La toxicité hématologique • LA THROMBOPENIE • > 25 000 plaquettes, pas de saignements : surveillance ( Exacyl en prévention?) • < 25 000 plaquettes , saignements ou état fébrile associé : concentrés plaquettaires, Exacyl ou autre, (facteurs de croissance en attente) • nadir retardé par rapport à celui de la leucopénie
La toxicité hématologique • L'ANEMIE • - le chiffre limite : Hb = 8 g ---- > transfusions - des transfusions sont légitimes à un taux d'Hb supérieur si sujet fatigué, âgé, tolérant mal l'anémie (insuffisant respiratoire notamment ou cardiaque). • - la chimiothérapie serait plus efficace chez un patient non anémique • - facteurs de croissance disponibles : les EPO • LA LYMPHOPENIE • risque infectieux (si < à 200 ou 400 / mm3) mycosique, viral ou parasitaire • long délai de récupération (10 à 12 mois après la CT)
