it is with great enthusiasm that i share with you the <strong>Annual</strong> report of the<br />
Department of Family medicine and Community Health at <strong>Case</strong> <strong>Western</strong><br />
reserve university School of medicine and university Hospitals <strong>Case</strong> medical<br />
Center. i am honored and privileged to serve as the department’s Chair over<br />
the last decade and to share with you the department’s accomplishments<br />
during the <strong>2011</strong>-<strong>2012</strong> academic year.<br />
One of the most exciting events of the academic year was the $25 million<br />
gift from the late Al Weatherhead and his wife, Celia, allowing us to establish<br />
the Weatherhead institute of Family medicine and Community Health. the<br />
institute will provide our medical students opportunities to understand the<br />
challenges of providing health care in urban and underserved areas, and the<br />
ability to trans<strong>for</strong>m community health in greater Cleveland and northeast Ohio.<br />
We are <strong>for</strong>tunate to have a large number of physicians recognized as top Docs in the Cleveland area and<br />
northeast Ohio. two of our physicians were specifically honored <strong>for</strong> their outstanding contributions in research<br />
and teaching. Kurt Stange, mD, PhD received the Curtis G. Hames research Award in Family medicine from the<br />
Society of teachers of Family medicine and masahiro morikawa, mD, mPH was selected as the Family Physician<br />
Educator of the year by the Ohio Academy of Family Physicians.<br />
in partnership with Care Alliance (a local federally-qualified health center), we received funding from the St. Luke’s<br />
Foundation to expand the residency program to Care Alliance. With support from the mt. Sinai foundation, we are<br />
able to offer a year-long series of health education topics to clients of the Hunger network of Greater Cleveland.<br />
in its seventh year, our House Calls program is thriving and we are able to provide services in two underserved<br />
urban communities, East Cleveland and the Slavic Village neighborhood.<br />
the department continues its national reputation <strong>for</strong> excellence. U.S. News & World <strong>Report</strong> ranked the Department<br />
of Family medicine and Community Health 12th and our Geriatrics Center 27th of best hospitals in the country.<br />
these rankings honor the exceptional education, research and services that the department provides. Our niH<br />
ranking among departments of Family medicine remains at 14.<br />
Health care is at a time of great change nationally. Primary care, in particular, will be at the <strong>for</strong>efront of the<br />
trans<strong>for</strong>mation of patient care, prevention, and health education. i am confident that the Department is well<br />
positioned to not just contribute to the advances and changes in our health system, but to be a leader in this<br />
trans<strong>for</strong>mation.<br />

$25 Million Commitment to<br />
school of Medicine from al and celia Weatherhead<br />
it is a great honor to announce that<br />
the <strong>Case</strong> <strong>Western</strong> reserve university<br />
School of medicine received a $25<br />
million commitment to launch an<br />
initiative dedicated to trans<strong>for</strong>ming<br />
community health in greater Cleveland<br />
and, ultimately, around the world.<br />
During the October <strong>2011</strong> public launch<br />
of Forward thinking: the Campaign<br />
<strong>for</strong> <strong>Case</strong> <strong>Western</strong> reserve university,<br />
President Barbara r. Snyder shared<br />
that the late Al Weatherhead and his<br />
wife, Celia, agreed to pledge $50<br />
million to support programs at the<br />
Weatherhead School of management<br />
and the School of medicine. the<br />
casemed.case.edu/fammed<br />
couple they wanted the funds<br />
divided equally between the two<br />
schools. With this pledge, the<br />
School of medicine founded the<br />
Weatherhead institute of Family<br />
medicine and Community Health.<br />
Al Weatherhead, a remarkably<br />
successful Cleveland-area businessman<br />
and generous philanthropist,<br />
died September 20, at the age of 86.<br />
His wife, Celia, accepted the university<br />
medal on her husband’s behalf<br />
during the gift announcement. Dr.<br />
George Kikano, the Dorothy Jones<br />
Weatherhead Professor of Family<br />
medicine and Chair of the Department<br />
of Family medicine (now known as<br />
the Department of Family medicine<br />
and Community Health), developed<br />
the model <strong>for</strong> the institute and<br />
serves as its first leader. the School<br />
of medicine’s family medicine faculty<br />
are among the nation’s best, and now<br />
will be part of an ambitious new ef<strong>for</strong>t<br />
to coordinate many outstanding community<br />
initiatives into a comprehensive<br />
whole – and in turn combine them with<br />
innovative programs of education,<br />
outreach, analytical research and<br />
policy advocacy.

