event of the 2013 <strong>PNAAF</strong> International Collaborative Conference II here in<br />
Bacolod City, Philippines. I want to congratulate first and foremost our<br />
Collaborator the Central Philippines Adventist College, School of Nursing for<br />
this splendid conference and to all your hardworking leaders and committee<br />
members.<br />
As the President of the PNAA Foundation, I am very excited with the Theme<br />
of the Conference "Promoting Quality Health through Transformative<br />
Leadership and Global linkages. It is so fitting since it reflects our<br />
missions.“The Philippine Nurses Association of America Foundation, Inc.<br />
(<strong>PNAAF</strong>), in partnership with the Philippine Nurses Association of America,<br />
Inc. (PNAA), supports the development of Filipino American nurses to their<br />
fullest potential and facilitates their contributions to health and well-being all<br />
through nursing care, education, leadership, management and research<br />

Message<br />
It is an honor and a great privilege to host the second Philippine Nurses Association of<br />
America Foundation (<strong>PNAAF</strong>) Collaborative Conference here in Bacolod City.<br />
My profound gratitude and appreciation to the <strong>PNAAF</strong> officers led by their President Ernie<br />
Rosas and Conference Chair Araceli Antonio for their genuine commitment and concern to<br />
come up with an organized program of sharing their expertise to their colleagues here in the<br />
Philippines. Likewise, I like to recognize the collaborative efforts of Director Lily Ann<br />
Baldago of Professional Regulation Commission, Region V1, who initiated the preparation<br />
for this event, the Philippine Nurses Association, Negros Occidental Chapter and the<br />
Association of Deans of Philippine Colleges of Nursing, Region V1.<br />
I like to congratulate and thank the working committees in their individual capacities;<br />
Mr. Teody Ray Llasos of <strong>PNAAF</strong>, Dr. Chris Sorongon of RGO, Dean Helen Correa, Dean<br />
Jocelyn Cadena, and Dean Maricor Famillaran and others who contributed their invaluable<br />

The theme for this conference is what we need to consider since quality care and competent<br />
workers are emphasized worldwide." Promoting Health Care Quality through Leadership<br />
and Global Linkages: Building Capacity for Evidence- Based Practice" is the conceptual<br />
framework of the sessions. This is a global call for improvement of health care quality<br />
through leadership and professional development in building our capacity to conduct<br />

Continuing Care Retirement Community: A Housing Alternative for Older Adults<br />

ABSTRACT<br />
Housing strategies must be explored for an expanding baby boomer population - as part of the advanced<br />
care planning for Older Adults. The Continuing Care Retirement Community (CCRC) is a housing<br />
alternative for the Older Adults. CCRC is part independent living, part assisted living and part skilled<br />
<br />
needs. Healthy adults can reside independently in single-family homes, apartments or condominiums.<br />
These communities give older adults the option to live in one location for the duration of their life, with<br />
much of their future care already planned. This provides a great level of comfort to both residents and<br />
their families as the stress of the care giving relationship is greatly relieved. (1)<br />
The author provides in depth information on <br />
Levels of care, services, care providers, and clientele of continuing care retirement communities will be<br />
described. The author will discuss the impact of regulations on quality of care and client outcomes, as<br />
well as the rationale for the recommended standards of CCRC Best Practices on Disclosure and<br />
Transparency (1).<br />
In California, CCRCs offer a long-term continuing care contract that provides for housing, residential<br />
services, and nursing care, usually in one location, and usually for a resident's lifetime. The California<br />
Department of Social Services is responsible for the oversight of continuing care providers. The<br />
Department's Community Care Licensing Division has two branches that participate in the regulation. The<br />
Senior Care Program monitors continuing care providers for compliance with the Community Care<br />
licensing laws and regulations regarding buildings and grounds, accommodations, care and supervision of<br />
residents, and quality of service. The Continuing Care Contracts Branch is responsible for reviewing and<br />
approving applications to operate a CCRC and monitors the ongoing financial condition of all CCRC<br />
providers and their ability to fulfill the long-term contractual obligations. (2)<br />

