30-45mg/kg/<strong>do</strong>se E.V 8/8h E.V (herpes zooster)<br />
200mg V.O 5x/dia herpes simples mucotâneo<br />
Áci<strong>do</strong> pipemídico 800mg/dia - 12/12h V.O ou c/ 24h<br />
Amicacina 15mg/kg/dia 1,5g/dia c/ 24h<br />
*Fibrose Cística infusão<br />
30-40mg/kg/dia 60 minutos ou IM 1x /dia<br />
Amoxicilina 1.500mg/dia 3g/dia 8/8h V.O ou 12/12h<br />
Amoxacilina BD 875mg 1750mg 12/12h V.O<br />
Amoxacilina ES 1.750mg 3g/dia 8/8h ou 12/12h<br />
