To decide if this drug will be covered, Anthem<br />
Blue Cross may ask your PCP doctor or the<br />
pharmacist (or both) for more information.<br />
Within 24 hours after getting this information,<br />
Anthem Blue Cross will tell your PCP doctor or<br />
the pharmacist if the drug will be covered. Your<br />
PCP doctor or the pharmacist will then tell you.<br />
If the drug is approved, you can get the drug at a<br />
pharmacy that works with Anthem Blue Cross. If<br />
the drug is not approved, you have the right to<br />
appeal the decision. An “appeal” is when you<br />
want a decision to be reviewed. You can learn<br />
more about this in the “Complaints: What should<br />
I do if I am unhappy?” section of this handbook.<br />
What drugs are covered?<br />
You can get the following drugs and other items<br />
when they are prescribed by your doctor and are<br />
medically necessary:<br />
Prescription drugs listed on the Anthem<br />
Blue Cross formulary<br />
Prescription drugs you get from a<br />
pharmacy not in Anthem Blue Cross’<br />
network when you have an emergency<br />
Non-prescription drugs or over-thecounter<br />
drugs (such as cough syrups,<br />
cough drops or aspirin) listed on the<br />
Anthem Blue Cross formulary<br />

Diabetic supplies: insulin, insulin<br />
syringes, glucose test strips, lancets and<br />
lancet puncture devices, pen delivery<br />
systems, blood glucose monitors including<br />
monitors for the visually impaired, and<br />
ketone urine testing strips<br />
FDA-approved birth control devices, birth<br />
control pills, diaphragms, condoms and<br />
contraceptive jellies on the Anthem Blue<br />
Cross formulary<br />
Emergency contraception<br />
EpiPens, ana-kits, peak flow meters, and<br />
spacers<br />
What drugs are not covered?<br />
Drugs from a non-network pharmacy,<br />
except drugs needed because of an<br />
emergency or out-of-area care<br />
Non-formulary drugs, except with an okay<br />
from Anthem Blue Cross<br />
Drugs that are experimental or<br />
investigational in nature, except in certain<br />
cases of terminal illness. If you have been<br />
denied an experimental or investigational<br />
drug, you have the right to request an<br />
Independent <strong>Medi</strong>cal Review (IMR). You<br />
can learn more about this in the<br />
“Complaints: What should I do if I am<br />
unhappy?” section of this handbook.<br />
Cosmetic drugs, except as prescribed for<br />
medically necessary conditions<br />
Dietary or nutritional products, except<br />
when medically necessary or for the<br />
treatment of Phenylketonuria<br />
Any injectable drug that is not medically<br />
necessary and not prescribed by a doctor<br />
Appetite suppressants, except as<br />
medically necessary for morbid obesity<br />
Compounded medications with formulary<br />
alternatives or those with no FDAapproved<br />
indications<br />
Replacement of lost or destroyed drugs no<br />
more than two (2) times each calendar<br />
year (from January to December)<br />
Emergency contraception (<strong>Plan</strong> B)<br />
You may get emergency contraceptive drugs<br />
