CHECK YOUR CLASS SCHEDULE ON CHANGES IN TUITION AND<br />

MyUW REGISTRATION PROCESSES<br />

The rooms and buildings assigned to course sections<br />

may change between the time you register and the<br />

beginning <strong>of</strong> the quarter. Be sure to check MyUW<br />

before the quarter begins to check the location <strong>of</strong> the<br />

courses on your schedule.<br />

CHANGING YOUR GRADE OPTION TO<br />

SATISFACTORYINOT SATISFACTORY<br />

You may make changes to or from SINS grading<br />

through the. end <strong>of</strong> the 7th week <strong>of</strong> the quarter (May 19<br />

for spring 2002). No changes are accepted after this<br />

date. Be sure to check your program requirements to<br />

determine if a grade <strong>of</strong> Satisfactory can count toward<br />

your degree requirements. For undergraduate students,<br />

courses with this grade are only counted for general<br />

electives. Graduate students must check with their<br />

graduate program adviser or Supervisory Committee<br />

Chairperson before making this selection.<br />

When you select a course for this grading option, the<br />

system annotates your registration record. The instructor<br />

submits a regular decimal grade and the registration<br />

system converts the grade to S or NS when the transcript<br />

record is created for that quarter. The instructor is<br />

not notified that you have chosen this grading method.<br />

Undergraduates must earn a minimum grade <strong>of</strong> 2.0 to<br />

earn a grade <strong>of</strong> Satisfactory and graduates must earn a<br />

minimum grade <strong>of</strong> 2.7 to earn a Satisfactory grade.<br />

CreditINo Credit grading is not the same as SINS.<br />

Courses are <strong>of</strong>fered by the department as Credit/No<br />

Credit and decimal grades are not used in the grading<br />

process.<br />

TUITION IS DUE IN FULL BY THE TUITION DUE<br />

DATE OF APRIL 19, 2002 AND REGISTRA­<br />

TIONS ARE NO LONGER AUTOMATICALLY<br />

CANCELLED. Effective with winter 2002, your<br />

registration Is not automatically cancelled If you do<br />

not pay your tuition. If you do not plan to continue<br />

attending for the quarter, you must <strong>of</strong>ficially<br />

withdraw (drop all your courses). Failure to withdraw<br />

by the deadline (June 7) will result in failing<br />

grades for the quarter. You will continue to owe the<br />

full tuition plus additional penalties. In addition, the<br />

late payment fee for balances over $150 has been<br />

raised $120. Until you pay your outstanding<br />

balance In full, you will be prevented from registering<br />

for the next quarter and obtaining a copy <strong>of</strong><br />

your transcript. Check with Student Fiscal Services<br />

APPLICATION FOR GRADUATION<br />

IN JUNE<br />

The deadline to apply for spring quarter graduation<br />

is April 19, 2002. The annual Commencement<br />

Exercises are scheduled for June 15. Students<br />

who plan to graduate at the end <strong>of</strong> summer quarter<br />

must apply for graduation by April 19 also In order<br />

to participate In commencement this year. Information<br />

REGISTRATION<br />

Registration can be done from any location<br />

with Intemet access to the Web or from a<br />

touchtone phone. Computers are available in the<br />

various computer labs on campus and in Schmitz<br />

Hall. Registration via the Web is through the<br />

Registration option <strong>of</strong> Student Personal Services<br />

on MyUW. Phone registration is through STAR<br />

(Student Telephone AssIsted Registration). If you<br />

use STAR to register, please follow the directions<br />

on the STAR Worksheet on the page opposite.<br />

The registration system is available all<br />

weekdays and most holidays and weekends.<br />

Weekend hours not specifically scheduled are not<br />

guaranteed and may be subject to equipment and<br />

s<strong>of</strong>tware maintenance requirements. Weekday<br />

hours are 6:00 a.m. to 12:00 midnight. Weekend<br />

hours are 9:00 a.m. to 12:00 midnight on<br />

Saturdays and 10:00 a.m. to 12:00 midnight on<br />

Sundays.<br />

SPECIAL FEATURES<br />

ADD AND DROP CODES<br />

Some courses require a special five..digit<br />

code that is avatlable through the department<br />

<strong>of</strong>fering the course. See ENTRY CODES and<br />

FACULTY NUMBERS on page 9 for more<br />

inlonnatlon on using these codes.<br />

ADDING AND DROPPING COURSES<br />

Courses may be added during Periods I,ll,<br />

III, and the Late Add Period. Beginning with the<br />

Late Add Period on April 8, an add code is<br />

required to add any course and a $20 Change <strong>of</strong><br />

Registration Service lee Is charged. This Is In<br />

addition to any tuition. Always check your balance<br />

alter adding a course. Sunday, April 21 (until<br />

midnight). Is the last day to add a course for spring<br />

quarter 2002. .<br />

You have unlimited drops through April 14<br />