the institute will build upon already<br />
robust academic programs that<br />
provide our medical students<br />
opportunities to understand the<br />
unique challenges of providing health<br />
care in urban areas. the new institute<br />
will also engage faculty already active<br />
in research on the large-scale impact<br />
of local interventions to address<br />
childhood hypertension, youth obesity,<br />
diabetes and other chronic diseases.<br />
The institute’s ultimate goals are<br />
to develop:<br />
– new generations of true physicianleaders<br />
actively engaged in direct<br />
care and rigorous research;<br />
– extensive studies rooted in sophisticated<br />
analysis of huge streams of<br />
community health data; and<br />
– advocacy initiatives that draw on<br />
the institute’s programs to support<br />
health policy re<strong>for</strong>ms that enhance<br />
patient outcomes even as they<br />
reduce overall costs.<br />

“the potential of this institute<br />
to advance our understanding of<br />
community health is extraordinary,”<br />
says School of medicine Pamela<br />
B. Davis, mD, PhD. “We are<br />
deeply grateful to Albert and<br />
Celia Weatherhead, and absolutely<br />
dedicated to realizing their great<br />
hopes <strong>for</strong> this ef<strong>for</strong>t.”<br />
the health disparities between<br />
Cleveland’s urban residents and<br />
their suburban neighbors are<br />
striking. twenty six percent of<br />
city-dwellers lack health coverage,<br />
compared with 13 percent countywide,<br />
according to Center <strong>for</strong><br />
Disease Control and Prevention<br />
statistics. Obesity rates are also<br />
higher among urban residents, as<br />
are rates of many chronic diseases.<br />
the institute will help the university<br />
maximize the impact of its current<br />
initiatives and expand its program<br />
offerings to address these issues<br />
through direct community outreach,<br />
population health research and<br />
medical education.<br />
“this institute will enable us<br />
to draw more students into<br />
primary care, prepare them<br />
to serve as leaders of their<br />
organizations and community<br />
advocates, and, at the same<br />
time, support communitybased<br />
research programs<br />
and gather the data necessary<br />
to affect policy changes<br />
that help populations and<br />
communities,” Kikano says.<br />

oafP recognizes Dr. Masahiro Morikawa as<br />
family Medicine educator of the Year<br />
Dr. masahiro morikawa received the <strong>2011</strong> Family medicine Educator of<br />
the year Award from the Ohio Academy of Family Physicians.<br />
Dr. morikawa has been practicing<br />
family medicine <strong>for</strong> 24 years<br />
with areas of interest that include:<br />
in-patient management, global<br />
health, bedside education and<br />
primary care in post-conflict<br />
communities. He serves many<br />
roles at university Hospitals <strong>Case</strong><br />
medical Center, including Course<br />
Director <strong>for</strong> the annual internal<br />
track workshop, Director <strong>for</strong><br />
inpatient Service and Director<br />
of the Global Health track.<br />
As director of the Global Health<br />
track <strong>for</strong> the residency program,<br />
Dr. morikawa developed an<br />
integrated global health curriculum<br />
including a yearly two-week<br />
workshop on global health, and<br />
recurrent journal clubs. Dr. morikawa<br />

is also responsible <strong>for</strong> creating a<br />
600-plus page global health<br />
handbook entitled international<br />
Health Handbook: Guidebook to<br />
Family medicine international<br />
Health track that contains core<br />
content, key resources and<br />
bibliographic material.<br />
Among other factors, Dr. morikawa<br />
was chosen <strong>for</strong> his experience and<br />
expertise abroad. He has traveled<br />
to Afghanistan, Vietnam, Guatemala,<br />
nepal, Honduras and Balkan<br />
training and sharing his knowledge<br />
and skills with family physicians<br />
and health care workers.<br />
“Dr. morikawa is an inspirational<br />
leader in that he not only talks the<br />
talk, he walks the walk. He is not<br />
only a strong educator and clinician,<br />
but he is a visionary leader in the<br />
education of learners in family<br />
medicine. His creativity, productivity<br />
and excellence in teaching learners<br />
of all levels is commendable, but his<br />
selfless commitment to the health<br />
and well-being of others makes him<br />
a role model <strong>for</strong> students, residents<br />
and fellow faculty,” said George<br />