Ms. Antonio is currently the Director of Health Services at Channing House, a CCRC in Palo Alto,<br />
California. She has worked in long term care for at least 35 years. Nursing positions held include<br />
Director of Nursing Services, Director of Staff Development, and Nursing Consultant. Other long term<br />
care management positions held include Director of Professional Services, Director of Clinical Services,<br />
and Vice President of Operations. Ms. Antonio is the Immediate Past President of the Philippine Nurses<br />
Association of America Foundation (<strong>PNAAF</strong>) and an Advisor of the Philippine Nurses Association of<br />
Northern California.<br />

ABSTRACT<br />
Dementia is a progressivee disabling disease that resulted in impaired memory, judgment and<br />
reasoning. Dementia is an umbrella term for a collection of symptoms that can be caused by a<br />
number of disorders that affect the brain. It causes damages to brain which causes a steady loss<br />
of memory, thinking and behavior. It is not part of aging and symptoms develop slowly and gets<br />
worse over time. (1)<br />
As the dementia progresses symptoms of the diseases varies. One in eight older Americans has<br />
<br />
unpaid caregivers which causes caregiver burnout.<br />
The author provides information on the different stages of dementia, triggers, and management<br />
through evidence-based literature. The author will share real stories and actual experience in<br />
dealing with the complex behaviors in the actual unit setting. The authors will state the nonpharmacological<br />
and pharmacological interventions in the care of the individual with dementia.<br />
Alzheimers organization goals are <br />
the latest research in the care of the dementia. Research continues and dementia remains a puzzle<br />
that treatment for this progressive disease remains incurable (2).<br />

A Case Study of the Implementation of Electronic Health Record in the Pre-Admission Testing<br />
and Same Day Surgery Units in Central New Jersey<br />

Abstract<br />
The purpose of this project was to construct a case study research on the implementation of an<br />
electronic health record (EHR) on the Pre-Admission Units and Day Surgery Centers in an acute<br />
care hospital in Central New Jersey. Rogers's Innovation-Diffusion Theory was used to guide<br />
the study of this technological innovation in the Pre-Admission Testing Unit and Day Surgery<br />
Center. Case study research permitted an in-depth investigation of the impact of diffusion of<br />
EHRS as a technological innovation and bounded phenomenon that was undertaken in real life<br />
situations. The use of multiple methods was considered to be a key activity promoting rigor in<br />
the case study method and constitutes methodological triangulation. For this case study, survey<br />
questionnaires, interview, time and motion study, and documentary data comprised the data<br />
sources for secondary analysis. Data were reduced to conceptual groups so that conclusions<br />
could be derived and a case description developed. The intended outcome of this project was to<br />
gain insight into the ways in which technological advances are diffused across nursing units.<br />
There were no statistically significant findings on the paired samples T-test comparing<br />
pre and post mean scores on the attitude scale (t=1.938, p =.094 (sig. 2-tailed). Comparing the<br />
pre-implementation and post-implementation mean scores revealed that nurses had more<br />
negative attitudes post-computerization as evidenced by a decreased mean attitude score from<br />
2.53 to 1.78.<br />
An interview provided more in-depth analysis to understand the phenomenon –<br />
diffusion and use of EHR and its impact on nursing practice and patient care from the<br />
perspective of working nurses. Overall, the nurses preferred the EHR to paper record but wanted<br />
it to better support their nursing practice. Nurses stated that there is no current policy and<br />
procedure for the EHR but training guidelines were being used for training. The time and motion<br />
study findings revealed variables and inefficient time practices that could be reduced/eliminated<br />
to improve nursing workflow and generate cost savings. Results of the completeness of the<br />
documentation review indicated that the EHR (N=20) provided a 100% compliance compared to<br />
11% paper chart record (N=16) with the elements selected for review.<br />
Results of this case study will be valuable to streamline EHRs and improve/redesign<br />
nursing workflow as well as to develop more organized plan and effective implementation<br />
strategies in the diffusion of the EHR hospital wide.