without entry on your transcript. The registration<br />

system closes at 6:00 p.m. on Friday. April 12, and<br />

remains closed on Saturday, April 13. Beginning<br />

AprU 8. a Change <strong>of</strong> Registration Service fee is<br />

charged to drop a course. This is in addition to<br />

any tuition forfeiture. Always check your balance<br />

alter dropping a course.<br />

If you use your annual drop that is available<br />

for that academic year, a Change <strong>of</strong> Registration<br />

Service fee will be charged.<br />

CHANGING YOUR INSURANCE PLAN<br />

AND OmONAL CHARGES<br />

II you wish to change your insurance plan<br />

you may do so on MyUWor STAR, but no later<br />

than April 7. Optional charges for WashPIRG and<br />

Affordable Tutlon Nowl may be changed on MyUW<br />

or STAR after your Inillal selection through the<br />

tuition due date. II you make the changes by<br />

STAR, please reler to the STAR Worksheet in this<br />

TIme Schedule for Instructions.<br />

CLOSED COURSE SECTIONS<br />

II the section requested is closed, you will be<br />

inlormed by the registration system <strong>of</strong> other open<br />

sections (including new sections added after the<br />

publication <strong>of</strong> the quarterly TIme Schedule). You<br />

may then select an altemate section to be added<br />

to your schedule. To overload a closed section<br />

see OVERLOAD GUIDELINES on page 9.<br />

CONFIRMED REGISTRATION<br />

You may confirm your registration schedule<br />

through MyUW or by calling STAR and using the<br />

List Schedule transaction (L#). This listing<br />

includes building and classroom locations.<br />

COURSE AVAILABILITY<br />

Check the TIme Schedule listings on the<br />

Web to determine availability <strong>of</strong> spaces In<br />

particular course sections, to help plan your<br />

registration.<br />

PREREQUISITES<br />

Certain courses require that you have<br />

prerequisites in order to add that course to your<br />

schedule. These courses are identified by a P<br />

symbol in the course listing section <strong>of</strong> the TIme<br />

Schedule. Prerequisites may be one or more<br />

courses, a minimum test score. or a minimum<br />

grsde in a prerequisite course. You may add a<br />

course section even if you have not completed the<br />

course prerequisite provided you are currently<br />

registered for the prerequisite course. However,<br />

departments may elect to have the course that<br />

required the prerequisite dropped from your<br />

schedule If you do not satisfactorily complete the<br />

prerequisite course. Courses subject to<br />

cancellation are identified by the PX symbol in the<br />

Time Schedule. If you do not satisfactorily<br />

complete the prerequisite course, the course that<br />

required the prerequisite will be dropped from your<br />

schedule no later than the third calendar day <strong>of</strong> the<br />

quarter.<br />

PRIVATE ACCESS CODE (PAC)<br />

If you wish to make transactions via STAR,<br />

you must have selected your Private Access<br />

Code (PAC). If you have been admitted lor \he<br />

current quarter, or if you have never changed<br />

your PAC from the one that was sent on your<br />

Registration Notice, then you will need to select a<br />

new PAC when you call STAR. Although you<br />

may continue to use the initial PAC to make<br />

transactions on MyUW, you will have additional<br />

security for your education records if you select a<br />

new PAC known only to you. .<br />

The first time you ever call STAR, enter<br />

your Inilial Private Access Code (which is<br />

provided in your Registration Notice) when STAR<br />

asks you to enter a Private Access Code. STAR<br />

will then prompt you to select a new Private<br />

Access Code or PAC <strong>of</strong> 4 to 6 digits that you can<br />

easily remember. STAR will not accept zero as<br />

the first number In your PAC, nor will it accept<br />

consecutive or repetitive numbers. Enter the<br />

PAC you select when prompted by STAR. You<br />

will then use your PAC for all future calls to<br />

STAR. If you wish to change your PAC, you may<br />

do so at any time using transaction code 9.<br />

Please refer to the STAR Worksheet in this TIme<br />

Schedule for instructions. If you forget your<br />

p,AC, you will need to contact the Registration<br />

Office, 225 Schmitz Hall, or cali (206) 543-8580<br />

to reset your PAC.<br />

FOLLOWING RULES AND REGULA­<br />

TIONS<br />

It is the <strong>University</strong>'s expectation that you will<br />

follow <strong>University</strong> rules and regulations as they are<br />

stated in the General Catalog and the Time<br />

Schedule. In Instances where no appeal<br />

procedure Is spelled out and you believe that a<br />

special set 01 circumstances makes appeal<br />

reasonable, you may appeal the application <strong>of</strong><br />

specific rules or regulations to the <strong>of</strong>fice <strong>of</strong> the<br />