According to one of Dr. morikawa’s<br />
students, his philosophy on patient<br />
care and commitment to best<br />
clinical outcomes is the reason<br />
why people truly trust him with<br />
their health and families.<br />
“i have yet to meet another<br />
clinician in my years of both<br />
general surgery and family medicine<br />
residency who makes rounds<br />
every day without fail at 3 a.m.;<br />
actually hand feeds patients; makes<br />
each and every day, each and every<br />
patient a learning experience; finds<br />
the time to mentor residents both<br />
in and out of his department, and<br />
continues to do this day in day out<br />
without fail <strong>for</strong> more than ten<br />
years,” explained Dr. Kerry Lecky,<br />
a past resident of Dr. morikawa.<br />
not only has Dr. morikawa had<br />
an impact overseas sharing his<br />
knowledge to the world, he has<br />
been a role model <strong>for</strong> his students<br />
and residents in Ohio.<br />
“Whether we had 30 seconds or<br />
an hour, he taught,” explained<br />
Dr. Hisam Goueli, a past student<br />
of Dr. morikawa. “He expanded<br />
my knowledge and challenged<br />
my understanding of medical<br />
concepts every time.”

Award in Family medicine honors<br />
those individuals whose careers<br />
over the years exemplify dedication<br />
to research in family medicine. in<br />
<strong>2012</strong>, the prestigious award went<br />
to Kurt Stange, mD, PhD, a family<br />
physician and epidemiologist with<br />
an active clinical practice at university<br />
Hospitals <strong>Case</strong> medical Center.<br />
At <strong>Case</strong> <strong>Western</strong> reserve university<br />
School of medicine, he is the<br />
Gertrude Donnelly Hess, mD,<br />
Professor of Oncology research,<br />
and professor of family medicine,<br />
epidemiology and biostatistics,<br />
oncology, and sociology. Dr. Stange<br />
is an American Cancer Society clinical<br />
research professor.<br />
He serves as editor <strong>for</strong> the peerreviewed,<br />
indexed, primary care<br />
research journal, the Annals of<br />
Family medicine, and directs the<br />
multi-site Center <strong>for</strong> research in<br />
Family Practice and Primary Care,<br />
one of three research centers<br />
funded by the American Academy<br />
of Family Physicians. Dr. Stange is<br />
actively engaged in ongoing basic<br />
and applied research that aims to<br />
<strong>2012</strong> Society of<br />

Each year, Crain’s Cleveland<br />
recognizes dedicated professionals<br />
of northeast Ohio’s medical and<br />
wellness communities <strong>for</strong> the<br />
difference they make in the region,<br />
economy and in the quality of<br />
everyday lives. in may <strong>2012</strong>,<br />
George E. Kikano, mD, was named<br />
a physician hero. He has served<br />
patients <strong>for</strong> more than two decades<br />
while championing research<br />
and programs that create ways to<br />
provide better patient care at less<br />
expense and has worked to attract<br />
more students to the family<br />
medicine profession.<br />
in October <strong>2011</strong>, George E. Kikano,<br />
mD, became <strong>Case</strong> <strong>Western</strong> reserve<br />
university School of medicine’s first<br />
Vice Dean <strong>for</strong> Community Health.<br />
in this role, Dr. Kikano is charged<br />
with leading the school’s new<br />
Weatherhead institute <strong>for</strong> Family<br />
medicine and Community Health,<br />
which is being established through<br />
the generosity of Celia and the late<br />
Al Weatherhead. Dr. Kikano also<br />
serves as the Dorothy Jones<br />
Weatherhead Professor at <strong>Case</strong><br />
<strong>Western</strong> reserve university and<br />
Chairman of the Department of<br />
Family medicine and Community<br />
Health at <strong>Case</strong> <strong>Western</strong> reserve<br />
university School of medicine<br />
and university Hospitals <strong>Case</strong><br />
medical Center.<br />
From the start, Dr. Kikano strived<br />
to inspire students to see the great<br />
opportunities in community health,<br />
casemed.case.edu/fammed<br />
to create models that enable<br />
homebound patients to receive the<br />
treatments that prevent hospitalizations,<br />
to persuade policymakers<br />
that common-sense re<strong>for</strong>ms<br />
benefit those in need and the<br />
economic bottom line.<br />
in a decade as Chair of the Department<br />
of Family medicine (now the<br />
Department of Family medicine<br />
and Community Health), Dr. Kikano<br />
maintained existing strengths and<br />
added new layers of distinction.<br />
today the department ranks 12th in<br />
rankings from U.S. News & World<br />
<strong>Report</strong> and 14th in overall funding<br />
from the national institutes of<br />
Health. its faculty constantly seek<br />
new opportunities to prepare medical<br />
students and residents to serve<br />
vulnerable populations and identify<br />
the most effective ways to better<br />
patient and community results.<br />
With the new Weatherhead<br />
institute, Dr. Kikano and his<br />
colleagues expand their work to<br />
involve faculty across the school and<br />
university. Among the programs<br />
under the institute’s umbrella are<br />
the urban Health initiative, the<br />

