On September 30, 2011, I conducted an unannounced inspection at the facility. All children were indoors. Preschool<br />

children were observed all in one room on cots with six children on cots that were worn down and laying on the floor.<br />

The infanUtoddler room was observed to be out of ratio in that nine children, ages 3-months to 19-months, were cared<br />

for by two caregivers.<br />

On September 30, 2011, it was additionally noted that group-size requirements were not maintained in the<br />

infanUtoddler room in that nine children under 24-months were present in one group, twenty-eight preschool children<br />

were observed on cots without sheets, the back door in the Three-Year-Olds Room was wide open without a screen,<br />

and the infant/toddler room exit door was blocked by a crib.<br />

19 CSR30-62.082 (2) (A) 2. states "Open windows and doors shall be screened securely. Barriers to prevent children<br />

from falling against windows or falling from windows shall be provided when windows are less than twenty-four inches<br />

.(24") from the floor and not constructed of safet lass or other non-breakable material."<br />

On September30, 2~011,the back door in the Three-Year-Olds Roomwas observed wide open without a screen.<br />

19 CSR 30-62.087 (1) (M) states "Means of egress is a continuous and unobstructed way of travel from any pointin a<br />

building or structure to a public way. A means of egre'ss consists of three (3) distinct parts: the exit access, the exit,<br />

and the exit discharge."<br />

19 CSR 30-62.092 (1) (A) 2. states "Furniture and equipment shall be arranged to provide a clear passage to all exits."<br />

On September 30, 2011, the infanUtoddler room exit door was observed to be blocked by a crib.<br />

19 CSR 30-62.092 (1) (A) 1. states "All furniture and equipment shall be constructed safely, in good condition and free<br />

of sharp, loose or pointed parts. Only lead free paint shall be used."<br />

On September 30,2011, six children were observed on cots that were wom down and laying on the floor<br />

19 CSR 30-62.092 (1) (B) 1. A. states "An individual cot or bed with an individually assigned sheet and blanket shall<br />

be provided for each child who naps or ?Ieeps. Upper levels of bunk beds shall not be used."<br />

On September 30, 2011, twenty-eight preschool children were observedon cots without sheets.<br />

19 CSR 30-62.112 (1) (A) states "The following staff/child ratios shall be maintained on the premises at all times: (A)<br />

Birth Through Two (2) Years. Groups composed of mixed ages through two (2)- years shall have no less than one (1)<br />

adult to four (4) children, with no more than eight (8) children in a group;"<br />

On September 30, 2011, ratios were not maintained in that nine children, ages 3-months to 19-months,were cared for by two<br />

caregivers and group size requirementswere not maintained in the infanUtoddlerroom in that nine children under 24-months<br />

were present in one group.<br />

The following corrections are required:<br />

.:. All staff shall sign a statement of compliance of having read and understand the rules and regulations<br />

regarding 19 CSR 30-62.082 Physical Requirements of Group <strong>Day</strong> <strong>Care</strong> Homes and <strong>Day</strong> <strong>Care</strong> Centers,<br />

19 CSR 30-62.092 Furniture Equipment and Materials, and 19 CSR 30-62.112 Staff/Child Ratios .<br />

•:. Additional unannounced inspections to verify compliance of staff/child ratios and group size.<br />

The signed statements shall be provided to CCFS Rhonda Delmain by November 15, 2011.<br />

Verification of corrections will be filed in the public record as they occur and/or are verified.<br />
