APPLICATION FOR ADMISSION: GENERAL INFORMATION AND INSTRUCTIONS
This form must be completed, in block letters and in full, by new students only. An incomplete application form will not be processed and will be returned to the
applicant.
The non-refundable application fee must accompany the application form or the application will not be processed.
Applicants for all instructional programmes (courses) are subject to academic selection.
Only one form per applicant must be submitted and it must indicate the first choice of study.
Please submit this completed application form, as well as the account application form to one of the given addresses on the first page.
Admission requirements are a National Senior Certificate (Grade 12) or equivalent qualification and subject requirements as per instructional programme. A
candidate may apply with his/her Grade 12 June or subsequent examination marks.
Certified copies of the following documents must be submitted together with your application:
• Identification document.
• Your academic record in respect of studies at another higher educational institution, if applicable.
• A certificate of conduct if you are/were enrolled at another higher educational institution and have not obtained a diploma/degree there.
• Undergraduate applicants: National Senior Certificate (Grade 12) or equivalent qualification for final registration. If still in Grade 12, see admission
requirements above.
• Post-graduate applicants: You must also submit official proof that you have complied with all the requirements for a diploma/degree.
The Central University of Technology, Free State (CUT) must be notified immediately of any change of address.
The student number allocated to you must be mentioned in all correspondence.
Should you decide not to continue your studies after submission of the application, or to change your instructional programme, the Department: Student
Administration must be notified immediately in writing.
The CUT reserves the right to refuse any application without furnishing reasons; but a student has the right to appeal against his/her unsuccessful application.
PHYSICAL FACILITIES AND DISABLED STUDENTS
The CUT maintains a policy of non-discrimination towards the disabled. Disabled prospective students must satisfy themselves as to the available physical facilities
at the CUT before applying for admission. If disabled students are admitted, the specific deviation from the normal examination procedures (depending on the
specific case) will be endorsed on the statement of results.
AANSOEK OM TOELATING: ALGEMENE INLIGTING EN INSTRUKSIES
Die vorm moet volledig voltooi en in blokletters ingevul word en is slegs vir nuweling studente. ‘n Onvolledige aansoekvorm sal nie verwerk word nie, en sal aan die
aansoeker teruggehandig en/of terugversend word.
Die aansoekfooi is nie terugbetaalbaar nie en moet u aansoekvorm vergesel, andersins word u aansoek nie verwerk nie.
Applikante vir alle onderrigprogramme (kursusse) is onderworpe aan akademiese keuring.
Slegs een aansoekvorm, wat die voorkeur-studierigting aandui, moet ingedien word.
Hierdie voltooide aansoekvorm, sowel as die rekening aansoekvorm, moet asseblief by een van die voorgemelde adresse op die eerste bladsy, ingelewer word.
Toelatingsvereistes is ‘n Nasionale Senior Sertifikaat (Graad 12) of ekwivalente kwalifikasie, sowel as vakvereistes soos per onderrigprogram. ‘n Kandidaat mag
aansoek doen met sy/haar Graad 12 Junie- of daaropvolgende eksamenpunte.
Gesertifiseerde afskrifte van die volgende dokumente moet u aansoek vergesel:
• Identitieitsdokument
• U akademiese rekord ten opsigte van studie by ‘n ander hoëronderwysinstelling (indien van toepassing).
• ‘n Gedragsertifikaat indien u by ‘n ander hoëronderwysinstelling ingeskryf is/was en nie ‘n diploma/graad verwerf het nie.
• Voorgraadse aansoekers: Nasionale Senior Sertifikaat (Graad 12) of ekwivalente kwalifikasie voor finale inskrywing. Indien u in Graad 12 is, sien
toelatingsvereistes hierbo.
• Nagraadse aansoekers: U moet ‘n amptelike bewys indien dat daar aan al die vereistes vir ‘n diploma/graad voldoen is.
Die Sentrale Universiteit vir Tegnologie, Vrystaat (SUT) moet onverwyld van enige adresverandering verwittig word.
Die studentenommer aan u toegeken moet in alle korrespondensie vermeld word.
