papilloedema. The dense central scotomata in cases 11, 12 and 13 were
caused by macular changes. The homonymous inferior quadrantanopia of
case 14 seemed to be due to surgical injury to the optic radiation. This
complication has also been described by Smith et al. (1966). In case 1.5 with
homonymous hemioptic nerve fibre degeneration and very wide lateral ven-
tricles, the homonymous hemianopia was caused by a lesion to the optic
tract or an early lesion to the optic radiation (Hoyt et al. 1973).
To sum up, the commonest cause of optic atrophy in this material
seemed to be direct or indirect compression of the anterior visual pathway
by a distended third ventricle. Other causes were papilloedema, macular
changes, and lesions to the post-chiasmatic visual pathway.
