Ejaculatory dysfunction (both reduction and pain) and libido are related to
LUTS
Sexual dysfunction is related to peak urinary flow and to the duration of LUTS
Age, comorbidities and concomitant therapy interfere with the assessment of the
impact of medical therapy on sexuality.
Placebo and nocebo effects interfere with the assessment of the impact of medical
therapy on sexuality.
The impact of α 1 -adrenoreceptor blockade on ejaculation differs according to the
compound used
The rate of retrograde ejaculation is highest with Tamsulosin. However, evaluated
with the nowadays available tools, it does not appear to be clinically significant
The treatment of LUTS/BPH with α 1 -adrenoreceptor blockers improves global
sexual function
5α-reductase inhibitors are associated with alterations of libido, erection and
ejaculation, mainly during the first year of treatment. However, the alterations do
not appear to be clinically significant
Ejaculatory dysfunction is frequently associated with surgery for LUTS/IPB (the
rate is significantly lower with TUIP)
Erectile dysfunction is rarely associated with surgery for LUTS/BPH, which on
the contrary could improve the disorder
All aspects of sexual function should be taken into consideration and assessed in
the presence of severe and/or long-standing LUTS/BPH
The side effects of medical therapy on sexual function should be discussed with
young and/or motivated patients
The possible effects of surgery on sexuality should be discussed with all patients
