ALLERGIC RHINITIS AND ITS IMPACT ON ASTHMA GUIDELINES ● 2010 – V. 9/8/2010
Recommendation 47: In patients with allergic rhinitis <strong>and</strong> asthma, we suggest sublingual specific
immunotherapy for treatment of asthma (conditional recommendation | low quality evidence).
Underlying values <strong>and</strong> preferences
This recommendation places a relatively high value on possible reduction of asthma symptoms, <strong>and</strong>
a relatively low value on avoiding adverse effects <strong>and</strong> limiting the cost of sublingual specific
immunotherapy.
Remarks
Sublingual specific immunotherapy may also be used in patients with asthma <strong>and</strong> concomitant
allergic rhinitis for treatment of rhinitis (see recommendations 34 <strong>and</strong> 35). Resource limitations will
have stronger implications for the implementation of this recommendation.
Recommendation 48: In patients with allergic rhinitis <strong>and</strong> asthma with a clear IgE-dependent
allergic component, uncontrolled despite optimal pharmacologic treatment <strong>and</strong> appropriate allergen
avoidance, we suggest monoclonal antibody against IgE for treatment of asthma (conditional
recommendation | moderate quality evidence).
Underlying values <strong>and</strong> preferences
This recommendation places a relatively high value on reduction of symptoms of asthma <strong>and</strong>
exacerbations in patients with severe asthma, <strong>and</strong> a relatively low value on avoiding the burden of
subcutaneous injections, cost of treatment, small risk of anaphylaxis <strong>and</strong> some uncertainty about the
risk of malignancy.
