Leonora Theresa Galang ABOUT THE SERVICE: The purpose of this service is to examine and diagnose cases pertaining to medico legal reasons (Battered wife, Women Abused, Child Abused, Raped, and Physical Injuries). It also examines Dead on Arrival cases for Autopsy such as stub wounds, death due to shooting, vehicular accident and any other questionable cause of death. The service is available at the Room 4-A, City Health Office (CHO) to any person/individual who needs medico legal assistance. WHO MAY AVAIL OF THE SERVICE: All bona-fide residents of Bacolod City FEES / CHARGES: Medico Legal Fee – P10.00 REQUIREMENTS: For Medico Legal Cases “Korasyon” Blotter from the Police Office (2 copies) SCHEDULE OF THE SERVICE AVAILABILITY: Monday to Friday – 8:00 am to 12:00 noon and 1:00 pm to 5:00 pm
HOW TO AVAIL OF THE SERVICE: Steps Time Frame For Medico –Legal Service Person(s) In-Charge 1. For client admittance and Staff on duty assessment, approach staff on 5 minutes duty, state need and other information. 2. Submit to Physical Examination. 3. Recording/encoding and releasing of certification. Physician on duty 5 to 10 minutes Dr. Eli C. Cong Medical Officer V Medico Legal Officer 5 to 10 minutes Staff on duty For Death Certificate: Local Civil Registrar’s Office issued Death Certificate to next kin in the event of death. If the deceased was not attended by the Hospital Doctors, it is the duty of the City Health Doctor to diagnose. When the deceased died upon the arrival in the hospital, the Hospital Doctor will issue for Dead on Arrival Form (DOA). WHO MAY AVAIL OF THE SERVICE: Close relative of the deceased FEES / CHARGES: None REQUIREMENTS: Non Traumatic/Natural Death 1. Barangay Certification that he is a bona-fide residents of Bacolod City (2 copies). 2. Death Certificate Form from Local Civil Registrar Traumatic Death and Foundling: 1. Barangay Certification that he is bona-fide residents of Bacolod City. 2. Acquire the request for Autopsy from Police Office or SOCO. 3. All victims & defendants who want the Medico Legal Officer to be a witness of any hearing must secure Medico Legal Certificate and/or Autopsy Report Trauma Sheet and Death Certificate issued by the Medico Legal Officer. 2. Inform your Fiscal or Lawyer about subpoena needed and inform Medico Legal Officer about the case of the victim to attend the scheduled investigation in the court.
