ALLEGATIONS: Staff/child ratios are incorrect almost every day. Right now there are 6 infant/toddlers<br />
with 1 teacher and 3 infant/toddlers with one teacher.<br />
SUMMARY OF INVESTIGATION<br />
On April 27, 2011, I conducted an unannounced inspection at the facility. I observed the following<br />
staff/child ratios: 5 infant/toddlers with 1 teacher, 4 infant/toddlers through two's with 1 teacher and 6<br />
preschool children with 1 teacher. One related child was also present in the preschool unit. The<br />
staff/child ratios were corrected when another teacher arrived.<br />
On April 27, 2011, Inna Rittenhouse, infant/toddler teacher and Sydnie Ballew, preschool teacher gave<br />
the following consistent statements:<br />
• I don't know the correct staff/child ratios.<br />
• I have not read the licensing rules.<br />
On April 27, 2011, Meaghan Keagy and Elizabeth Hoock both infant/toddler teachers gave the following<br />
consistent statement:<br />
• Staff/child ratios are incorrect every day.<br />
On April 28, 2011, I contacted Carrie Johnsson, infant/toddler teacher by telephone and she gave the<br />
following information:<br />
• I have been employed 1 Y2 months.<br />
• Staff/child ratios are usually correct, but I was told they were not correct prior to my<br />
employment.<br />
On April 28, 2011, I contacted Felicia Long, infant/toddler teacher, by telephone and she gave the<br />
following information:<br />
• I have been employed since April 25, 201l.<br />
• On Monday or Tuesday I had 5 infant/toddlers by myself because someone was late for ork.<br />
On April 28, 2011, I contacted Cindy Thompson, preschool teacher by telephone and she gave th<br />
following information:<br />
• Staff/child ratios are correct unless someone is late for work.
On May 3, 2011, I conducted another unannounced inspection at the facility. I observed the following<br />
staff/child ratios: 6 infant/toddlers with 2 teachers, 5 two's with 1 teacher and 6 preschoolers with 1<br />
teacher. Two children age toddler-two years were on a field trip along with 6 preschool age children and<br />
2 teachers at the discovery center located at 1395 Triad Center Dr.<br />
RULES VIOLATED<br />
Based on the investigation the allegations are substantiated. This is a violation ofthe following licensing<br />
rule:<br />
19 CSR30-62.112 (1) (A): Which states-liThe following staff/child ratios shall be maintained on t e<br />
premises at all times: Birth Through Two (2) Years. Groups composed of mixed ages through two 2)<br />
years shall have no less than one (1) adult to four (4) children, with no more than eight (8) childre in a<br />
group."<br />
On April 27, 2011, during an unannounced inspection, I observed 5 infant/toddlers with 1 teacher.<br />
CORRECTIONS<br />

2. Additional unannounced inspections will be conducted.<br />