Prevention research Center <strong>for</strong><br />
Healthy neighborhoods, education<br />
initiatives focused on training<br />
primary care physicians and<br />
epidemiology/biostatistics-based<br />
research programs.<br />
One flagship program led by<br />
Dr. Kikano is the “House Calls”<br />
program, an ef<strong>for</strong>t from the<br />
Department of Family medicine<br />
and Community Health to maintain<br />
elders’ health so that they may<br />
remain in their own homes, rather<br />
than being placed in nursing home<br />
facilities. Dr. Kikano has also<br />
overseen the development of an<br />
urban medicine track within the<br />
medical school curriculum to<br />
specifically engage students to<br />
provide family medicine or primary<br />
care in urban environments.<br />
Since 2002, Dr. Kikano has been<br />
annually recognized as one of the<br />
Best Doctors in America; as one of<br />
northeast Ohio’s top Docs in northern<br />
Ohio Live magazine and Cleveland<br />
magazine; and has received many<br />
other teaching awards and honors.
stephen Zyzanski, PhD: recognizing More than<br />
30 Years of Mentorship and research<br />
June 1 and 2, <strong>2012</strong>, the Department of Family medicine and Community<br />
Health hosted a scientific symposium to honor Stephen Zyzanski, PhD,<br />
who will be transitioning to Emeritus Professor in the Fall of <strong>2012</strong>.<br />
Dr. Zyzanski is one of family<br />
medicine’s most productive<br />
researchers nationally. throughout<br />
his long and distinguished career,<br />
he has contributed significantly to<br />
published literature on a host of<br />
important subjects, including<br />
behavioral risk factors and coronary<br />
artery disease, community-oriented<br />
primary care, family systems<br />
health issues, mental health and<br />
substance abuse in primary care<br />
and clinical preventive services.<br />
Dr. Zyzanski joined the<br />
Department of Family medicine<br />
in 1979. He served as the Director<br />
of research in the Department<br />
until 2008. Dr. Zyzanski’s work,<br />
along with that of his colleagues,<br />
led to <strong>Case</strong> <strong>Western</strong> reserve<br />
university School of medicine<br />
developing its widely recognized<br />
reputation as one of the premier<br />
Family medicine research groups<br />
in the country. Due to his reputation<br />
as a world-class mentor and<br />
methodologist, fellows have<br />
continued to seek out Dr. Zyzanski<br />
<strong>for</strong> research training. many fellowship<br />
graduates are now prominent<br />
family medicine researchers,<br />
academics and leaders around<br />
the country.<br />
Steve’s unwavering optimism<br />
and enthusiasm <strong>for</strong> answering<br />
important research questions<br />
illuminates conversations about<br />
research aims and analyses. the<br />
excitement and positive energy<br />
during consultations with Steve<br />
help to revitalize the fun of doing<br />
research, contributing to discovery<br />
and propelling novice and experienced<br />

is deemed paramount in seeking<br />
to cure, relieve and com<strong>for</strong>t. Dr.<br />
thomas mettee’s career speaks<br />
to this very issue. tom made a<br />
concerted ef<strong>for</strong>t every day to<br />
share his vast practical skills and<br />
knowledge, while imparting<br />
wisdom gleaned from the ages.<br />
tom represented the essence<br />
of a physician, friend, role model<br />
and teacher.<br />
tom joined the newly established<br />
<strong>Western</strong> reserve university<br />
Department of Family medicine<br />
in 1978. Shortly thereafter, he<br />
founded Chesterland Family<br />
Practice in Geauga County.<br />

