she says. “They
were happy to have someone they could ask for
clarification.”
Since she returned to the states, Stearns has heard via
email from some physicians with questions they have
about what they’re seeing on sonograms. One of the
questions was about an artifact that they were seeing

on scans of pregnant mothers. “I was able to tell them
it was due to the physics of sound and that, ‘Yes, you’re
doing it right’ and give them the encouragement to
keep practicing,” Stearns says.
Patients who came to the clinic volunteered to serve
as models. The sonographers focused on diseases that
are commonly found in Haiti and those conditions
where abdominal and OBGYN sonograms would be
useful, Borgstede says. “We were looking at things like
abscesses and basic OB so that it would be as practical
and useful as possible,” he says.
“They weren’t interested in vascular or echo,” Stearns
adds. “We concentrated on OB and abdominal studies
because that’s what they’re going to see in their
clinics.”
One of the volunteers was an older woman who spoke
Creole and whose stomach was extended. Stearns was
suspicious when she scanned the woman’s abdomen
prior to the training session. It turned out she had
cancer that had spread to her liver. “We found out later
that she died soon after we were there,” Stearns says.
“But they were able to get her some help.”
‘Long days’ but worth it
The training days were long. “We started early, at 6
a.m.,” Whitten says. “And we didn’t return to our hotel
until late in the evening.”
Short says the ACR was thrilled to have Whitten
and Stearns. “They both were excited to attend
and it wasn’t just their clinical experience, but their
enthusiasm they brought to the endeavor that made
it meaningful for both the healthcare practitioners
and patients and to our overall efforts on the ground
there.”

Even though attendance was greater than expected
everything went smoothly. “It all went off without a
hitch,” Short says.
Two of the 12 delegates on the trip were from the
American Society of Radiologic Technologists (ASRT),
Jim Temme, MPA, RT(R)(QM), ASRT chairman of
the board, and Kevin Powers, EdS, RT(R)(M), ASRT’s
director of education.
More trips likely
An ACR delegation is planning on returning to Haiti
to provide more education and equipment. “We don’t
know the details yet,” Short says. “But we are planning
more trips. Our commitment is to not only improve
radiology services in Haiti but to make it sustainable.
We understand that it is going to take a long-term
commitment and education is going to be part of that
commitment.”
Borgstede says that thanks to everyone’s input, the
education days were a tremendous success. “Now

it’s time to assess what we achieved and decide
where we go from here,” he says. Borgstede, who
has volunteered in other impoverished countries,
says Haiti has the advantage of proximity to the U.S.
“You can get back and forth in a day, so it’s really an
opportunity for us to assist in Haiti.”
Whitten says the experience was so overwhelming
that he found it very difficult to leave his newly made
physician friends in Haiti and the delegates with
whom he traveled. “I would love to go again,” he says.
“If there was another trip next month, I would go. It
was so worth it.”
Stearns, too, would love to go back and teach in Haiti
again, she says. “In a heartbeat.”

Throughout high school,
Courtney excelled in math
and science and knew she
wanted to pursue a career in
the medical field. She tossed around the options of
becoming a psychiatrist or radiologic technologist.
Although she loved helping people, she thought
SDMS News Wave June 2012 5
psychiatry would not allow her to use her math skills
as much as she wanted to and the thought of viewing
broken bones through x-ray for the rest of her life
was not appealing. Through more research, Courtney
found diagnostic medical sonography and knew
the field was a fit for her career goals. The aspect
of sonography that inspired her the most was the
detective work and solving puzzles. She was intrigued
by the way clinical symptoms could be correlated
with the findings of an ultrasound which eventually
led to diagnosing the patient.
Courtney said her favorite SDMS benefit so far is the
Journal of Diagnostic Medical Sonography (JDMS).
The hard copy of the journal she receives in the mail
allows her to continue learning while being away
from an electronic environment and school. Courtney
has often times applied what she has read in the
journal to her every day scanning.

Why did your facility make the commitment to
employ/hire only credentialed sonographers?
By committing to hiring only credentialed
sonographers, it shows our commitment to their
professionalism and the willingness to maintain high
standards of care for our patients. It also demonstrates
a degree of competency for our sonographers. We
were also looking to the future with all of the healthcare
costs and reimbursement issues surrounding
us, it may be just a matter of time before our
insurance companies require the sonographers
to be credentialed in order to receive maximum
reimbursement.
How has your facility benefitted from having an “all
credentialed sonographer” environment?
Without a doubt, being credentialed requires every
associate to maintain their continuing education
credits. By doing so, they are kept up-to-date in
their field of ultrasound with the latest and greatest
techniques and procedures. Most all of them are
also members of SDMS. Many times, with the use
of internet and media, patients are looking for the
best care in their area, and by having all credentialed

sonographers, they are confident the sonograms
they are receiving at WFHC-All Saints is the best in
the area.
How have your patients benefitted from having an
“all credentialed sonographer” environment?
We feel like the quality of care we give our
patients is improved by having all credentialed
sonographers. Patients can be re-assured we are
operating with high standards and that they are
being scanned by professionals who take much
pride in their work. Knowing all of this, patient
satisfaction is higher as well.
