REGISTRATION NOTICE<br />

Applications are invited for registration <strong>of</strong> contractors in the<br />

Dehradun Cirle <strong>of</strong> <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> (A.S.I.) for execution<br />

<strong>of</strong> works in and around the monument and supply <strong>of</strong> material for the<br />

maintenance <strong>of</strong> the monuments. The registration <strong>of</strong> contractors are<br />

invited under two categories- for works upto Rs.25.00 lacs and upto<br />

Rs.15.00 lacs. Enlistment processing fee amounting to Rs.4,000/- and<br />

Rs.3,000/- is to be deposited for category I and Cagetory II respectively.<br />

A Bankers certificate <strong>of</strong> Rs.16.00 lacs and Rs.10.50 lacs is also to be<br />

submitted with the application for category I & category II respectively.<br />

The contractors must have an experience <strong>of</strong> at least three works each<br />

above Rs.6.00 lacs during the last five years with the A.S.I. for category<br />

upto Rs.25.00 lacs and at least three works each above Rs.3.00 lacs<br />

during last five years with the A.S.I. for category upto Rs.15.00 lacs.<br />

The application form may be obtained from the Office <strong>of</strong> the<br />

Superintending Archaeologist, <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong>,<br />

Dehradun Circle, “Dharohar”, Tyagi Road, Dehradun on or before 10 st<br />

May, 2013 at the cost <strong>of</strong> Rs.500/- each (non-refundable). The forms can<br />

be downloaded from the website (www.asidehraduncircle.in) and be<br />

submitted along with a draft <strong>of</strong> Rs.500/-.The application form, duly<br />

filled, along with necessary documents as detailed in the application<br />

form must be submitted to the <strong>of</strong>fice <strong>of</strong> the Superintending<br />

Archaeologist, <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong>, , Dehradun Circle,<br />

“Dharohar”, Tyagi Road, Dehradun latest by 15 th May, 2013. It is<br />

clarified that mere making an application for registration shall not<br />

guarantee him/her to be accepted as a contractor/supplier. The<br />

undersigned reserves the rights to accept or reject any application<br />

without assigning any reason.<br />

(8) Whether the applicant has a permanent Engineering Organization capable <strong>of</strong> dealing with large<br />

works in all their stages and has sufficient machinery and tools etc. at their disposal (Full<br />

details <strong>of</strong> organization and machinery and tools to be given).<br />

(9) Whether the applicant has any <strong>of</strong>ficer in charge or a Guaduate Engineer with not less than ten<br />

years experience to deal with works in all their stages. If so, give particulars.<br />

(10) Whether the applicant is a licensed plumber or has a licensed plumber under his<br />

employment (Attested copy <strong>of</strong> plumber’s license to be enclosed).<br />

(11) Whether the applicant has a properly fitted workshop with sufficient large stocks <strong>of</strong> timber,<br />

paints, polishes and other finishing equipment and employs designers capable <strong>of</strong> producing<br />

well thought out schemes for large buildings. (Full details and location <strong>of</strong> their workshop<br />

may be furnished.)<br />

(12) Whether the applicant owns a good workshop; with sufficient large stocks <strong>of</strong> timber and<br />

has management for producing good quality <strong>of</strong> furniture and arrangements for good<br />

quality polishing and painting. (Full details and location <strong>of</strong> the workshop may be<br />

furnished).<br />

(13) Whether the applicant possesses a valid License and has an <strong>of</strong>fice with sufficient technical<br />

personnel capable <strong>of</strong> preparing estimates and submitting tenders and bills for works done and<br />

supervision <strong>of</strong> works. (Full details to be furnished). (Attested copy <strong>of</strong> Elect. License to be<br />

enclosed).<br />

(14) Whether the applicant has a valid Elect. License. (Attested copy <strong>of</strong> the License to be enclosed).<br />

(15) Whether the applicant is already enlisted in the <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong>, if so, in which<br />

class and category.<br />

(16) Whether the applicant is a share-holder or partner <strong>of</strong> any firm enlisted in this Department.<br />

