personal care plan for each<br />

individual, based on their <strong>and</strong><br />

their families’ needs <strong>and</strong><br />

agreed outcomes<br />

<strong>Barking</strong> <strong>and</strong> Dagenham will<br />

need to ensure that in<br />

completing these reviews that<br />

we “ ensure that all individuals<br />

have the information, advice<br />

<strong>and</strong> advocacy support they<br />

need to underst<strong>and</strong> <strong>and</strong> have<br />

the opportunity to express their<br />
