Ferraccioli et al. que è possibile molto semplicemente individuare i pazienti che non hanno attività di malattia residua, che non progrediscono dal punto di vista strutturale e che saranno in remissione US-Doppler. Questo fornisce delle linee guida molto semplici per l’attività clinica quotidiana del reumatologo. L’analisi delle coorti ha altresì consentito di documentare che il 66.7% dei pazienti ERA in remissione ACR vs il 26.1% (p=0.0001) dei pazienti AR long standing sempre in remissione ACR, erano anche in remissione eco-doppler, a dimostrazione che neppure lo stringente criterio ACR consente di individuare i pazienti in persistente remissione Eco-Doppler ma che la remissione ACR nell’ERA identifica i 2/3 dei pazienti in remissione US. Un altro dato di grande interesse è che nella coorte ERA, gli elevati livelli basali di anti-CCP2 sono risultati insieme con l’età dei pazienti gli unici predittori di futura remissione. Questo è un dato biologico che è stato di recente analizzato e confermato su tre coorti di differente background (giapponese, scandinava ed americana) (9). CONCLUSIONI In conclusione la RMDA sembra essere il target oggettivamente più utile per definire un controllo clinico adeguato, un livello di malattia residua che non fa progredire il danno strutturale, l’obiettivo reale della terapia che può essere tarata nel mondo reale, e che consente un elevatissimo livello di remissione funzionale.
