As the first and most experienced hospice program in the region, CHC is the preferred choice for area families and referral sources. A longtime proponent for interdisciplinary care across the spectrum of needs for those with life-limiting illnesses, CHC provides palliative care, hospice care, grief services, and educational programming for the entire community. Everything we do is intended to enlighten patients and families to the choices available for them to make the most of life. Incorporated as a not-for-profit organization in 1978 by a group of visionary volunteers, our original hospice program began serving patients in 1980. Covering eight counties in northern Indiana with three care offices, two inpatient units or Hospice Houses, and one stand alone community bereavement center, we care for more patients annually than any hospice program in Indiana. The greater medical community has played a fundamental role in our success. We are grateful to have received support from hundreds of physicians, group practices, hospitals and other healthcare facilities and educational institutions. Additional partnership opportunities include agreements to provide hospice services with more than 80 different extended care facilities throughout northern Indiana. These include licensed skilled nursing homes, as well as group homes and assisted living facilities. While we have made strides over the past 33 years, much remains to be done and most of it resides with public education and helping people understand they have choices. And that it’s best to make some of those choices well before you have to. Right now many Americans are spending more time planning this summer’s vacation than they have ever spent planning their final journey or letting their loved ones know what their end of life or health care wishes are.
