L’IRM <strong>de</strong> perfusion et <strong>la</strong> mise en évi<strong>de</strong>nce <strong>de</strong> zones <strong>de</strong><br />
<strong>rehaussement</strong> <strong>tardif</strong> du signal sur <strong>de</strong>s acquisitions<br />
pondérées en T1 après injection <strong>de</strong> sels <strong>de</strong> gadolinium<br />
en font un outil <strong>de</strong> choix l’évaluation <strong>de</strong> <strong>la</strong> viabilité . Le<br />
<strong>de</strong>gré d’extension transmurale <strong>de</strong> l’hypersignal<br />
correspondant à <strong>la</strong> nécrose semble corrélé aux<br />
possibilités <strong>de</strong> récupération fonctionnelle après<br />
revascu<strong>la</strong>risation.<br />
Cette étu<strong>de</strong> prospective a com<strong>par</strong>é l’efficacité<br />
diagnostique <strong>de</strong> 2 séquences optimisées pour<br />
l’évaluation <strong>de</strong> <strong>la</strong> viabilité <strong>myocardique</strong> : IR<br />
TurboFLASH segmentée 3D (A) ,reconnue comme <strong>la</strong><br />
séquence <strong>de</strong> référence, et contraste <strong>de</strong> phase IR<br />
TrueFISP 2 D (B).
