This licensing rule violation is based on statements made by Maurice Phipps and' Tiara Marbley. Mr.<br />

Phipps stated that he did not count the children as they entered the van on February 7,2012. Ms.<br />

Marbley said that she and Mr. Phipps picked up between 15 and 18 children that morning. Both Mr.<br />

Phipps and -Ms. Marbley stated,that they did not count the children when they took them inside.<br />

19 CSR30-62.222 (l)which states~ "The child care provider shall maintain accurate records to meet<br />

administrative requirements and to ensure knowledge of the individual-needs of children and their<br />

fainilies."<br />

This licensing rule violation is based on statements made by Maurice Phipps, Tiara Marbley and<br />

Lavonder Torrence: Maurice Phipps and "TiaraMarbley stated they sign children in when they pick them<br />

up, however, children are not signed out until the children are taken home. _SCCR staff requested to see<br />

the sign in sheet that is maintain in the van, however, it was not produced. Ms. Torrence stated that she<br />

takes attendance ror the facility during the day. She stated that she ~alks through the classrooms and .<br />

takes a head count and then by memory, Ms. Torrence marks a "P" on the attendance sheet to indicate a<br />

child is present on the sheet that she keeps in her office. Ms. Torrence stated that she did not take<br />

attendance on Febrqary 7, 2012.<br />

APPEAL PROCEDURE<br />

You may appeal the decision to revoke your license application by requesting a hearing<br />

before the Administrative Hearing Commission (Commission). To receive a hearing, the Missouri<br />

Department of Health and Senior Services, Section for Child Care Regulation, must receive a<br />

written request within thirty (30) days of the date of this letter. Please direct the request to the<br />

attention of Sue Porting, Assistant Section Administrator, P.O. Box 570, Jefferson City,<br />

Missouri 65102. The fax number for Ms. Porting is (573) 526-5345.<br />
