•• at shortly before 9am, reporter observed 2 teachers with 5 children, one teacher left the room and went on the
playground to throwaway a diaper, leaving the room out of ratio
" at 10:25am, 9 infants were observed in one room, exceeding group size limitations
• at 10:40am, 10 infants were observed in one room, exceeding group size limitations
•• at 11am, 2 children were left unobserved in cribs in one side of the infant/toddler room while all tIle staff were on
the other side of the room

•• at 1:50pm, there were 9 infants present in one room .

SUMMARY OF INVESTIGATION Attach pages as needed.
On 4.19.2011, I conducted an unannounced investigation at the facility with CCS Nancy Scherer. Director Donna
Griffin was not present. Veronica Lloyd was present and in-charge Upon arrival, attendance was first observed and
ratios were maintained in all rooms. The allegations were reviewed with Veronica Lloyd and Katherine Cochrell.
Veronica Lloyd stated that both incidents of violations had occurred and were corrected right away. She believes that
Donna Griffin spoke with the staff but is unsure if there was written documentation of such. It was discussed the issue
of the infant/toddler rooms not having a safety gate or barrier to securely prevent children from crossing over from one
side of the room to the other.
DISPOSITION: SUBSTANTIATED list statute number or rule number(s) of substantiated vioiation(s).
19 CSR 30-62.082 (2) (B) 2. B. (V) For all other group day care homes licensed for more than four (4) infant/toddlers
and for all day care centers licensed for more than twenty (20) children or more than four (4) infant/toddlers, the
following shall apply: (V) In facilities initially Ifcensed for infant/toddler care after the effective date of these rules or
facilities adding new infant/toddler space, no more than twenty-four (24) infants/toddlers and/or two(2)-year aids shall
be in a room. A room shall be defined as an area separated from other parts of the building by floor-to-ceiling walls.

Stable partitions a minimum of four feet (4') in height shall be used to separate the infant/toddler or two (2)-year old
groups, or both, in a room
On 4.19.2011, the infant room was observed without a safety gate to fully separate the two sides of the room and the
toddler room was observed with a safety gate that was not secured.
19 CSR 30-62.112 (1) (A) The following staff/child ratios shall be maintained on the premises at all times: (A) Birth
Through Two (2) Years Groups composed of mixed ages through two (2)-years shall have no less than one (1) adult
to four (4) children, with no more than eight (8) children in a group;
On 4.19.2011, Veronica Lloyd stated that the allegations of ratio and group size violations had occurred on 44.2011
and 4.52011.
19 CSR 30-62.182 (1) (A) 1. Caregivers shall not leave any child without competent adult supervision
AND ~
19 CSR 30-62.182 (1) (A) 3. Caregivers shall provide frequent, direct contact so children are not left unobserved
on the premises.
AND
19 CSR 30-62.182 (1) (D) 1. Infants and toddlers shall have constant care and supervision.
On 4.19.2011, Veronica Lloyd stated that the allegations of staff all walking to on8 side of the room had occurred on
4.42011.
REQUIRED CORRECTIONS AND DEADLINES Attach pages as needed.
The following corrections are required:
.:. All staff shall sign a statement of compliance of having read and understand the rules and regulations
regarding 19 CSR 30-62.112 Staff I Child Ratios and 19 CSR 30-62.182 Child Care Program .
•:. All staff shall sign a statement that staff / child ratios and group size limitations shall be maintained at all times .
•:. A plan shall be developed on how the facility will maintain staff Ichild ratios and group size limitations .
•:. Signs shall be posted at the entrance of all rooms displaying the required staff/child ratios approved for tile
room .
•:. Safety gates shall be secured to separate groups of both the Infant Room & Toddler Room The safety gates
must be secured to prevent being moved by children
Evidence of corrections made shall be provided to CCFS Caron Ahlemeyer by May 20, 2011.
Verification of corrections will be filed in the public record as they occur and/or are verified.