work, and synergistic collaboration<br />
among doctors, staff, nurses,<br />
patients, families and caregivers.<br />
tom mentored students because<br />
he cared deeply about educating<br />
the physicians of tomorrow. His<br />
intellect, compassion and ever<br />
caring manner, with his diverse<br />
patient clientele, especially the<br />
homebound, influenced many.<br />

of teaching hours and countless<br />
hours of care: tom truly believed<br />
and lived the Hippocratic Oath.<br />
tom’s ability to understand other’s<br />
feelings was unsurpassed. His<br />
endless knowledge, his touch of<br />
reassurance, his soothing words of<br />
com<strong>for</strong>t and always a bit of humor<br />
were amazing qualities he applied<br />
to illness and suffering. this made<br />
him extraordinary. He was truly a<br />
master of his trade. He was often<br />
heard saying, “in my opinion, the<br />
only way you learn medicine is at<br />
the elbow of a mentor.”<br />
tom was the first recipient of the<br />
<strong>Case</strong> <strong>Western</strong> reserve School of<br />
medicine’s mettee Educational<br />
Award, honoring doctors who<br />
demonstrate a passion <strong>for</strong> medicine.<br />
An endowment fund is also<br />
being created to sustain the<br />
thomas muther mettee <strong>Annual</strong><br />
Lectureship in Family medicine,<br />
an educational venue <strong>for</strong> students<br />
from the Schools of medicine,<br />
nursing and Social Sciences.<br />
We will all, each one of us, pass<br />
on a more tender kindness and<br />
compassion because we have<br />
been graced by his knowledge,<br />
wisdom, care and values. When his<br />
students would say, “Doc, you are<br />
amazing, and i will never <strong>for</strong>get this<br />
time with you.” His response was<br />
always, “you’d better not!” And<br />

Urban Health inititative launches<br />
in June, <strong>2011</strong> Pamela Davis, mD,<br />

Affairs, established the urban<br />
Health initiative (uHi). uHi aims to<br />
improve the health of Cleveland<br />
area residents by leveraging the<br />
institution’s research, education and<br />
service resources. By increasing<br />
attention to the social and environmental<br />
factors that affect the ability<br />
of residents to lead healthy lives,<br />
seek and obtain health care, and<br />
manage their illnesses, uHi focuses<br />
on issues that can improve health<br />
of the area population and engage<br />
more residents in the process.<br />

through establishing more and<br />
stronger partnerships among<br />
faculty, staff and students across<br />
the university; and with area<br />
government organizations, health<br />

Amy Sheon, PhD, mPH was<br />
recruited to serve as Executive<br />
Director. With an adjunct appointment<br />
in the Department of Family<br />
medicine, Sheon brings two<br />
decades of experience addressing<br />
urban health issues including HiV<br />
prevention at the national institutes<br />
of Health, establishing multidisci-<br />
plinary health research centers at<br />
the university of michigan, and<br />
developing an obesity surveillance<br />
system <strong>for</strong> the State of michigan.<br />
in its first year, the uHi has defined<br />
a mission and core strategies <strong>for</strong><br />
better leveraging the research,<br />
health profession education, and<br />
service assets of the university<br />
and its clinical affiliates. the<br />
program’s website and listserve<br />
(http://urbanhealth.case.edu) help<br />
to publicize activities and connect<br />
students, faculty and community<br />
programs. in the works is a searchable<br />
inventory of partnerships<br />
between the university and the<br />
community to enable students<br />
and trainees to find opportunities<br />
of interest <strong>for</strong> clinical training or<br />
research projects. in partnership<br />
with the Clinical and translational<br />
Science Consortium, uHi offered a<br />
competition <strong>for</strong> urban health pilot<br />
grants. One of the three awardees<br />
is in the Department of Family<br />
medicine and Community Health.<br />
Dr. Lynda Montgomery, Department of Family Medicine and Community Health, “Learning Leadership through<br />
Community Engagement: the Improve Children’s Health Access Project”<br />
the national Coordinator of youth leadership and Development <strong>for</strong> the Children’s Defense Fund will connect teams<br />
of 1st and 2nd year medical students with neighborhood organizations where they will gain first-hand understanding<br />
of barriers children face in seeking health care and living healthy lives. the program will increase medical students’<br />
awareness of the need <strong>for</strong> health care in underserved urban areas. Such experiences have been shown to increase<br />
the likelihood that medical students will continue their work with underserved populations after graduation.

