Ted Whitten, BA, RDMS, RVT, was one of two<br />

<strong>SDMS</strong> Board Members who traveled to Haiti<br />

to provide didactic and hands-on tra<strong>in</strong><strong>in</strong>g <strong>in</strong><br />

ultrasound to health-care providers as they<br />

attempt to rebuild from the earthquake that<br />

further devastated the poorest country <strong>in</strong> the<br />

Western Hemisphere.<br />

“I have to say that it truly was a life-chang<strong>in</strong>g<br />

experience,” says Whitten, an ultrasound<br />

practitioner at Elliot Hospital <strong>in</strong> Manchester, N.H.<br />

The two days Whitten, and fellow sonographer,<br />

Tammy Stearns, MS, RT(R), RDMS, RVT, of<br />

Missouri, an adjunct professor for Florida<br />

Hospital College of Health Sciences, spent<br />

teach<strong>in</strong>g sonographic techniques <strong>in</strong> Haiti’s<br />

capital and largest city was part of the American<br />

College of Radiology Foundation’s ongo<strong>in</strong>g<br />

<strong>Haitian</strong> Radiology <strong>Relief</strong> Program.<br />

When the earthquake hit Haiti <strong>in</strong> January 2010,<br />

the devastation was so massive and its needs<br />

so great that the country became a priority for<br />

Program, which it had established prior to the<br />

<strong>Haitian</strong> disaster.<br />

About a year after the earthquake, a delegation<br />

Phelps, MD, a long-term volunteer at Grace Children’s<br />

Hospital, had gone on a fact-f<strong>in</strong>d<strong>in</strong>g mission to<br />

determ<strong>in</strong>e the state of the country’s imag<strong>in</strong>g services.<br />

They also looked for the best ways for ACR could assist<br />

<strong>in</strong> improv<strong>in</strong>g radiology there.<br />

“When they came back, they discussed a longerterm<br />

plan,” says Brad Short, senior director of the<br />

ACR Member Services Department and staff for the<br />

<strong>in</strong>ternational outreach program, who accompanied<br />

them.<br />

Sonography tra<strong>in</strong><strong>in</strong>g a priority<br />

One of the big needs they saw on their fact-f<strong>in</strong>d<strong>in</strong>g<br />

mission was the need for tra<strong>in</strong><strong>in</strong>g <strong>in</strong> sonography.<br />

“Anyth<strong>in</strong>g they can treat there that needs crosssectional<br />

imag<strong>in</strong>g can be accomplished with<br />

sonography,” says Borgstede. “If you need a CT scanner<br />

or MR, for the most part, they couldn’t treat it anyway.”<br />

Also, ultrasound equipment is portable and lightweight<br />

and relatively <strong>in</strong>expensive.<br />

So the organizers planned two “Radiation Education<br />

Days” for early June of this year that <strong>in</strong>cluded<br />

<strong>in</strong>struction <strong>in</strong> pla<strong>in</strong> film X-ray and sonography. They<br />

<strong>in</strong>vited health-care practitioners from across the<br />

country to attend. (Haiti only has 19 radiologists for<br />

a population of about 10 million people, Borgstede<br />

notes.)<br />

The ACR also <strong>in</strong>vited <strong>SDMS</strong> to participate. The <strong>SDMS</strong><br />

and <strong>SDMS</strong> Foundation boards were both eager to<br />

support the relief effort. <strong>SDMS</strong> and the Foundation<br />

not only contributed f<strong>in</strong>ancially but also identified<br />

sonographers – Whitten and Stearns – who were<br />

will<strong>in</strong>g and able to travel to Haiti on relatively short<br />

notice to help teach.<br />

Stearns was scheduled to give a lecture on<br />

sonography. “But I didn’t know the level of experience<br />

or education those attend<strong>in</strong>g would have,” she says.<br />

So she prepared two lectures – one on the very basics<br />

and one for advance scanners. Turns out she didn’t<br />

need the second. “When we got there, we found only<br />

seven of those who attended had any experience<br />

scann<strong>in</strong>g,” she says.<br />

Grace Children’s Hospital has one ultrasound mach<strong>in</strong>e.<br />

Sonosite provided three portable ultrasound units<br />

and GE Healthcare one for the delegation to use <strong>in</strong><br />

teach<strong>in</strong>g. The units and multiple transducers were on<br />

loan as part of the hands-on tra<strong>in</strong><strong>in</strong>g program.<br />

