As state in the Sub-Clause 38.3, Point B of the Invitation To Bid (ITB) “Experience” the form No.7 “Performance Statement Form” will be required in the bid document and insert in the Amendment II. All the forms required above must but be fully completed using English language and signed. Bid/No Bid Confirmation Form The bidder shall submit this from separately to procurementmmoc@unops.org before the deadline for submission Check-List Form It is a form which helps the bidder to check if he/she has completed all the forms and also UNOPS to find out if all the necessary bidding documents are provided or if any documents are missing. This form should be well completed and submitted along with the bid This form will be updated in the Amendment II Joint Venture Partner Information Form This form shall be submitted only if applicable Bid Submission Form It is one of the most significant forms which should be properly and fully filled in with careful attention and without any alteration or substitution. This form should be Dated and Signed. Any alteration or incomplete completion shall be subject to the reject of the bid. Price Schedule Form The bidder should mention the price in line with goods specified by UNOPS. It is also one of the most important forms and therefore need to be properly completed. Questions & Answers Q1: Regarding the financial statement, can we provide UNOPS with our own created form? A1: The bidder can use self-created form like Financial Audit report, Annual Financial report Q2: How should we use the performance statement form and fill in the previous experience? A2: Performance statement form should be used to describe the past 3 years’ experience regarding the supply of similar goods. This form must be signed and submitted with the bid. Page 2 of 5
Q3: Do we need to provide the banking information? What if we don’t have bank account? A3: The bidder shall provide banking information. However, if the bidder doesn’t have bank account, he should reflect this in the bid submitted.. Q4: We cannot know about the shipping information. Do we have to complete that information in the bidding document? A4: It will be preferable to indicate this information in the bid document in order to allow us to estimate the cost of transportation of the good to the different distribution point. However, if the supplier cannot fulfill this part at the time of bidding, that information must be provided by the successful bidder(s) after awarding the contract and before shipping of the goods. Q5: Can we submit the alternative bids (different brands) which are in line with the specifications? A5: As per the clause 13 of the Invitation To Bid (ITB) “Alternative bids”, alternative will be accepted for different Model(s), Brand(s) or range of product(s) complying to the specifications Alternative offer must be placed in the same envelop together with the Original Bid. The alternative bid must bear “ALTERNATIVE BID”. If the bidders submit alternative bids, they should be accompanied with separate bid submission form, price schedule form and technical compliance sheet. Q6: What is the payment system of UNOPS? A6: As per the clause GCC 35 of the Section V “Special Conditions of Contract” Payment shall be made within 30 days after reception based on the proof of delivery and acceptance. UNOPS will transfer the money to the successful bidder’s company(ies) account according to the banking information provided by the bidder. If the bidder cannot provide the banking information at the time of bidding, payment will be made in cheque. Q7: Where do we have to provide the after sales services; in Yangon area or other site areas? A7: Since the delivery place of the goods is in Yangon so that on-site service provision need to be delivered in Yangon area. Page 3 of 5
