informati<strong>on</strong>. After 1 year the remediati<strong>on</strong> group had a significant decrease in symptom days after<br />

remodelling, whereas these parameters in the c<strong>on</strong>trol group did not change. The remediati<strong>on</strong> group<br />

had a lower rate of acute care vis<str<strong>on</strong>g>its</str<strong>on</strong>g> compared with c<strong>on</strong>trol group, but the difference was not<br />

significant (10.0% vs 28.1%).In a sec<strong>on</strong>d r<str<strong>on</strong>g>and</str<strong>on</strong>g>omised, cross-over trial (119) centrally installed<br />

ultraviolet (UV) irradiati<strong>on</strong> un<str<strong>on</strong>g>its</str<strong>on</strong>g> were investigated am<strong>on</strong>g 19 asthmatic children sensitized to<br />

moulds. The authors reported improvement in asthma symptom scores, the number of days with<br />

asthma symptoms, total asthma medicati<strong>on</strong> use in children provided with UV irradiati<strong>on</strong> un<str<strong>on</strong>g>its</str<strong>on</strong>g><br />

versus placebo, but this trial had methodological limitati<strong>on</strong>s <str<strong>on</strong>g>and</str<strong>on</strong>g> it was not clear if the difference<br />

between the groups was clinically important.<br />

Harms<br />

There were no definite adverse effects of these interventi<strong>on</strong>s but there is burden <str<strong>on</strong>g>and</str<strong>on</strong>g> cost.<br />

C<strong>on</strong>clusi<strong>on</strong>s <str<strong>on</strong>g>and</str<strong>on</strong>g> research needs<br />

Any net clinical benefit from methods aimed at reducing exposure to household moulds is very<br />

uncertain. Trials had methodological limitati<strong>on</strong>s, results were inc<strong>on</strong>sistent, <str<strong>on</strong>g>and</str<strong>on</strong>g> there is some<br />

uncertainty about the directness of multiple interventi<strong>on</strong>s used in the study by Kercsmar <str<strong>on</strong>g>and</str<strong>on</strong>g><br />

colleagues (118).<br />

Well designed <str<strong>on</strong>g>and</str<strong>on</strong>g> rigorously executed r<str<strong>on</strong>g>and</str<strong>on</strong>g>omised trials of methods aimed at reducing exposure to<br />

household moulds that measure <str<strong>on</strong>g>and</str<strong>on</strong>g> properly report (74, 75) patient-important outcomes are needed.<br />

If d<strong>on</strong>e, they are likely to have an important impact <strong>on</strong> this recommendati<strong>on</strong>.<br />

Recommendati<strong>on</strong><br />

In patients allergic to indoor moulds, we suggest avoiding exposure to these allergens at home<br />

(c<strong>on</strong>diti<strong>on</strong>al recommendati<strong>on</strong> | very low quality evidence).<br />

Underlying values <str<strong>on</strong>g>and</str<strong>on</strong>g> preferences: This recommendati<strong>on</strong> places a relatively high value <strong>on</strong><br />

possible reducti<strong>on</strong> in the symptoms of rhinitis <str<strong>on</strong>g>and</str<strong>on</strong>g> asthma, <str<strong>on</strong>g>and</str<strong>on</strong>g> a relatively low value <strong>on</strong> the burden<br />

<str<strong>on</strong>g>and</str<strong>on</strong>g> cost of interventi<strong>on</strong>s aimed at reducing exposure to household moulds.<br />

Questi<strong>on</strong> 9<br />

Should patients allergic to animal d<str<strong>on</strong>g>and</str<strong>on</strong>g>er avoid exposure to these<br />

allergens at home?<br />

Summary of findings<br />

We did not identify any systematic review directly addressing the questi<strong>on</strong> of pet allergen c<strong>on</strong>trol<br />

measures in patients with allergic rhinitis. However, <strong>on</strong>e recent systematic review reported 2 studies<br />

that investigated the clinical efficacy of air filtrati<strong>on</strong> un<str<strong>on</strong>g>its</str<strong>on</strong>g> <str<strong>on</strong>g>and</str<strong>on</strong>g> c<strong>on</strong>comitant envir<strong>on</strong>mental<br />

interventi<strong>on</strong>s in the homes of people with pet-allergic asthma who had pets (120). Authors’ last<br />

literature search in September 2006 did not reveal any new trials since the original review was<br />

published in 2001.<br />
