MEDICATIA ADMINISTRATA<br />

IN TIMPUL SARCINII SI ALAPTARII<br />

Agentia Americana pentru Controlul Alimentelor <strong>si</strong> Medicamentelor (FDA)<br />

cla<strong>si</strong>fica medicamentele <strong>in</strong> functie de <strong>si</strong>guranta adm<strong>in</strong>istrarii lor <strong>in</strong> timpul<br />

sarc<strong>in</strong>ii <strong>in</strong> c<strong>in</strong>ci categorii:<br />

- categoria A – studii controlate pe subiectii umani care nu au demonstrat<br />

existenta vreunui risc pentru fat – cele mai <strong>si</strong>gure medicamente<br />

- categoria B – studii pe animale care demonstreaza po<strong>si</strong>bilitatea<br />

existentei unui anumit grad de risc fetal<br />

- categoria C – nu exista studii pe animale <strong>si</strong> subiecti umani care sa<br />

demonstreze efectele negative ale acestora<br />

- categoria D – exista dovezi clare ca sunt riscante <strong>in</strong> sarc<strong>in</strong>a, pentru<br />

efectele negative fetale (se vor folo<strong>si</strong> numai <strong>in</strong> <strong>si</strong>tuatii vitale sau afectiuni<br />

severe, <strong>in</strong> care beneficiile depasesc riscurile adm<strong>in</strong>istrarii)<br />

- categoria X – risc fetal dovedit, care nu justifica adm<strong>in</strong>istrarea <strong>in</strong>diferent<br />

de beneficii<br />

Medicamentele adm<strong>in</strong>istrate <strong>in</strong> sarc<strong>in</strong>a actioneaza asupra produsului de<br />

conceptie pr<strong>in</strong> multiple mecanisme:<br />

- direct asupra embrionului, rezultand un efect toxic, teratogenic<br />

- vasoconstrictie la nivelul placentei, alterand schimburile de oxigen<br />

<strong>si</strong> substante nutritive mama-fat<br />

- produc hipertonie uter<strong>in</strong>a cu anoxie fetala consecutiva<br />

- altereaza d<strong>in</strong>amica biochimica a mamei, afectand <strong>in</strong>direct fatul.<br />

Severitatea afectarii fatului dep<strong>in</strong>de de varsta acestuia, potenta medicamentului<br />

<strong>si</strong> doza adm<strong>in</strong>istrata.<br />

Adm<strong>in</strong>istrarea medicamentelor <strong>in</strong> faza embrionara sau zigotica (<strong>in</strong>a<strong>in</strong>tea<br />

zilei 20 postconceptie) fie omoara embrionul, fie nu il afecteaza deloc.<br />

Perioada organogenezei (3-8 saptamani postconceptie) este critica pentru<br />

teratogeneza. Adm<strong>in</strong>istrarea medicamentelor <strong>in</strong> aceasta perioada se<br />

poate solda cu: avort, malformatii neletale, defecte metabolice sau functionale<br />

care se vor manifesta ulterior <strong>in</strong> viata.<br />

Dupa <strong>in</strong>cheierea organogenezei (trim. 2,3) medicamentele par a nu mai<br />

afecta teratogenic dar pot afecta cresterea <strong>si</strong> functionalitatea organelor<br />

fetale.<br />

Factorii care <strong>in</strong>fluenteaza trecerea medicamentelor <strong>in</strong> laptele matern sunt:<br />

- factori materni (metabolismul, doza <strong>si</strong> durata terapiei, rata adm<strong>in</strong>istrarii)<br />

- factori <strong>in</strong>fantili (varsta, absorbtia medicamentelor, metabolismul,<br />

timpul alaptarii)<br />

- factorii medicamento<strong>si</strong> (solubilitatea lipidica, legarea de prote<strong>in</strong>e,<br />

biodisponibilitatea).<br />
