further elucidati<strong>on</strong>. However, it is unlikely to influence this recommendati<strong>on</strong>.<br />

What others are saying<br />

Paediatric <str<strong>on</strong>g>and</str<strong>on</strong>g> obstetrical professi<strong>on</strong>al societies recommend breastfeeding for all infants unless<br />

there are c<strong>on</strong>traindicati<strong>on</strong>s (58-61), because of reduced risk of infecti<strong>on</strong>s observed both in<br />

developing <str<strong>on</strong>g>and</str<strong>on</strong>g> in developed counties, lower risk of other diseases suggested in many studies, <str<strong>on</strong>g>and</str<strong>on</strong>g><br />

because of maternal health benef<str<strong>on</strong>g>its</str<strong>on</strong>g>.<br />

Recommendati<strong>on</strong><br />

We suggest exclusive breastfeeding for at least the first three m<strong>on</strong>ths for all infants irrespective of<br />

their family history of atopy (c<strong>on</strong>diti<strong>on</strong>al recommendati<strong>on</strong> | low quality evidence).<br />

Values <str<strong>on</strong>g>and</str<strong>on</strong>g> preferences: This recommendati<strong>on</strong> places a relatively high value <strong>on</strong> the preventi<strong>on</strong> of<br />

allergy <str<strong>on</strong>g>and</str<strong>on</strong>g> asthma, <str<strong>on</strong>g>and</str<strong>on</strong>g> a relatively low value <strong>on</strong> challenges or burden of breastfeeding in certain<br />

situati<strong>on</strong>s.<br />

Remarks: The evidence, that exclusive breastfeeding for at least the first three m<strong>on</strong>ths reduces the<br />

risk of allergy or asthma, is not c<strong>on</strong>vincing <str<strong>on</strong>g>and</str<strong>on</strong>g> the recommendati<strong>on</strong> to exclusively breastfeed is<br />

c<strong>on</strong>diti<strong>on</strong>al. This recommendati<strong>on</strong> applies to situati<strong>on</strong>s in which other reas<strong>on</strong>s do not suggest harm<br />

from breastfeeding (e.g. classic galactosemia, active untreated tuberculosis or human<br />

immunodeficiency virus infecti<strong>on</strong> in the mother, antimetabolites, chemotherapeutic agents or<br />

radioactive isotopes used in the mother until they clear from the milk, <str<strong>on</strong>g>and</str<strong>on</strong>g> bacterial or viral<br />

infecti<strong>on</strong> of a breast).<br />

Should antigen avoidance diet be used in pregnant or breastfeeding<br />

women to prevent development of allergy in children?<br />

Summary of findings<br />

One systematic review reporting <strong>on</strong> 4 studies assessed maternal dietary antigen avoidance during<br />

pregnancy <str<strong>on</strong>g>and</str<strong>on</strong>g>/or lactati<strong>on</strong> for preventing or treating atopic disease in the child (62) (see<br />

accompanying evidence profile).<br />

Benef<str<strong>on</strong>g>its</str<strong>on</strong>g><br />

Results of available trials suggest that it is unlikely that antigen avoidance diet prevents<br />

development of allergy or asthma, although the results are inc<strong>on</strong>sistent, very imprecise <str<strong>on</strong>g>and</str<strong>on</strong>g> the<br />

follow-up was limited to the first 18 m<strong>on</strong>ths of life.<br />

Harms<br />

There is evidence of very low quality that antigen avoidance diet may increase the risk of preterm<br />

birth.<br />