Response was overwhelm<strong>in</strong>g<br />

The Association of Private Hospitals had sent a mass<br />

mail<strong>in</strong>g about the education days to its members,<br />

which <strong>in</strong>cluded more than 100 <strong>Haitian</strong> physicians.<br />

Interest was greater than the organizers anticipated.<br />

“We were plann<strong>in</strong>g for 60 but we had 90 attendees,”<br />

Whitten says.<br />

Physicians, technicians and sonographers from<br />

throughout Haiti <strong>in</strong>clud<strong>in</strong>g Port-au-Pr<strong>in</strong>ce, Cap <strong>Haitian</strong>,<br />

St. Marc, Fond des Blancs and the Central Plateau<br />

attended. The event was held <strong>in</strong> a small conference<br />

room <strong>in</strong> the adm<strong>in</strong>istrative office build<strong>in</strong>g that housed<br />

International Child Care (ICC) and was adjacent to a lot<br />

that once conta<strong>in</strong>ed Grace Children’s Hospital. Eighty<br />

to 90 percent of the hospital had been destroyed <strong>in</strong><br />

the earthquake, Short says.<br />

A Canadian, John Yates, MD, former medical director<br />

at Grace, has been assist<strong>in</strong>g the rebuild<strong>in</strong>g efforts of<br />

the hospital. Yates’ children, who had grown up <strong>in</strong> Haiti<br />

and returned to work there after earn<strong>in</strong>g their degrees<br />

<strong>in</strong> Canada, helped the members of the delegation<br />

navigate customs and provided logistical support for<br />

the meet<strong>in</strong>g. Their support was very helpful, Whitten<br />

says.<br />

Ernst Garcon, MD, another radiologist delegate who<br />

practices at Columbia University <strong>in</strong> New York City, was<br />

born and raised <strong>in</strong> northern Haiti and was eager to<br />

return to his homeland. “He was very helpful to us as<br />

well because he speaks French and Creole,” Whitten<br />

says.<br />

Some of the <strong>Haitian</strong> attendees spoke English, but the<br />

ACR had to arrange for translators. Although French


is the official language, many people <strong>in</strong> Haiti speak<br />

Creole.<br />

The weather was far from ideal. It was like a sauna<br />

– hot and humid and quite oppressive, Whitten<br />

says. Some of the hands-on classes were held <strong>in</strong> a<br />

temporary facility across the street from the hospital<br />

that consisted of conta<strong>in</strong>er pod build<strong>in</strong>gs. “Two of the<br />

