•• at shortly before 9am, reporter observed 2 teachers with 5 children, one teacher left the room and went on the<br />

playground to throwaway a diaper, leaving the room out of ratio<br />

" at 10:25am, 9 infants were observed in one room, exceeding group size limitations<br />

• at 10:40am, 10 infants were observed in one room, exceeding group size limitations<br />

•• at 11am, 2 children were left unobserved in cribs in one side of the infant/toddler room while a<strong>ll</strong> tIle sta<strong>ff</strong> were on<br />

the other side of the room<br />

'" teachers observed violating ratios and group size are Alisa Felton, Myrtle Purne<strong>ll</strong>, & Erica Gri<strong>ff</strong>in<br />

on 452011:<br />

•• at 1:50pm, there were 9 infants present in one room .<br />

•• teachers observed violating ratios and group size are Donza Hampton, Keisha Perkins, & Donna Gri<strong>ff</strong>in<br />

SUMMARY OF INVESTIGATION Attach pages as needed.<br />

On 4.19.2011, I conducted an unannounced investigation at the facility with CCS Nancy Scherer. Director Donna<br />

Gri<strong>ff</strong>in was not present. Veronica Lloyd was present and in-charge Upon arrival, attendance was first observed and<br />

ratios were maintained in a<strong>ll</strong> rooms. The a<strong>ll</strong>egations were reviewed with Veronica Lloyd and Katherine Cochre<strong>ll</strong>.<br />

Veronica Lloyd stated that both incidents of violations had occurred and were corrected right away. She believes that<br />

Donna Gri<strong>ff</strong>in spoke with the sta<strong>ff</strong> but is unsure if there was written documentation of such. It was discussed the issue<br />

of the infant/toddler rooms not having a safety gate or barrier to securely prevent children from crossing over from one<br />

side of the room to the other.<br />

DISPOSITION: SUBSTANTIATED list statute number or rule number(s) of substantiated vioiation(s).<br />

19 CSR 30-62.082 (2) (B) 2. B. (V) For a<strong>ll</strong> other group day care homes licensed for more than four (4) infant/toddlers<br />

and for a<strong>ll</strong> day care centers licensed for more than twenty (20) children or more than four (4) infant/toddlers, the<br />

fo<strong>ll</strong>owing sha<strong>ll</strong> apply: (V) In facilities initia<strong>ll</strong>y Ifcensed for infant/toddler care after the e<strong>ff</strong>ective date of these rules or<br />

facilities adding new infant/toddler space, no more than twenty-four (24) infants/toddlers and/or two(2)-year aids sha<strong>ll</strong><br />

be in a room. A room sha<strong>ll</strong> be defined as an area separated from other parts of the building by floor-to-ceiling wa<strong>ll</strong>s.<br />

Stable partitions a minimum of four feet (4') in height sha<strong>ll</strong> be used to separate the infant/toddler or two (2)-year old<br />

groups, or both, in a room<br />

On 4.19.2011, the infant room was observed without a safety gate to fu<strong>ll</strong>y separate the two sides of the room and the<br />

toddler room was observed with a safety gate that was not secured.<br />

19 CSR 30-62.112 (1) (A) The fo<strong>ll</strong>owing sta<strong>ff</strong>/child ratios sha<strong>ll</strong> be maintained on the premises at a<strong>ll</strong> times: (A) Birth<br />

Through Two (2) Years Groups composed of mixed ages through two (2)-years sha<strong>ll</strong> have no less than one (1) adult<br />

to four (4) children, with no more than eight (8) children in a group;<br />

On 4.19.2011, Veronica Lloyd stated that the a<strong>ll</strong>egations of ratio and group size violations had occurred on 44.2011<br />

and 4.52011.<br />

19 CSR 30-62.182 (1) (A) 1. Caregivers sha<strong>ll</strong> not leave any child without competent adult supervision<br />

19 CSR 30-62.182 (1) (A) 3. Caregivers sha<strong>ll</strong> provide frequent, direct contact so children are not left unobserved<br />

on the premises.<br />

19 CSR 30-62.182 (1) (D) 1. Infants and toddlers sha<strong>ll</strong> have constant care and supervision.<br />

On 4.19.2011, Veronica Lloyd stated that the a<strong>ll</strong>egations of sta<strong>ff</strong> a<strong>ll</strong> walking to on8 side of the room had occurred on<br />

4.42011.<br />

REQUIRED CORRECTIONS AND DEADLINES Attach pages as needed.<br />

The fo<strong>ll</strong>owing corrections are required:<br />

.:. A<strong>ll</strong> sta<strong>ff</strong> sha<strong>ll</strong> sign a statement of compliance of having read and understand the rules and regulations<br />

regarding 19 CSR 30-62.112 Sta<strong>ff</strong> I Child Ratios and 19 CSR 30-62.182 Child Care Program .<br />

•:. A<strong>ll</strong> sta<strong>ff</strong> sha<strong>ll</strong> sign a statement that sta<strong>ff</strong> / child ratios and group size limitations sha<strong>ll</strong> be maintained at a<strong>ll</strong> times .<br />

•:. A plan sha<strong>ll</strong> be developed on how the facility wi<strong>ll</strong> maintain sta<strong>ff</strong> Ichild ratios and group size limitations .<br />

•:. Signs sha<strong>ll</strong> be posted at the entrance of a<strong>ll</strong> rooms displaying the required sta<strong>ff</strong>/child ratios approved for tile<br />

room .<br />

•:. Safety gates sha<strong>ll</strong> be secured to separate groups of both the Infant Room & Toddler Room The safety gates<br />

must be secured to prevent being moved by children<br />

Evidence of corrections made sha<strong>ll</strong> be provided to CCFS Caron Ahlemeyer by May 20, 2011.<br />

Verification of corrections wi<strong>ll</strong> be filed in the public record as they occur and/or are verified.<br />
