Prophylaxe: ständiger verbaler Kontakt mit dem Patienten
Intoxikation mit Lokalanästhetika
1. Zentralnervensystem:
Unwohlsein, metallischer Geschmack,
Benommenheit, Schwindel, Ohrensausen;
Desorientiertheit, Schläfrigkeit, Bewusstlosigkeit;
Nystagmus, Konvulsionen
Sofort LA-Zufuhr stoppen!
Hyperventilation mit hohem O2-Anteil, Sedierung, ggf. Antikonvulsiva
2. Herz-Kreislauf-System:
Bradyarrhythmie, QRS-Verbreiterung, Blockbilder;
negative Inotropie, direkte / indirekte Vasodilatiation;
Asystolie oder Kammerflimmern („stone heart“)
kardiopulmonale Reanimation,
ggf. Herz-Lungen-Maschine

Intralipid R als Antagonist ?

In cardiac arrest secondary to local anaesthetic toxicity, which is
unresponsive to standard therapy, intravenous administration of a lipid
such as Intralipid R is recommended in the following regimen:
1. give 1 ml/kg over 1 min,
2. repeat twice more at 3-5 min intervals,
3. then (or sooner if stability is restored), convert to an infusion at a rate
of 0.25 ml/kg/min, continuing until haemodynamic stability is restored,
4. increasing the dose beyond 8 ml/kg is unlikely to be useful
5. in practice, in resuscitating an adult weighing 70 kg:
take a 500-ml bag of Intralipid R 20% and a 50-ml syringe,
draw up 50 ml and give it stat intravenously, and then draw up and give another 20 ml,
do exactly the same thing up to twice more as you gave epinephrine – if necessary or
appropriate,
then attach the Intralipid bag to a giving set and run it intravenously over the next 15 min.