(17) Whether the applicant or any <strong>of</strong> his partner or share holders is/are member(s) <strong>of</strong> the <strong>India</strong>n<br />

Parliament or any State Legislature?<br />

(18) Whether the applicant or any <strong>of</strong> his partners or share-holders is/are dismissed Govt.<br />

Servants(s)<br />

(19) Has the applicant or any <strong>of</strong> his partners or share-holders been blacklisted, or removed from the<br />

approved list <strong>of</strong> contractors, or demoted to lower class or orders passed banning/suspending<br />

business with the applicant etc. by any Deptt. in the past.<br />

(20) Whether the applicant has a dismissed Govt. servant under his employment.<br />

(21) Whether the contractor has read the rules for enlistment <strong>of</strong> contractors and is prepared to<br />

abide by them ?<br />

(22) A list <strong>of</strong> those persons who are working with the applicant in any capacity and who are near<br />

relatives to the <strong>of</strong>ficers in the <strong>Archaeological</strong> survey <strong>of</strong> <strong>India</strong> or in the Ministry should be<br />

submitted. The applicant should also intimate the names <strong>of</strong> such persons who are employed by<br />

him subsequently?


(23) The contractors are not entitled to tender for works in the Circle in which their near<br />

relative are working. (Any breach <strong>of</strong> this condition by any contractor would be sufficient<br />

to debar him from registration).(by the term near relative is meant wife, husband, parents<br />

and grand parents, children and grand children, brothers and sisters, uncles, aunts and<br />

cousins and their corresponding in-laws.)<br />

(24) I/We agree to notify the Officer accepting this application and registering my/our names<br />

on the <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> list <strong>of</strong> contractors <strong>of</strong> any changes in the foregoing<br />

particulars as they occur and to verify and confirm these particulars annually on 1 st<br />

January.<br />

(25) I/We agree to execute a BOND in favour <strong>of</strong> the President <strong>of</strong> <strong>India</strong> in the form and to the<br />

amount prescribed as security against proper performance <strong>of</strong> work entrusted to me/us for<br />

execution in accordance with the applicable conditions <strong>of</strong> contract, as a condition<br />

precedent to my/our name being registered on the <strong>Archaeological</strong> survey <strong>of</strong> <strong>India</strong> list <strong>of</strong><br />

contractors.<br />

(26) I/We understand and agree that the appropriate Authority <strong>of</strong> the <strong>Archaeological</strong> survey <strong>of</strong><br />

<strong>India</strong> has the right as he may decide, not to issue tender form in any particular case; also suspend,<br />

remove or blacklist my/our submission <strong>of</strong> non-bonafide tenders or for technical or other delinquency in<br />

regard to which the decision <strong>of</strong> the appropriate <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> Authority shall be final<br />

and conclusive.<br />

(27) I/We certify that the above particulars are correct and should it be found that I/We have<br />

given a false certificate <strong>of</strong> that if I/We fail to notify the fact <strong>of</strong> my/our subsequent<br />

amalgamation with another contractor or firm, and any contract that I/We may be holding<br />

at the time is liable to be rescinded.<br />

(28) I/We undertake not to employ directly or indirectly any Army or Civilian Personnel<br />

dismissed from services in my/our business.<br />

(29) I/We note that registration as <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> Contractor does not carry<br />

with it the right to tender for all <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> works.<br />

INSTRUCTION FOR COMPLETION OF FORM NO. ASIW-80 FOR ENROLMENT AS A<br />

CONTRACTOR<br />

1. Categories <strong>of</strong> works<br />

Categories <strong>of</strong> work for which enlistment is made are shown in opening part <strong>of</strong> the enrolment<br />

from. The categories <strong>of</strong> work for which the enlistment is not desired should be deleted with a single<br />

stroke <strong>of</strong> the pen.<br />

2. Particulars <strong>of</strong> contractors<br />

Photographs <strong>of</strong> the proprietor/partners and Managing Directors in the cases <strong>of</strong> Limited<br />

Concerns should be pasted in the space provided for this purpose in the enrolment form and if this<br />

space is insufficient the space at the bottom <strong>of</strong> page 2 may be used for this purpose.<br />