scann<strong>in</strong>g rooms had AC, but Tammy and I were not so<br />

fortunate,” Whitten says with a laugh.<br />

Whitten, Stearns impressed with <strong>in</strong>terest<br />

Whitten and Stearns were impressed with how quickly<br />

the participants caught on to what they showed them.<br />

“Hands-on scann<strong>in</strong>g takes time to learn, but they<br />

grasped the concepts fairly quickly,” Whitten says.<br />

“They couldn’t possibly learn everyth<strong>in</strong>g they needed<br />

to know <strong>in</strong> that first session, but every time we go<br />

back, they’ll have an opportunity to improve their<br />

scann<strong>in</strong>g skills and learn more.”<br />

Because it was hard to cover all that was necessary<br />

<strong>in</strong> such a short time, they shared emails for future<br />

contact. “That way if they have questions they<br />

can contact us,” he says. “We expect ongo<strong>in</strong>g<br />

communications with them.”<br />

Stearns, too, was impressed at how eager the<br />

participants were to learn. “At breaks, we would be<br />

met by participants clutch<strong>in</strong>g the one book they<br />

had that was completely worn through that they<br />

had highlighted with questions,” she says. “They<br />

were happy to have someone they could ask for<br />

clarification.”<br />

S<strong>in</strong>ce she returned to the states, Stearns has heard via<br />

email from some physicians with questions they have<br />

about what they’re see<strong>in</strong>g on sonograms. One of the<br />

questions was about an artifact that they were see<strong>in</strong>g<br />

on scans of pregnant mothers. “I was able to tell them<br />

it was due to the physics of sound and that, ‘Yes, you’re<br />

do<strong>in</strong>g it right’ and give them the encouragement to<br />

keep practic<strong>in</strong>g,” Stearns says.<br />

Patients who came to the cl<strong>in</strong>ic volunteered to serve<br />

as models. The sonographers focused on diseases that<br />

are commonly found <strong>in</strong> Haiti and those conditions<br />

where abdom<strong>in</strong>al and OBGYN sonograms would be<br />

useful, Borgstede says. “We were look<strong>in</strong>g at th<strong>in</strong>gs like<br />

abscesses and basic OB so that it would be as practical<br />

and useful as possible,” he says.<br />

“They weren’t <strong>in</strong>terested <strong>in</strong> vascular or echo,” Stearns<br />

adds. “We concentrated on OB and abdom<strong>in</strong>al studies<br />

because that’s what they’re go<strong>in</strong>g to see <strong>in</strong> their<br />

cl<strong>in</strong>ics.”<br />

One of the volunteers was an older woman who spoke<br />

Creole and whose stomach was extended. Stearns was<br />

suspicious when she scanned the woman’s abdomen<br />

prior to the tra<strong>in</strong><strong>in</strong>g session. It turned out she had<br />

cancer that had spread to her liver. “We found out later<br />

that she died soon after we were there,” Stearns says.<br />

“But they were able to get her some help.”<br />

‘Long days’ but worth it<br />

The tra<strong>in</strong><strong>in</strong>g days were long. “We started early, at 6<br />

a.m.,” Whitten says. “And we didn’t return to our hotel<br />

until late <strong>in</strong> the even<strong>in</strong>g.”<br />

Short says the ACR was thrilled to have Whitten<br />

and Stearns. “They both were excited to attend<br />

and it wasn’t just their cl<strong>in</strong>ical experience, but their<br />

enthusiasm they brought to the endeavor that made<br />

it mean<strong>in</strong>gful for both the healthcare practitioners<br />

and patients and to our overall efforts on the ground<br />

there.”


Even though attendance was greater than expected<br />

everyth<strong>in</strong>g went smoothly. “It all went off without a<br />

hitch,” Short says.<br />

Two of the 12 delegates on the trip were from the<br />

American Society of Radiologic Technologists (ASRT),<br />

Jim Temme, MPA, RT(R)(QM), ASRT chairman of<br />

the board, and Kev<strong>in</strong> Powers, EdS, RT(R)(M), ASRT’s<br />

director of education.<br />

More trips likely<br />

An ACR delegation is plann<strong>in</strong>g on return<strong>in</strong>g to Haiti<br />

to provide more education and equipment. “We don’t<br />

know the details yet,” Short says. “But we are plann<strong>in</strong>g<br />

more trips. Our commitment is to not only improve<br />

radiology services <strong>in</strong> Haiti but to make it susta<strong>in</strong>able.<br />

