154 12 Deaths from psychiatric causes protective against relapses of pre-existing serious mental illness, particularly if the woman has stopped her usual medication at the beginning of pregnancy. Women who have had a previous episode of a serious mental illness, either following childbirth or at other times, are at an increased risk of developing a postpartum onset illness even if they have been well during pregnancy and for many years previously. This risk is estimated as at least one in two 5,6 . The last two Reports 7,8 found that over half of the women who died from suicide had a previous history of serious mental illness. It is also known that a family history of bipolar disorder increases the risk of a woman developing puerperal psychosis following childbirth 6 . Specialist perinatal psychiatric services Serious psychiatric illnesses in the last few weeks of pregnancy and the fi rst few weeks following childbirth have a number of distinctive clinical features including, importantly, the tendency of sudden onset and rapid deterioration. Half of all cases of puerperal psychoses will have presented by day seven postnatally, and 90% by three months postpartum4 , as highlighted in the last two Reports. This, together with other distinctive symptoms9 and the special needs of women and their infants at this time, has led to national and international acceptance of the need for special services for perinatal psychiatric disorder10,11 . This includes the recommendation that new mothers who require admission to a psychiatric unit following birth should be admitted together with their infant to a specialised mother and baby unit. The fi ndings of the last two Enquiries underpin the importance of this strategy. Maternal suicide Until recently it had been thought that the maternal suicide rate was lower than would be expected12 , with pregnancy exerting a so-called “protective effect”. However, the last two Enquiry Reports found that maternal suicide was more common than previously thought and was in fact the leading overall cause of maternal death. These Reports also called into question “the protective effect of maternity”. However, suicide during pregnancy remains relatively uncommon and the majority of suicides occur following childbirth. Overall, the suicide rate following delivery is no different to that amongst women in the general population, but it may be that for a sub-group of women, those suffering from serious mental illness, the suicide rate is substantially elevated13 . One of the reasons for the misunderstanding about maternal suicide is that suicide research over the last 40 years has consistently shown that rates based upon coroners’ verdicts alone are under-estimates. For the last three triennia, the Offi ce for National Statistics (ONS) has been able to link mothers’ deaths up to one year after delivery with recorded births. This revealed that around half of all maternal suicides had not been reported directly to this Enquiry. Once this underreporting was corrected, a more realistic estimate of maternal suicide was possible. Summary of key fi ndings for 2003-05 Suicide The deaths of thirty-three women who, in the opinion of the assessors, committed suicide in the years 2003-05, were reported to the Enquiry. The circumstances surrounding two of these cases were suspicious of suicide but could not be proven. The ONS record linkage study, described in Chapter 1, only identifi ed four more cases of suicide that appeared unknown to the Enquiry for this triennium. In total, therefore, it appears 37 women committed suicide whilst pregnant or during their fi rst postnatal year in the UK for 2003- 05. Twelve deaths from suicide occurred during pregnancy or shortly after delivery and these cases are counted as Indirect deaths in this Chapter. The other deaths, which occurred after the 42 day postpartum
cut off point for maternal deaths, are counted in Chapter 14 as Late deaths. All but three of these later deaths were considered to be directly due to a postnatal mental illness and are counted as Late Indirect, the three others occurring in women with long standing mental health problems and were not considered related to pregnancy. Substance abuse An overdose of drugs of abuse was responsible for the deaths of twenty-two women, three of which occurring during pregnancy and three shortly after birth. These six cases are counted in this Chapter as Indirect maternal deaths as the circumstances surrounding the pregnancy and death meant the assessors could not rule out an intentional overdose. The others deaths were most probably accidental and occurred later after childbirth. These deaths are counted in Chapter 14, as Late Coincidental deaths. Deaths from suicide and accidental drug overdose are considered to be psychiatric deaths and are described in this Chapter. However, they are counted either in this Chapter, or Chapter 14 depending on the timing of the mothers’ death in relation to childbirth. The fi gures for this triennium are given in Table 12.1.