Indien u na indiening van die aansoek sou besluit om nie met u voorgenome studie voort te gaan nie, of van onderrigprogram te verander, moet die Departement
Studente-administrasie sonder versuim skriftelik in kennis gestel word.
Die SUT behou die reg voor om enige aansoek te weier sonder vermelding van redes, maar ‘n student het die reg tot appèl teen sy/haar onsuksesvolle aansoek.
FISIESE GERIEWE EN GESTREMDE STUDENTE
Die SUT handhaaf ‘n beleid van nie-diskriminasie teenoor gestremdes. Gestremde voornemende studente moet hulleself egter vergewis van die beskikbare fisiese
geriewe by die SUT voordat hulle aansoek doen om toelating. Indien gestremde persone toegelaat word, sal die spesifieke afwyking van die normale
eksamineringsprosedures (afhangende van die geval) op die staat van uitslae geëndosseer word.

2. I hereby renounce any possible action against the Central University of Technology, Free State and indemnify the Central University of Technology, Free State from any claim that may arise from the
following:
2.1 any loss of or damage to property, movable or immovable, including any consequential damage directly arising from damage to such property;
2.2 any injury, illness or death;
2.3 any event, incident or accident;
2.4 any legal costs or expenses relating to claims or lawsuits arising from the specified incidents; and
2.5 any costs incurred for medical treatment; where such loss, damage, illness, injury, death, event or incident arises from my visit to, training at and/or accommodation at the Central University of
Technology, Free State and/or any associated tour/trip/excursion/visit/experiential training or transport that may occur during my period of study at the CUT.
3. I accept that I participate in the activities mentioned in paragraph 2 at my own risk and I voluntarily accept all risks associated therewith.
4. Irrespective of any bursary or loan that has been received or is to be received, I hereby accept liability/accountability, as the responsible person, for the prompt and punctual payment of all tuition, class,
residence and other fees, of whatever nature, owed to the Central University of Technology, Free State.
5. I hereby accept and confirm that I will not occupy any residence of the CUT before all minimum claimable fees have been paid.
6. I hereby accept and confirm that I will not attend any classes of the CUT before the minimum claimable fees have been paid, provided that no student will be considered for registration before all minimum
fees have been paid and all outstanding payments arising from previous commitments have been settled.
7. I hereby accept liability/accountability for the payment of all legal fees of the CUT, including attorneys' and client fees, as well as collection fees, if I should fail to honour any of my commitments with
respect to payments.
8. I am aware of the fact that my enrolment is only valid if it complies with the regulations of the programme concerned, notwithstanding acceptance of this application by the CUT.
9. If this application is accepted, it will constitute part of the contract with the Central University of Technology, Free State.
10. I hereby undertake to respect the provisions of the Copyright Act and I indemnify the CUT from any claims that may arise from allegations of copyright violation by me as student. I furthermore undertake
to purchase all original works, as required for my studies, from my personal funds, and I accept that the CUT is not responsible for providing original works, copies of original works, or copyright licences
on original works.

1.4 dat alle besonderhede soos aan die Universiteit verstrek, waar en korrek is, in gebreke waarvan my registrasie met onmiddellike en outomatiese effek gekanselleer sal word;
1.5 dat die ooreenkoms wat uit die ondertekening van hierdie aansoek voortspruit, nieteenstaande die plek van ondertekening, deur my geag word te Bloemfontein tot stand te kom, met dien verstande
dat hierdie aansoek alleenlik ‘n geldige en bindende ooreenkoms word ná my amptelike inskrywing by die Hoofkampus in Bloemfontein. ‘n Bewys van laasgenoemde kan van die Studentenavraeafdeling
verkry word;
1.6 dat ek die Departement: Studente-administrasie onmiddellik skriftelik in kennis sal stel indien ek van adres verander.
