Description: Disability Day Programs: The Junction Works Disability<br />
Day Programs aim to develop skills for independence,<br />
recreation, social inclusion, and employment for adults with<br />
a disability. We provide support to Community Participation<br />
(CP) and Transition to Work (TTW) clients.<br />
Disability Case Management and Brokerage Program:<br />
Case managers will provide links to existing services,<br />
arrange the purchase of some brokerage services and<br />
assist people to develop and maintain links in the<br />
community. Priority Access is given to people who have an<br />
intellectual disability, have no other agencies involved, are<br />
at risk of entering the Juvenile Justice system, having<br />
ageing carers, and are at risk of becoming homeless.<br />
Disability Assistance Packages: Provide access to<br />
respite services for older carers over 60 and Indigenous<br />
carers over 45, who provide primary care for a person with<br />
a disability. DAP referrals are received through ADHC and<br />
allocated annually. The package is for one year only. Other<br />
community services can also broker respite support from<br />
the Junction works DAP program.<br />
Young People Leaving Care Program: Supports young<br />
people with an intellectual disability, aged 15years and<br />
over, who are in the care of Children’s Court for 12months<br />
or more and are requiring continued support through ADHC<br />
services. The Program Provides Case Management in Daily<br />
In-Home Support. Referrals are received through ADHC.<br />
Sibling Support Program: Provides support networks to<br />
young people aged between 8 to 18 years, who have a<br />
sibling with a disability. The main goal for the Sibling<br />
Support program is to support siblings to identify and use<br />
positive strategies for living with a brother(s) or sister(s)<br />
with a disability.<br />
