Biventrikulär pacing som generell behandlingsmetod<br />

vid svår hjärtsvikt kan dock inte<br />

anses vara en rekommenderad metod, utan<br />

snarare en tilläggsbehandling för en selekterad<br />

patientgrupp. Ytterligare förfining av<br />

urvalskriterier behövs för att öka andelen<br />

patienter med positiv effekt av biventrikulär<br />

pacing. Vilka patienter som skall ha kombinerad<br />

behandling med ICD och biventrikulär<br />

pacing återstår också att definiera.<br />

Tydliga resultat, med minskad sjukvårdkonsumtion<br />

för hjärtsviktsbehandling efter<br />

inledande av biventrikulär pacemakerbehandling,<br />

visar på betydande samhällsekonomiska<br />

vinster. Införande av behandlingsmetoden<br />

ställer dock stora krav på teknisk<br />

utrustning och skicklighet hos implantatören.<br />

Tillsvidare bör behandlingsmetoden<br />

vara begränsad till center med stor erfarenhet<br />

av pacing, utöver detta bör patienter som<br />

erhåller biventrikulär pacemakerbehandling<br />

ingå i kliniska studier för fortsatt dokumentation<br />

av behandlingsmetoden. ■<br />
