costante (29): infatti studi in vivo
condotti su donne a termine
(≥ 37 settimane di gestazione)
hanno evidenziato che a membrane
integre è presente una correlazione
lineare tra quantita di
P G E 2 rilasciata e durata del trattamento
(Figura 4) (26).
In caso di PROM la quantità di
P G E 2 rilasciata è maggiore a
causa della variazione del pH
vaginale (PROM = 6 vs membrane
integre = 4) e non è stato
possibile rilevare la correlazione
lineare prima descritta (26).
Tuttavia nel range di pH vaginale
tra 6,5 e 7,5 la PGE 2 si trova
principalmente in forma ionizzata,
condizione che ostacola il suo passaggio
nel circolo materno: ciò spiega perché in
caso di PROM la concentrazione di PGE 2 e
dei suoi metaboliti nel plasma materno non
differisca da quanto osservato in caso di
membrane integre, con conseguente mancato
incremento del rischio di iperstimolazione
del miometrio (26).
Da ultimo una nota tecnica rilevabile dai
riassunti delle caratteristiche del prodotti a
base di dinoprostone: mentre il gel, in tutti i
suoi dosaggi, è controindicato nella PROM
(30), per il dispositivo vaginale è raccomandata
cautela (29).
Per un uso corretto dei farmaci si raccomanda
comunque un’attenta lettura dei rispettivi
R.C.P. (29, 30).