We understand that it is go<strong>in</strong>g to take a long-term<br />

commitment and education is go<strong>in</strong>g to be part of that<br />

commitment.”<br />

Borgstede says that thanks to everyone’s <strong>in</strong>put, the<br />

education days were a tremendous success. “Now<br />

<strong>SDMS</strong> News Wave June 2012 4<br />

it’s time to assess what we achieved and decide<br />

where we go from here,” he says. Borgstede, who<br />

has volunteered <strong>in</strong> other impoverished countries,<br />

says Haiti has the advantage of proximity to the U.S.<br />

“You can get back and forth <strong>in</strong> a day, so it’s really an<br />

opportunity for us to assist <strong>in</strong> Haiti.”<br />

Whitten says the experience was so overwhelm<strong>in</strong>g<br />

that he found it very difficult to leave his newly made<br />

physician friends <strong>in</strong> Haiti and the delegates with<br />

whom he traveled. “I would love to go aga<strong>in</strong>,” he says.<br />

“If there was another trip next month, I would go. It<br />

was so worth it.”<br />

Stearns, too, would love to go back and teach <strong>in</strong> Haiti<br />

aga<strong>in</strong>, she says. “In a heartbeat.”<br />

Courtney Hartner recently<br />

graduated from St. Cloud<br />

State University with an<br />

A.A.S. Sonography degree.<br />

She is also <strong>in</strong> the process of<br />

earn<strong>in</strong>g her Bachelor’s degree<br />

<strong>in</strong> BioMedical Sciences.<br />

Throughout high school,<br />

Courtney excelled <strong>in</strong> math<br />

and science and knew she<br />

wanted to pursue a career <strong>in</strong><br />

the medical field. She tossed around the options of<br />

becom<strong>in</strong>g a psychiatrist or radiologic technologist.<br />

Although she loved help<strong>in</strong>g people, she thought<br />

<strong>SDMS</strong> News Wave June 2012 5<br />

psychiatry would not allow her to use her math skills<br />

as much as she wanted to and the thought of view<strong>in</strong>g<br />

broken bones through x-ray for the rest of her life<br />

was not appeal<strong>in</strong>g. Through more research, Courtney<br />

found diagnostic medical sonography and knew<br />

the field was a fit for her career goals. The aspect<br />

of sonography that <strong>in</strong>spired her the most was the<br />

detective work and solv<strong>in</strong>g puzzles. She was <strong>in</strong>trigued<br />

by the way cl<strong>in</strong>ical symptoms could be correlated<br />

with the f<strong>in</strong>d<strong>in</strong>gs of an ultrasound which eventually<br />

led to diagnos<strong>in</strong>g the patient.<br />

Courtney said her favorite <strong>SDMS</strong> benefit so far is the<br />

Journal of Diagnostic Medical Sonography (JDMS).<br />

The hard copy of the journal she receives <strong>in</strong> the mail<br />

allows her to cont<strong>in</strong>ue learn<strong>in</strong>g while be<strong>in</strong>g away<br />

from an electronic environment and school. Courtney<br />

has often times applied what she has read <strong>in</strong> the<br />

journal to her every day scann<strong>in</strong>g.<br />

Why did your facility make the commitment to<br />

employ/hire only credentialed sonographers?<br />

By committ<strong>in</strong>g to hir<strong>in</strong>g only credentialed<br />

sonographers, it shows our commitment to their<br />

professionalism and the will<strong>in</strong>gness to ma<strong>in</strong>ta<strong>in</strong> high<br />

standards of care for our patients. It also demonstrates<br />

a degree of competency for our sonographers. We<br />

were also look<strong>in</strong>g to the future with all of the healthcare<br />

costs and reimbursement issues surround<strong>in</strong>g<br />

us, it may be just a matter of time before our<br />

<strong>in</strong>surance companies require the sonographers<br />

to be credentialed <strong>in</strong> order to receive maximum<br />

reimbursement.<br />

credentialed sonographer” environment?<br />

Without a doubt, be<strong>in</strong>g credentialed requires every<br />

associate to ma<strong>in</strong>ta<strong>in</strong> their cont<strong>in</strong>u<strong>in</strong>g education<br />

credits. By do<strong>in</strong>g so, they are kept up-to-date <strong>in</strong><br />

their field of ultrasound with the latest and greatest<br />

techniques and procedures. Most all of them are<br />

also members of <strong>SDMS</strong>. Many times, with the use<br />

of <strong>in</strong>ternet and media, patients are look<strong>in</strong>g for the<br />

best care <strong>in</strong> their area, and by hav<strong>in</strong>g all credentialed


sonographers, they are confident the sonograms<br />

they are receiv<strong>in</strong>g at WFHC-All Sa<strong>in</strong>ts is the best <strong>in</strong><br />

the area.<br />

How have your patients benefitted from hav<strong>in</strong>g an<br />

“all credentialed sonographer” environment?<br />

We feel like the quality of care we give our<br />

patients is improved by hav<strong>in</strong>g all credentialed<br />

sonographers. Patients can be re-assured we are<br />

operat<strong>in</strong>g with high standards and that they are<br />

be<strong>in</strong>g scanned by professionals who take much<br />

pride <strong>in</strong> their work. Know<strong>in</strong>g all of this, patient<br />

satisfaction is higher as well.<br />
