203 A clear record should be kept <strong>of</strong> any <strong>isolation</strong>s which are to remain on the<br />
<strong>plant</strong> after work is complete. Such <strong>isolation</strong> should be the subject <strong>of</strong> formal risk<br />
assessment <strong>and</strong> change control.<br />
204 ‘Extended term’ <strong>isolation</strong>s (eg parts <strong>of</strong> <strong>plant</strong> which have been mothballed)<br />
should be marked on the <strong>plant</strong> P&IDs. <strong>The</strong>se should be positively isolated.<br />
Appropriate controls include:<br />

a register which identifies all such <strong>isolation</strong>s <strong>and</strong> the reasons for <strong>isolation</strong>;<br />
a system to periodically check the status <strong>and</strong> integrity <strong>of</strong> each <strong>isolation</strong>; <strong>and</strong><br />
periodic review <strong>of</strong> the status <strong>of</strong> each item to decide if the <strong>isolation</strong> is still<br />
appropriate, whether the <strong>equipment</strong> should be permanently removed etc.<br />
205 You should also consider the need for additional measures such as nitrogen<br />
purging.<br />
