U.S. Securities and Exchange Commission.<br />
Howard R. Benditzson, Northbrook – public accountant license (065-006662)<br />
reprimanded for giving incorrect advice to a client regarding termination <strong>of</strong> the client’s qualified<br />
small business retirement plan and the requirement <strong>of</strong> filing annual reports for the plan, and<br />
giving incorrect advice regarding corrective procedures for unfiled reports.<br />
Brian Joseph Cox, Hinsdale – public accountant license (065-013378) indefinitely<br />

000252) licenses indefinitely suspended for aiding and abetting the unlicensed practice <strong>of</strong><br />
dentistry by allowing unlicensed dental assistants to perform orthodontic treatments, billing a<br />
patient in excess <strong>of</strong> the amount agreed upon for dental services, performing orthodontic<br />
treatments in a substandard manner, and failing to maintain proper patient records.<br />
Mary Hatzis, Wheaton – dentist license (019-022258) reprimanded for failing to<br />
maintain patient records including dispensing or prescribing non-controlled substances to a<br />
patient, and failing to take proper radiographs <strong>of</strong> a patient.<br />
Thomas Stein, Ann Arbor, MI – dentist license (019-015342) indefinitely suspended<br />
after the U.S. Veterans Administration, Ann Arbor Healthcare System terminated his Veterans<br />
Administration practice privileges.<br />
Karen E. Williams, Bloomington – dentist license (019-025001) reprimanded and fined<br />
$1,000 for over diagnosing the need for periodontal treatment in a patient.<br />

private security contractor agency (122-000699) and Kates’ private security contractor (119-<br />
000813) licenses each placed on probation for two years and each fined $30,000 for billing for<br />
services not performed, employing 82 individuals in an armed capacity who did not have active<br />
firearm authorization cards, employing four individuals in an armed capacity who did not have<br />
active permanent employee registration cards or firearm authorization cards, employing two<br />
individuals who did not have active permanent employee registration cards or pending<br />
applications, submitting three applications for permanent employee registration cards late, and<br />
advertising to provide private alarm contractor services without being licensed.<br />
ESI Investigations, Peoria – private detective agency license (117-000925) placed on<br />
probation for one year and fined $1,500 for practicing while its license was in an inoperative<br />
status.<br />
Chenice Laury, Robbins – permanent employee registration card (129-241306)<br />
suspended for six months followed by probation for three years for failing to report her arrest<br />
and/or conviction to the <strong>Department</strong>.<br />
Patrick M. O’Connell, Minooka and Tanner Security Services Inc., Chicago –<br />
O’Connell’s private security contractor (119-000977) and Tanner Security Services’ private<br />
security contractor agency (122-000934) licenses each reprimanded and each fined $2,250 for<br />
