Untuk keperluan penyampaian konfirmasi transaksi efek dari SINARMAS kepada Pemohon, maka apabila Pemohon tidak mempunyai mesinfaksimili atau mesin faksimili Pemohon tidak dapat dipergunakan atau tidak mempunyai alamat e-mail, maka kantor pusat SINARMAS atau kantorcabang tempat Pemohon membuka rekening untuk diambil sendiri oleh Pemohon;For the purpose of informing securities transaction confirmation from SINARMAS to Applicant, then if the Applicant does not have the faximilemachine or the faximile machine of Application does not work or does not have e-mail address, then Applicant will submit request in writing in orderthat securities transaction confirmation in the name of Applicant can be sent to the head office of SINARMAS or branch office where the Applicantopen an account to be picked-up by the Applicant itself;9. Pemohon menyatakan bahwa dengan ini mengetahui dan menyetujui Formulir Pembukaan Rekening Efek individu ini merupakan satu kesatuanyang tidak terpisahkan dengan ketentuan Transaksi Efek (terlampir);Applicant states that hereby know and agrees that this individual Securities Account Opening Form constitutes an insperable part of SecuritiesTransaction Provision (enclosed);10.Pemohon menjamin bahwa dana ataupun Efek yang dipergunakan dalam rangka melakukan transaksi bukan dana ataupun Efek yang berasal daritindak pidana apapun juga sesuai dengan peraturan perundang-undangan yang berlaku di Indonesia;Applicant secures that the fund or Securities used in the framework to the make transaction is not the fund or Securities orginating from any criminalact in accordance with the prevailing statutory regulation in Indonesia.11.12.Bahwa Pemohon menjamin pembukaan rekening Efek ini tidak dimaksudkan dan atau ditujukan dalam rangka upaya melakukan tindak pidanapencucian uang;That Applicant secures this Securities account opening is not itended and or designated in the framework to carry out criminal act of moneylaundering;Bahwa Pemohon menjamin transaksi tidak dilakukan untuk maksud mengelabui, mengaburkan atau menghindari pelaporan kepada PPATKberdasarkan ketentuan yang berlaku;That Applicant secure the transaction is not made for the purpose to fade, vague or avoid reporting to PPATK based on the preaviling provision;13.Bahwa Pemohon bertanggung jawab sepenuhnya apabila pemohon melakukan tindak pidana pencucian uang yang dilakukan diluar pengetahuanSINARMAS;That Applicant takes responsible fully when applicant carries out criminal act of money laundering made without knowing the SINARMAS14. Bahwa pemohon bersedia untuk membayar denda sebesar 0.125% (nol koma seratus dua puluh lima persen) per hari dari seluruh jumlah kewajibanatas keterlambatan penyelesaian transaksi terhitung sejak tanggal jatuh tempo;That Applicant is prepared to pay penalty amounting to 0.125% (nul point one hundred and twenty five percent) per day of all obligation on the delayof transaction settlement commencing from due date;15.Dalam hal Pemohon telah atau akan membuka Rekening Efek pada perusahaan Efek lain, maka Pemohon menjamin bahwa data identitas diri (nama,tempat dan tanggal lahir) yang dipergunakan adalah sama;In case Applicant has or will open Securities Account to other Securities Company then Applicant secures that data of self identity (name,place anddate of birth) used are the same;16. Apabila di kemudian hari diketahui Pemohon menyampaikan informasi/data atau pernyataan yang tidak benar, maka SINARMAS berwenang untukmengambil tindakan tertentu termasuk menutup Rekening Efek milik Pemohon.Should in the future it is known that applicant submit incorrect information/data or statement, then SINARMAS is authorized to take specific actionincluding to close Securities Account of the Applicant17.