dean <strong>of</strong> the school or college in which you are<br />

enrolled in the case <strong>of</strong> an academic matter, or to<br />

the Office <strong>of</strong> the Vice President for Student Affairs<br />

in the case <strong>of</strong> a nonacademic matter. These<br />

<strong>of</strong>fices will either render a decision on the appeal<br />

or refer you to the proper <strong>of</strong>fice for a decision.<br />

GRADUATION RATES<br />

The graduation rates <strong>of</strong> full-time freshmen<br />

beginning their studies at the <strong>University</strong> <strong>of</strong><br />

<strong>Washington</strong> either summer or autumn quarter<br />

1994 are 63.2% in 5 years and 68.7"", In 6 years.<br />

IMMUNIZATION<br />

All new and retumlng former students, and<br />

all students living In <strong>University</strong> <strong>of</strong> <strong>Washington</strong><br />

residence halls or sIngle-student apartmenls, are<br />

required either to provide pro<strong>of</strong> <strong>of</strong> measles<br />

(rubeola) Immunity or request that the requirement<br />

be waived for religious, personal, or medical<br />

reasons. The Measles Immunity Verification form<br />

is malled with the <strong>of</strong>fer <strong>of</strong> admission or registration<br />

notice to all new and appHcable retuming<br />

students. You will not be allowed to register for<br />

any classes until the Hall Health Primary Care<br />

Center (the student health center on campus)<br />

confirms that you have satisfied this requirement.<br />

You must complete the student health<br />

center's Measles Immunity Verification Form and<br />

provide either certifications by your heaJth-care<br />

provider or copies <strong>of</strong> original immunization records<br />

as pro<strong>of</strong> <strong>of</strong>: (1) two live virus measles (rubeola)<br />

vaccinations, both given alter 1967 with at least<br />

one given alter 1980; or (2) positive measles<br />

(rubeola) titer (blood test for antibodies against<br />

measles); or (3) a history from a health-care<br />

provider <strong>of</strong> measles (rubeola) disease. Students<br />

bom before 1957 are considered Immune to<br />

measles.<br />

While continuing students are not required<br />

to provide the Hall Health Primary Care Center<br />

with pro<strong>of</strong> <strong>of</strong> measles Immunity, they are<br />

encouraged to do so. In the event <strong>of</strong> an outbreak<br />

on the campus, all students lacking acceptable<br />

documentation <strong>of</strong> measles immunity, including<br />

students with waivers, will be barred from classes,<br />

living groups, and other campus activities until<br />

health <strong>of</strong>ficIals determine that there is no risk <strong>of</strong><br />

them transmitting the disease to others. Measles<br />

ImmunIty Verillcatlon Forms are available at the<br />

Hall Health Primary Care Center and the<br />

Registration Office at 225 Schmitz Hall. For more<br />

information, students should contact the Hall<br />

Health Primary Care Center at (206) 616-4672<br />

and leave volcemail messages to which members<br />

<strong>of</strong> the nursing staff will respond.<br />

RELEASE OF STUDENT DIRECTORY<br />

INFORMATION<br />

The Family Educational Rights and Privacy<br />

Act (FERPA) <strong>of</strong> 1974 protects the privacy <strong>of</strong> your<br />

education records. As a general rule the<br />

UnIversIty will not release a student's education<br />

records to a third party without written consent <strong>of</strong><br />

the student. This includes tuition account<br />

information. The complete <strong>University</strong> policy on<br />

student education records and the tocation <strong>of</strong> such<br />

records may be found in the <strong>Washington</strong><br />

Administrative Code under WAC 478-140.<br />

Manuats containing the Code are available at<br />

Reference Stations on campus and at the Visitor's<br />

Information Center, 4014 <strong>University</strong> Way NE.<br />

Informallon release forms for tuition account<br />

information only may be obtained from Student<br />

Fiscal Servlcas, 129 Schmitz.<br />

The following information is considered<br />

public or "directory" information and may be<br />

released to anyone unless you inform the Office <strong>of</strong><br />

the Registrar that you do not wish any information<br />

released: name, address, telephone number,<br />

email address, major field <strong>of</strong> studies, dates <strong>of</strong><br />

