This booklet provides a list of Amerigroup network pharmacies. All network pharmacies may
not be listed in this directory. Pharmacies may have been added or removed from the list after
this directory was printed. To get current information about Amerigroup network pharmacies in
your area, please visit our Web site at www.myamerigroupcorp.com/medicare or call our
Member Services at 1-866-805-4589, 8:00 a.m. to 8:00 p.m. local time, 7 days a week. (TTY
Users should call 1-800-855-2880.)
Beneficiaries must use network pharmacies to access their prescription drug benefit. Benefits,
formulary, pharmacy network, premium and/or copayments/coinsurance may change on
January 1, 2013.
Amerigroup is an HMO plan with a Medicare contract.