The photographs should be <strong>of</strong> Passport size and duly attested.<br />

The name <strong>of</strong> the person should be written/typed on his photograph.<br />

3. Particulars <strong>of</strong> Property<br />

(a) Certificate from a Scheduled Bank should be submitted in original in respect <strong>of</strong><br />

movable property. This will be retained in Government record.<br />

(b) Statement <strong>of</strong> Immovable property should be certified by a Deputy<br />

Commissioner/Collector or a First Class Magistrate and Should be submitted in<br />

original. This will be retained on Government record.<br />

4. Particulars <strong>of</strong> experience<br />

A list <strong>of</strong> works carried out during the last three years and work under execution, if any, should<br />

be submitted giving the following particulars in respect <strong>of</strong> each work:-<br />

(a) Name and description <strong>of</strong> work.<br />

(b) Contract Number and Year.<br />

(c) Value.<br />

(d) Department, Chief Engineer, Superintending Engineer and Executive Engineer under whom executed.<br />

(e) Date <strong>of</strong> completion.<br />

5. Particulars <strong>of</strong> Contractor’s Organization and Supervision Staff<br />

In addition to particulars to be given in para 4 and 5 <strong>of</strong> the enrolment form, the following are<br />

also to be submitted:-<br />

(a) A statement giving the details <strong>of</strong> tools and plant and transport owned by the contractor showing the<br />

registered numbers <strong>of</strong> the vehicles etc. with authentic evidence.<br />

(b) Original power <strong>of</strong> attorney in favour <strong>of</strong> the signatory to the enrolment form and in favour <strong>of</strong> the<br />

person/persons competent to enter into contracts and arbitration agreements on behalf <strong>of</strong> the<br />

contractor. An attested copy <strong>of</strong> the power <strong>of</strong> attorney on court stamp paper should also be submitted<br />

if the return <strong>of</strong> the original is desired.<br />

(c) In case <strong>of</strong> Sole Proprietor concern and affidavit in original to the effect that the applicant is the Sole<br />

Proprietor should be submitted. This will be retained in Govt. record.<br />

(d) In case <strong>of</strong> Partnership concern—<br />

(i) Partnership deed in original together with an attested copy <strong>of</strong> there<strong>of</strong> on court stamp paper.<br />

The original will be returned after verification.<br />

(ii) <strong>Registration</strong> certificate with the Registrar <strong>of</strong> companies, in original. This will be returned after<br />

verification, keeping an attested copy <strong>of</strong> the same.<br />

(iii) An attested copy <strong>of</strong> the audited balance sheet and the pr<strong>of</strong>it and loss account <strong>of</strong> the concern<br />

for the last two years.<br />

(e) In the case <strong>of</strong> limited concerns:-<br />

(i) Attested copy <strong>of</strong> the Memorandum & Article <strong>of</strong> Association.<br />

(ii) Attested copy <strong>of</strong> the Balance Sheet and Pr<strong>of</strong>it and loss Account <strong>of</strong> the previous year.


(iii) Attested copy <strong>of</strong> the Resolution passed by the Board <strong>of</strong> Directors appointing the Managing<br />

Director(s) with the power delegated to him on behalf <strong>of</strong> the firm.<br />

(f) In case enlistment is requested for manufacture <strong>of</strong> furniture etc. the following additional particulars<br />

should be furnished:-<br />

(i) Whether the applicant owns a workshop and/or show rooms for the furniture, if so complete<br />

address <strong>of</strong> workshop/show room be given.<br />

(ii) Whether the manufacture <strong>of</strong> furniture is applicant’s normal trade.<br />

(iii) List <strong>of</strong> furniture works executed by the applicant giving particulars as in para 4(a) to (e) above.<br />

(g) Names <strong>of</strong> permanent qualified technical staff with their technical qualification employed by the<br />

contractor.<br />

6. Income Tax Clearance and other Certificate<br />

(a) An Income Tax Clearance Certificate on the form given in the enrolment form is to be<br />

submitted in original. Without this the request for enlistment will not be considered.<br />