attendance, full- or part-lime enrollment status,<br />

mosl recent previous educationallnstltullon<br />

attended, degrees and awards received, as well as<br />

notices <strong>of</strong> awards or academic honors to home<br />

town newspapers, and for athletes, date <strong>of</strong> birth,<br />

weight, and height.<br />

If you do not wish to authorize directory<br />

release and do not want your directory information<br />

to appear in the published or electronic Student<br />

Directory you may restrict the release <strong>of</strong> this<br />

inlormallon usIng the Change <strong>of</strong> Address option in<br />

the Student Personal Services section <strong>of</strong> MyUW or<br />

by calling the address change service at (206)<br />

543-3868. No information will be released on<br />

students or to students, who have restricted<br />

release <strong>of</strong> directory information. If you wish to<br />

change your authorization and allow release, you<br />

must go to the Registration Office, 225 Schmitz<br />

and present photo Identification or make the<br />

change via MyUW.<br />

Under the provisions <strong>of</strong> FERPA, you have<br />

the right to:<br />

1_ Inspect and review information contained in<br />

your education records within 45 days <strong>of</strong> the day<br />

the <strong>University</strong> receives a request for access. You<br />

should submit to the registrar, dean, chair <strong>of</strong> the<br />

academic department, or other appropriate <strong>of</strong>ficial,<br />

written requests that identify the record(s) you<br />

wish to inspect. The <strong>University</strong> <strong>of</strong>ficial will make<br />

arrangements for access and notify you <strong>of</strong> the time<br />

and place where the records may be Inspected. If<br />

the records are not maintained by the <strong>University</strong><br />

<strong>of</strong>ficial to whom the request was submitted, Ihat<br />

<strong>of</strong>ficial shall advise you <strong>of</strong> the correct <strong>of</strong>ficial to<br />

whom the request should be addressed.<br />

2. Request the amendment <strong>of</strong> your education<br />

records that you believe are inaccurate or<br />

misleading. You should write the <strong>University</strong> <strong>of</strong>ficial<br />

responsible for the record, cleariy Identify the part<br />

<strong>of</strong> the record you want changed, and specify why it<br />

is inaccurate or misleading. If the UnIversity<br />

decides not to amend the record as requested, the<br />

<strong>University</strong> will notify you 6f the decision and advise<br />

you <strong>of</strong> your right to a hearing.<br />

3. Consent to disclosures <strong>of</strong> personally<br />

identifiable information contained in your<br />

education records, except to the extent that<br />

FERPA authorizes disclosure without consent.<br />

One exception, which permits disclosure without<br />

consent. Is disclosure to school <strong>of</strong>ficIals with<br />

legitimate educational interests. A school <strong>of</strong>ficial<br />

is a person employed by the UniversIty In an<br />

administrative, supervisory, and academic or<br />

research, or support slaff position (including law<br />

enforcement unit personnel and health staff); a<br />

person or company with whom the <strong>University</strong><br />

has contracted (such as an attorney, audnor, or<br />

collection agent); a person serving on the Board<br />

<strong>of</strong> Trustees; or a student serving on an <strong>of</strong>ficial<br />

commIttee, such as a disciplinary or grievance<br />

committee, or aSSisting another school <strong>of</strong>ficial in<br />

performIng his or her tasks. A school <strong>of</strong>ficial has<br />

a legitimate educational interest if the <strong>of</strong>ficial<br />

needs to review an education record in order to<br />

fulfill his or her pr<strong>of</strong>essional responsibility.<br />

4. File a complaint with the Department <strong>of</strong><br />

Education conceming alleged failures by the<br />

<strong>University</strong> <strong>of</strong> <strong>Washington</strong> to comply with the<br />

requirements <strong>of</strong> FERPA. Written complaints<br />

SEXUAL HARASSMENT GRIEVANCE<br />

PROCEDURE<br />

Students, as well as members <strong>of</strong> the<br />

faculty and staff, who wish to file a complaint<br />

regarding sexual harassment may contact either<br />

programs, including many high-demand courses that are difficult to get into<br />

during other quarters. Shorter four-and-a-half-week courses are available to<br />

accommodate students with summer jobs.<br />

Get the classes you need<br />

Class size is typically smaller during Summer Quarter, and with fewer students registering, you're<br />

more likely to get the classes you need. Summer enrollment is especially appropriate for seniors<br />

who lack credits for graduation, for juniors planning to graduate in June 2003, and for incoming<br />

freshmen who want to get a head start on their studies. (If you can't attend summer classes, you<br />

can earn credits through UW Distance Learning.)<br />

A special time to laam<br />

More than 1,600 summer courses are <strong>of</strong>fered, including many classes and workshops scheduled<br />

only in the summer. Some classes open only to majors in other quarters may be taken by<br />

nonmajors in the summer. You can earn up to a year's language credit in nine weeks by completing<br />

an intensive summer language course.<br />
