ABSTRACT “Clinical and laboratorial characterization of a Rheumatoid Arthritis patients’ cohort” Objective: To characterize, of the clinical and laboratorial point of view, a Rheumatoid Arthritis patients’ cohort and to assess the effects of adverse prognostic factors in the evolution of the disease in this sample. Methods: In this retrospective observacional study, the clinical processes of a patients’ sample, with the Rheumatoid Arthritis diagnosis, according to ACR classification criteria, modified in 1987, followed in the Rheumatology Service of the Centro Hospitalar da Cova da Beira, were consulted. This cohort’s selection was made, based on registry of the GDH code - 7140 in the informatic system Sistema de Apoio ao Médico®, at the moment of the study. Later it was proceeded the analysis of the relation between the investigated parameters through the informatic program SPSS, version 17.0. Results: Thirty nine patients with the Rheumatoid Arthritis diagnosis were included in this study, of whom 27 were women (69.2%) and 12 were men (30.8%). The patients’ age varied between 30 and 81 years (median age of 59,64 and standard deviation of 12, 98 years). RA test was positive in 25 patients (64.1%) and Waaler-Rose test in 20 (53.1%). Nineteen patients (51.3%) were Anti-cyclic citrullinated peptide positive and 13 were negative (33.3%). The majority of the patients presented a moderate disease activity level determined with Disease Activity Score 28 (41.0%). Seven patients were in remission (17.9%). High and low activity levels were found in equal percentages of patients (20.5%). The most frequent extra-articular manifestations found were fatigue (59%), keratoconjunctivitis sicca (43.6%), imagiologic signs suggestive of pulmonary fibrosis (38.5%) and anemia (35.9%). Conclusion: In this study, the number of years of the disease evolution was main the predictor of the structural damage. These data come to highlight the xii
importance of the precocious diagnosis and referencing of the Rheumatoid Arthritis patients Among the several adverse prognostic factors cited in the literature and investigated in this coorte, the value of the C-reactive protein was the only associated with the disease activity level.
