BARBER<br />

Percy C. Conway, Lockport -- barber license reprimanded and fined $200 for<br />

working part-time for eight years while his license was non-renewed.<br />

John Morrison, Chicago -- ordered to cease and desist aiding and abetting the<br />

unlicensed practice <strong>of</strong> barbering and ordered to pay a civil penality <strong>of</strong> $100.<br />

The following individuals barber licenses were issued with reprimand and fined $50 for<br />

engaging in the unlicensed practice <strong>of</strong> barbering one month before applying for barber<br />

examination.<br />

Gregory Edge Jr., Chicago Keith Harris, Chicago<br />

COSMETOLOGIST<br />

LaVance Douglas, Park Forest -- cosmetologist license reprimanded and fined<br />

$200 for practicing on a non-renewed license.<br />

Theresa Heerema, Crete -- cosmetologist license issued with reprimand and<br />

fined $500 for practicing for five years without an <strong>Illinois</strong> license.<br />

reprimand and fined $750 for practicing without a valid license for five years.<br />

Andrea Marshall, Bethalto -- cosmetology license reprimanded and fined $500<br />

for permitting tattooing in her salon.<br />

Cheryl McGill, Chicago -- cosmetologist license reprimanded and fined $200 for<br />

altering a pocket card expiration date and practicing cosmetology on an expired license.<br />

Michelle McGill, Chicago -- cosmetologist license reprimanded and fined $400<br />

for practicing on an expired license from 1991 to 1995..<br />

Barbara Mouial, Chicago -- cosmetologist license reprimanded and fined $200<br />

for practicing prior to being licensed.<br />

Hollis Price Jr., Chicago -- cosmetologist license revoked for being more than<br />

thirty days delinquent in the payment <strong>of</strong> child support.<br />

Mary C. Remiasz, Lemont -- cosmetologist license placed on probation for two<br />

years and fined $500 for failing to submit pro<strong>of</strong> <strong>of</strong> continuing education hours<br />

during a <strong>Department</strong> audit.<br />

Ruby Roberson, Chicago -- cosmetologist license reprimanded and fined $500<br />

for allowing unlicensed practice.<br />

Emma Somlar, Bloomington -- cosmetologist license reprimanded and fined<br />

$100 for practicing on a non-renewed license.<br />

Joseph Czarnecki, Schaumburg -- private alarm contractor license revoked for<br />

being licenseee in charge <strong>of</strong> Versatile Security from 1986 to 1995 without active<br />

participation in the operations <strong>of</strong> the agency and for working for an agency with a<br />

Permanent Employee Registration Card.<br />

Nathaniel McCullor, Jr., Chicago -- permanent employee registration card<br />

revoked after being convicted in the Circuit Court <strong>of</strong> Cook County <strong>of</strong> possession <strong>of</strong> a<br />

stolen vehicle, a felony, and theft <strong>of</strong> labor/services.<br />

Steffan Schay, Moline -- private detective license indefinitely suspended due to<br />

delinquent taxes owed the <strong>Illinois</strong> <strong>Department</strong> <strong>of</strong> Revenue and failing to file income tax<br />

returns for the years 1991 through 1995.<br />

Jovani Velez, Chicago -- permanent employee registration card reprimanded<br />

for submitting an altered permanent employee registration card to a prospective<br />

employer.<br />

Walker Williams Jr., Chicago -- permanent employee registration card placed<br />

on probation for two years for acting in an unpr<strong>of</strong>essional manner in that he had been<br />

drinking alcohol while actively employed as a security <strong>of</strong>ficer and was arrested for an<br />

aggravated assault charge.<br />