(b) Besides the above the Central Sales Tax and Local Sales Tax Certificates should also be<br />

furnished.<br />

7. Relations in the <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong>, In the Ministry <strong>of</strong> Education and<br />

Culture.<br />

A list <strong>of</strong> persons who are working with the contractor applicant and who are near relatives <strong>of</strong> Officer<br />

and staff in the <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> or any <strong>of</strong>ficer <strong>of</strong> the Ministry <strong>of</strong> Education & Culture<br />

should be submitted with the Enrollment form the full name, present <strong>of</strong>ficial address, residential<br />

address and the appointment held.<br />

OR<br />

The following certificates should be submitted duly signed by the Proprietor/all the partners in<br />

case <strong>of</strong> partnership concern/Managing Directors in the case <strong>of</strong> limited concern.<br />

8. A list <strong>of</strong> persons related to the contractors or the Directors/partners/owners <strong>of</strong> firms serving in<br />

the <strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong> or the Ministry <strong>of</strong> Education & Culture has to be attached, or the<br />

following Certificate may be submitted by the Contractor/firm:-<br />

In the event <strong>of</strong> the enlistment <strong>of</strong> *my/our name as approved contractor or the <strong>Archaeological</strong><br />

<strong>Survey</strong> <strong>of</strong> <strong>India</strong> we undertake to notify the name(s) <strong>of</strong> any such persons employed by us subsequently.<br />

*I/We also understand that we will not be entitled to tender for any works which our near relatives will<br />

be deal with or with which our relatives are connected in any way and any break <strong>of</strong> this condition may<br />

be considered sufficient for the removal <strong>of</strong> *my/our name from the list <strong>of</strong> approved contractor.<br />

Note:<br />

-1 By the term “near relative” is meant wife/husband, parents, children, brothers, sisters,<br />

brother/sister-in-law, sons/daughter-in-law, uncle, first cousins and their corresponding in-law.<br />

-2 Certificate/Credential etc.<br />

All certificate/credential etc. should be submitted in original together with two attested copies<br />

<strong>of</strong> each there<strong>of</strong> if the originals are desired to be returned. Copies should be attested by a First Class<br />

Magistrate or a Gazetted Officer with his court/<strong>of</strong>fice stamp attested thereon.

1. Memorandum and articles <strong>of</strong> Association/Partnership Deed/Affidavit.<br />

4. Power <strong>of</strong> Attorney (wherever necessary)<br />

5. Name <strong>of</strong> Bankers.<br />

6. Latest entry in Form “A” from the Register <strong>of</strong> the Registrar <strong>of</strong> Firms in the case <strong>of</strong><br />

Partnership concerns.<br />

7. Local Sales-tax certificate<br />

8. Central Sales-Tax Certificate.<br />

9. Photo copies <strong>of</strong> registration with C.P.W.D. or any other departments like Railways and<br />

M.E.S. etc., duly attested.<br />

10. A declaration to the effect as to whether the Contractor/firm was ever black-listed by any<br />

department <strong>of</strong> the Government <strong>of</strong> <strong>India</strong>, and if so, the reasons and duration <strong>of</strong> blacklisting<br />

have be to specified.<br />

11. Performance statement.<br />

12. Photographs <strong>of</strong> Partners/Owners <strong>of</strong> the firms.<br />

13. List <strong>of</strong> persons holding powers <strong>of</strong> attorney, together with their specimen signatures and<br />

photographs duly attested by a Gazetted <strong>of</strong>ficer.<br />

14. 14. Certificate <strong>of</strong> experience.<br />

15. List <strong>of</strong> supervising staff with their qualifications.<br />

16. Names <strong>of</strong> Govt. Departments in which registered as <strong>Contractors</strong>, together with attested<br />

copies <strong>of</strong> registration certificates.<br />

17. List <strong>of</strong> persons, who are related to the partners/owners <strong>of</strong> the firms, working in the<br />

<strong>Archaeological</strong> <strong>Survey</strong> <strong>of</strong> <strong>India</strong>.<br />

18. Copy <strong>of</strong> Licence for holding business.<br />

19. Copy <strong>of</strong> Electric Licence held by the firm.
