This recommendati<strong>on</strong> places a relatively high value <strong>on</strong> avoiding the uncertain adverse effects of<br />

butterbur, <str<strong>on</strong>g>and</str<strong>on</strong>g> a relatively low value <strong>on</strong> equally uncertain reducti<strong>on</strong> in symptoms of rhinitis.<br />

Remarks<br />

In patients who are less risk averse an alternative may be equally reas<strong>on</strong>able. However, if <strong>on</strong>e<br />

chooses to use butterbur <strong>on</strong>e should c<strong>on</strong>sider <strong>on</strong>ly commercial preparati<strong>on</strong>s in which butterbur<br />

extract does not c<strong>on</strong>tain toxic pyrrolizidine alkaloids.<br />

Recommendati<strong>on</strong> 40: In patients with allergic rhinitis, we suggest clinicians do not administer <str<strong>on</strong>g>and</str<strong>on</strong>g><br />

patients do not use herbal medicines (c<strong>on</strong>diti<strong>on</strong>al recommendati<strong>on</strong> | very low quality evidence).<br />

III. Treatment of asthma in patients with allergic rhinitis <str<strong>on</strong>g>and</str<strong>on</strong>g> asthma<br />

Recommendati<strong>on</strong> 42: In patients (both children <str<strong>on</strong>g>and</str<strong>on</strong>g> adults) with allergic rhinitis <str<strong>on</strong>g>and</str<strong>on</strong>g> asthma, we<br />

suggest clinicians do not administer <str<strong>on</strong>g>and</str<strong>on</strong>g> patients do not use oral H1-antihistamines for the treatment<br />

of asthma (c<strong>on</strong>diti<strong>on</strong>al recommendati<strong>on</strong> | very low quality evidence).<br />

Underlying values <str<strong>on</strong>g>and</str<strong>on</strong>g> preferences<br />

The recommendati<strong>on</strong> not to use oral H1-antihistamines in adults with allergic rhinitis <str<strong>on</strong>g>and</str<strong>on</strong>g> asthma<br />

for the treatment of asthma places a relatively high value <strong>on</strong> avoiding their adverse effects, <str<strong>on</strong>g>and</str<strong>on</strong>g> a<br />

relatively low value <strong>on</strong> their very uncertain effect <strong>on</strong> symptoms of asthma.<br />

The recommendati<strong>on</strong> not to use oral H1-antihistamines in children with allergic rhinitis for the<br />

treatment of asthma or wheeze, despite the evidence of efficacy of ketotifen when used al<strong>on</strong>e in<br />

children with mild to moderate asthma, places a relatively high value <strong>on</strong> avoiding <str<strong>on</strong>g>its</str<strong>on</strong>g> side effects,<br />

<str<strong>on</strong>g>and</str<strong>on</strong>g> a relatively low value <strong>on</strong> <str<strong>on</strong>g>its</str<strong>on</strong>g> unknown efficacy in children already using inhaled corticosteroids,<br />

since inhaled corticosteroids are currently c<strong>on</strong>sidered medicati<strong>on</strong>s of first choice in treatment of<br />

chr<strong>on</strong>ic asthma.<br />