Behavioral Objectives: At the end of the session attendees will be able to <br />
1. Identify three goals of an Adult Day Healthcare Center (ADHC)<br />
2. Discuss the roles of the core services of an ADHC<br />
3. Differentiate between a restorative therapy and a maintenance program.<br />
4. State five mandated requirements to meet licensure for an ADHC<br />
The Center shall provide core services: nursing, social work, maintenance program, and activity<br />
Program. Specialized services are required: Restorative therapy (i.e. physical therapy.<br />
Occupational therapy and speech pathology services), Dietary, and psychiatry/psychology<br />
services. Other services stipulated by state regulations are food services and transportation to<br />
and from the center.<br />
Mandates for both core and specialized services are: Initial evaluations and evidence-based<br />
treatments, reassessments every six months, quarterly progress reports, and documentation every<br />
treatment visit.<br />
Finally, the interdisciplinary team comprised of a nurse, social worker, dietitian, therapists, and<br />
<br />
. Patients who were given their<br />
six month reassessment and those provided with initial evaluations are discussed . An<br />
individualized patient care plan (IPC) is established for each patient discussed during this<br />
interdisciplinary team meeting.<br />
Each member representing the interdisciplinary treatment team implements the treatment for<br />
each patient per established IPC.<br />

Methods: Questionnaires and Focus Groups<br />
Results: Descriptive analysis was used to determine the differences or significance of the responses.<br />
The data was used to address social determinants of health disparities.<br />
Conclusions and Implications:<br />
This presentation focuses on clinical management of patients with Type 2 Diabetes Mellitus (T2DM)<br />
based on review of literature and the latest evidence-based guidelines. The guidelines suggest using a<br />
team of specialists for patient-centered care.<br />
According to the National Guideline Clearing House website, the clinical practice guidelines (CPGs)<br />
for diagnosis and management of type 2 DM were developed by several international organizations.<br />
In regards to Pharmacotherapy, there was agreement on the key principles for the development of the<br />
CPGs, except for the disclosure of conflicts of interest (COIs). As a result of the controversy over the<br />
COIs, the policy for Guideline Committees was changed in 2010 so that at least 50% of writing<br />
committee members, plus the chair may have no relationship with industry (RWI). The CPGs for<br />
drug therapy are primarily founded on expert consensus rather than on evidence-base grading of the<br />
literature.<br />
Two important lifestyle changes in the clinical management of T2DM are medical nutritional therapy<br />
(MNT) and physical activity. Carbohydrate counting of food has been found effective in the<br />
glycemic control of patients with Type 2 DM on insulin therapy.<br />
About the Presenter: Remedios A. Solarte, MSN, RN, NP, is a Professor Emeritus of Nursing at<br />
Oakland Community College, Waterford, Michigan, where she was on the faculty for 32 years. She<br />
obtained her nursing diploma and BSN from Far Eastern University, Philippines, and MSN from<br />
Wayne State University. In 2010, she was the Project Director of the PNA Michigan Phase I<br />
Capacity Building Project, funded by the Michigan Department of Community Health, Health<br />
Disparities Reduction Minority Health (MDCH-HDRMH). This project addressed social<br />
determinants of health disparities among Filipinos. Her other career highlights include: caring for<br />
patients as a part time clinical nurse specialist for eight years in the Diabetic Clinic, St. Joseph Mercy<br />
Hospital, Michigan; President of the PNAA from 1990-1992; and Member, Michigan Board of<br />
Nursing for 4 years, for which she was awarded a Certificate of Appreciation by Governor James<br />
Blanchard and honored with a Resolution from the Department of Commerce for unselfish service to<br />
the people of Michigan.

Methods: Information data were collected through review of literature from different authors who have interest<br />
on the pharmacological effects of plants/trees as alternative treatment in the promotion of health and prevention<br />

Data collection method of its effect was done through actual utilization of processed plant/trees product.<br />
Sampling population are from the (1) community who are interested to use the natural product after receiving<br />
information through lectures with problems identified in the related literature and its positive outcome to<br />
alleviate suffering as relief from respiratory allergy, skin allergy, prevention from mosquito bites, severe sore<br />
throat, respiratory problem as bronchitis, laryngitis with no voice, lowering of blood sugar, blood pressure,<br />
removal of warts, decrease face wrinkles, prostate cancer, abdominal cancer (2) Patients or (residents) of the<br />
skilled nursing facility where the author is working; residents who have skin problems as pressure sore,<br />
psoriasis, heat rash, skin cancer, herpes zoster/simplex.<br />
Results of the pharmacological effects were collected through testimonials and actual<br />
observation from population who utilized the plants/trees product.<br />
Conclusion and Implications: Based on the quantified evidences on the use and effect of garden products,<br />
pharmacy in nature is an alternative treatment of ailment and promotion of health.<br />