researcH<br />
Division<br />
the research division continues to grow and mature.<br />
Our primary goals are to improve the health of<br />
individuals, practices and local communities. this is<br />
accomplished by conducting research that spans from<br />
basic science to intervention development and implementation<br />
science. Focal areas include research to<br />
improve physician-patient communication, facilitating<br />
practice engagement and change, raising awareness<br />
of individual risk to prompt change / action, developing<br />
web-based tools <strong>for</strong> family history, and conducting<br />
cancer epidemiology to understand how genetics,<br />
the environment and health behaviors change the<br />
likelihood of getting cancer.<br />
the eleven faculty members in the research Division<br />
had a strong year in terms of grant applications, grants<br />
awarded, and peer reviewed publications. those in the<br />
division submitted 31 grant applications during the<br />
<strong>2011</strong>-<strong>2012</strong> academic year. ten new research grants<br />
were funded during this period, and many of the<br />
proposals are still pending. there are 43 ongoing<br />
grants in the division, including investigator initiated<br />
research, research training grants and grants <strong>for</strong><br />
program leadership. Of the 43 funded grants, 25<br />
involve a family medicine faculty member as the<br />
principal investigator.<br />
Funded training programs include a post-doctoral<br />
research training program in Computational Genomic<br />
Epidemiology of Cancer, co-directed by Li Li, mD, PhD.<br />
James Werner, PhD, is leading a HrSA-funded training<br />
program to expand and improve faculty skills in<br />
behavioral medicine. Other faculty highlights during<br />
the past academic year include an editorial published<br />
in JAmA by Louise Acheson, mD, mS on the value of<br />
family history <strong>for</strong> cancer prevention, and an article by<br />
Cheryl thompson, PhD. in Cancer about short duration<br />
of sleep and the increased risk of colorectal adenoma.<br />
Our department continues as the editorial home of the<br />
Annals of Family medicine. Kurt Stange, mD, PhD is<br />
editor, and Stephen Zyzanski, PhD and Dr. Acheson<br />
serve as associate editors.<br />

training the next generation<br />
eDUcational<br />
Division<br />
the Educational Division of the Department of<br />
Family medicine and Community Health has had<br />
another successful year. the two areas within the<br />
Division, Predoctoral Education and residency<br />
Education, each presented programming and projects<br />
that culminated in the graduation of strong future<br />
family medicine physicians.<br />
Predoctoral Education was awarded a training in Primary<br />
Care grant from Health resources and Services<br />
Administration. the goal is to prepare students to care<br />
<strong>for</strong> underserved populations in urban settings. the<br />
Predoctoral Education faculty implemented its summer<br />
fellows program, Leadership and Advocacy in urban<br />
and Community Health (LAunCH). medical and<br />
pharmacy students worked together in implementing<br />
a needs assessment with Harbor Light, a facility in<br />
downtown Cleveland that provides services to the<br />
most disenfranchised members of society, including<br />
homeless men, women and children, those with<br />
substance abuse issues, and those making the<br />
transition from the penal system back to society.<br />
Additionally, the Family medicine interest Group held<br />
many popular sessions <strong>for</strong> the entire student body,<br />
including “Strolling through the match,” af<strong>for</strong>ding a<br />
career in primary care, and clinical skills workshops.<br />
With over 15 years of service to the department,<br />
Al Cadesky, mD, the residency Director, decided to<br />
accept a position in a group practice in Cleveland.<br />
Wanda Cruz-Knight, mD, has assumed the position<br />
of Program Director of the residency Program.<br />

