Tips <strong>for</strong> Recorders Using Screening Forms<br />
Thank you <strong>for</strong> your time and help in collecting in<strong>for</strong>mation on the oral health status of <strong>Special</strong> <strong>Olympics</strong><br />
athletes! Your ef<strong>for</strong>ts now in quality assurance will be very helpful after the event in ensuring complete<br />
in<strong>for</strong>mation and in analyzing the data. These analyzed data will be used by <strong>Special</strong> <strong>Olympics</strong> <strong>Special</strong> Smiles<br />
to learn more about the oral health status of athletes and, based on the findings, to direct advocacy ef<strong>for</strong>ts<br />
<strong>for</strong> people with special needs.<br />
Following are some useful guidelines and suggestions to help you fill out the Screening Form:<br />
It is very important that you collect and retain a Screening Form from each and every athlete, even if<br />
the person refused or could not be screened. Be<strong>for</strong>e starting to screen the athletes, know the<br />
designated place to store the completed <strong>for</strong>ms.<br />
Fill in the Screener s name or ID number.<br />
Coaches may be screened as a courtesy if they request it, but do not fill out Screening Forms <strong>for</strong><br />
anyone other than <strong>Special</strong> <strong>Olympics</strong> athletes. Athletes are the only group that data are being collected<br />
on at this time.<br />
You are the last person to see the Screening Form while the athlete is still present; there<strong>for</strong>e it is your<br />
responsibility to scan the <strong>for</strong>m and be sure that all areas are completed, even if some areas on the<br />
<strong>for</strong>m are assigned to be filled out by others at the site. The exception is the Screening section, left<br />
blank only if the athlete was edentulous or refused the screening. To identify these cases, you should<br />
fill out the No. 3, Athlete refused/could not screen, box, or on No. 4, Edentulous, mark Yes.<br />