2. Hiermee doen ek afstand van enige moontlike aksies teen die Sentrale Universiteit vir Tegnologie, en vrywaar hiermee die Sentrale Universiteit vir Tegnologie ten opsigte van enige eis wat uit die
volgende mag ontstaan:
2.1 enige verlies van of skade aan eiendom, roerend of onroerend, insluitend enige gevolgskade wat regstreeks voortvloei uit die skade aan sodanige eiendom;
2.2 enige besering, siekte of dood;
2.3 enige gebeurtenis, voorval of ongeluk;
2.4 enige regskoste of uitgawe redelikerwys in verband met eise of hofgedinge wat uit voornoemde gevalle mag voortspruit; en
2.5 enige koste vir mediese behandeling; waar sodanige verlies, skade, siekte, besering, dood, gebeurtenis of voorval voortspruit uit my besoek, opleiding en/of verblyf aan die Sentrale Universiteit vir
Tegnologie en/of enige gepaardgaande toer/uitstappie/ekskursie/besoek/ervaringsopleiding of vervoer, wat sal plaasvind gedurende my studietydperk aan die Universiteit.
3 Ek aanvaar dat ek op my eie verantwoordelikheid die genoemde bedrywighede in paragraaf 2 meemaak en die risiko verbonde daaraan vrywillig aanvaar.
4 Onafhanklik van enige beurs of lening wat ontvang is, of ontvang moet word, aanvaar ek hiermee aanspreeklikheid, as die verantwoordelike persoon, vir die stiptelike betaling van alle studie-, klas-,
koshuis-, en ander gelde, van watter aard ookal, verskuldig aan die Sentrale Universiteit vir Tegnologie.
5 Ek aanvaar en bevestig hiermee dat geen intrek in ‘n koshuis van die Universiteit geneem sal word alvorens die minimum opeisbare gelde betaal is nie.
6 Ek aanvaar en bevestig hiermee dat geen klasse van die Universiteit bygewoon sal word alvorens die minimum opeisbare gelde betaal is nie, met dien verstande dat geen student vir registrasie oorweeg
sal word voordat alle minimum gelde betaal is, en alle uitstaande betalings weens vorige verpligtinge gemaak is nie.
7 Ek aanvaar hiermee aanspreeklikheid vir betaling van alle regskostes van die Universiteit, insluitend prokureurs- en kliëntekoste, asook invorderingskoste, indien ek sou nalaat om enige verpligtinge ten
opsigte van betalings na te kom.
8 Ek neem kennis dat my inskrywing slegs geldig is indien dit voldoen aan die regulasies van die betrokke program, nieteenstaande aanvaarding van hierdie aansoek deur die Universiteit.
9 Indien hierdie aansoek aanvaar word, sal dit deel uitmaak van die kontrak met die Sentrale Universiteit vir Tegnologie, Vrystaat.
10 Hiermee onderneem ek om die bepalinge van die Wet op Outeursreg te respekteer en vrywaar ek die Universiteit van enige eise wat mag voortspruit uit beweringe van kopieregskending deur my as
student. Ek onderneem voorts om alle oorspronklike werke soos benodig vir my studie uit eie fondse aan te skaf, en aanvaar dat die Universiteit nie verantwoordelik is vir die verskaffing van oorspronklike
werke, afskrifte van oorspronklike werke of outeursreglisensies op oorspronklike werke nie.

3. Ek aanvaar dat my seun/dogter op eie verantwoordelikheid die genoemde bedrywighede in paragraaf 2 meemaak, en ek aanvaar die risiko verbonde daaraan vrywilliglik.
4. Ek aanvaar hiermee gesamentlik en afsonderlik met my seun/dogter, met afstanddoening van die eksepsies van uitwinning, skuldsplitsing en sessie van aksie, aanspreeklikheid vir die stiptelike betaling
van alle bedrae, van watter aard ookal, hoegenaamd wat my seun/dogter aan die Universiteit verskuldig mag wees, hetsy hy/sy die bogenoemde voorgenome kursus of enige ander kursus volg, en ook
ten opsigte van koshuisverblyf. Hierdie sekerheidstelling kan net met die Universiteit se skriftelike toestemming gekanselleer of teruggetrek word.
5. Ek onderneem om alle regskostes van die Universiteit, insluitend prokureurs- en kliëntkoste, asook invorderingskoste, te betaal indien ek sou nalaat om enige verpligtinge ten opsigte van betaling na te
kom.
6. Ek kom hiermee ooreen dat die ooreenkoms wat uit die ondertekening van hierdie dokument voortspruit, nieteenstaande die plek van ondertekening, deur my geag word te Bloemfontein tot stand te kom;