replacing Dr. navracruz as the Associate Program<br />
Director is rasai Ernst, mD, a graduate of our program<br />
and fellowship trained in academic medicine. Sonja<br />
Harris-Haywood, mD, mS, assumed the role of Director<br />
of Predoctoral Education. this new leadership team<br />
promises to take our educational programs to even<br />
greater heights.<br />
Lisa navracruz, mD, assumed the directorship of our<br />
ef<strong>for</strong>ts to expand our residency training program to<br />
Care Alliance, a Federally Qualified Health Center<br />
(FQHC) that provides primary medical care, dental<br />
care and supportive services to individuals living in<br />
extreme poverty in Cleveland, Ohio. this project is<br />
supported by a generous grant from the St. Luke’s<br />
Foundation. During the <strong>2012</strong>-2013, the residency<br />
program will be recruiting two individuals who see<br />
continuity patients in the Care Alliance Clinic. Additionally,<br />
the Preventive medicine residency program,<br />
directed by Kurt Stange, mD, PhD, continued to<br />
thrive. this program combines classroom and experiential<br />
learning in Preventive medicine and engages<br />
both the County Board of Health and the Cleveland<br />
Health Department as key collaborators in training.<br />
interweaving this training with the Family medicine<br />
and Community Health residency program results in a<br />
4 year program and dual board eligibility. the Preventive<br />
medicine residency program has been highly<br />
successful and the first graduate, Johnie rose, mD,<br />
PhD, joined the Department faculty in <strong>2011</strong>.
geriatric Medicine in the community<br />
the Center is a collaborative ef<strong>for</strong>t<br />
between the Department of Family<br />
medicine and Community Health,<br />
the Department of medicine, and<br />
the Department of internal medicine.<br />
it encompasses geriatric<br />
medicine and palliative care<br />
services at university Hospitals<br />
and is responsible <strong>for</strong> the education<br />
and training of <strong>Case</strong><strong>Western</strong><br />
reserve health professional<br />
students and university Hospitals<br />
residents in the fields of geriatric<br />
medicine and palliative care.<br />
this year, we witnessed the<br />
continued growth of the uH/<strong>Case</strong><br />
House Calls program (which now<br />
provides home-based medical<br />
care to over 400 older patients),<br />
the mcGregor PACE program<br />
(a medical home <strong>for</strong> frail elders<br />
and people with chronic diseases<br />
who are eligible <strong>for</strong> nursing home<br />
placement but live in the commu-<br />
the university Hospitals Center <strong>for</strong> Geriatric medicine is a national<br />
leader in clinical services <strong>for</strong> older adults who are frail or have chronic<br />
diseases. Again this year, uH was ranked by US News and World<br />
<strong>Report</strong> as one of the top 30 geriatric medicine programs in the<br />
country. the Center <strong>for</strong> Geriatric medicine is led by Peter<br />
DeGolia, mD, CmD, the Jack H. medalie Endowed<br />
Professor in Home-Centered Health Care at the<br />
<strong>Case</strong> <strong>Western</strong> reserve School of medicine, and<br />
Director of the Center, and Lori Lozier, LiSW,<br />
LnHA, CHPCA, Director of Post Acute Services,<br />
Geriatric medicine, and Palliative Care.<br />
nity), and the uHCmC Palliative<br />
Care Consultation Service (which<br />
encountered over 700 new palliative<br />
care consults this past year,<br />
in an interdisciplinary team of<br />
physicians, nurses, and support<br />
professionals). We opened a new<br />
outpatient geriatric medicine<br />
service at the uH Otis moss<br />
Health Center in Cleveland’s Fairfax<br />
neighborhood. in addition, the<br />
mcGregor PACE program became<br />
a training site <strong>for</strong> students and<br />
residents to learn the new model<br />
of care provided in a medical home<br />
setting. this year, the Center staff<br />
began to work with uH richmond<br />
Family medicine and internal<br />
medicine programs to provide<br />
education and training in geriatric<br />
medicine and palliative care. Also,<br />
university Foley ElderHealth Center<br />
serves a frail older population<br />
experiencing geriatric syndromes<br />
such as dementia, polypharmacy,<br />
depression, and falls.<br />
the center sponsored two key<br />
symposia on the care of older<br />
hospitalized adults. they highlighted<br />
the center’s work in promoting<br />
and expanding the niCHE program<br />
(nurses improving Care <strong>for</strong> Healthsystem<br />
Elders) throughout the<br />
university Hospitals system. in<br />
fact, through the leadership of<br />
Catherine roscoe-Herbert, DnP,<br />
this program was a national leader<br />
in staff development. Our symposium<br />
on palliative care emphasized<br />
the growing movement toward<br />
developing programs that address<br />
suffering of acutely ill and terminal<br />
hospitalized patients as well as<br />
those who care <strong>for</strong> them.<br />
