junto con una i<strong>de</strong>ntificación oficial con fotografía.<br />
Deberás llamarnos <strong>de</strong> inmediato para reportar tu ingreso al hospital<br />
al teléfono 54-48-67-11 <strong>de</strong> la Ciudad <strong>de</strong> México o bien al 01 800 5227 696<br />
sin costo <strong>de</strong>l Interior <strong>de</strong> la República Méxicana.<br />
Cuando se realice tu reporte te proporcionaremos un número <strong>de</strong><br />
i<strong>de</strong>ntificación para tu trámite, el cuál será indispensable lo conserves<br />
por si se requiere mayor nformación acerca <strong>de</strong> tu tratamiento.<br />
El personal <strong>de</strong> Seguros Monterrey New York Life revisará la información<br />
para su dictamen y te informará si el evento está o no cubierto.
