REGISTRATION NOTICE
Applications are invited for registration of contractors in the
Dehradun Cirle of Archaeological Survey of India (A.S.I.) for execution
of works in and around the monument and supply of material for the
maintenance of the monuments. The registration of contractors are
invited under two categories- for works upto Rs.25.00 lacs and upto
Rs.15.00 lacs. Enlistment processing fee amounting to Rs.4,000/- and
Rs.3,000/- is to be deposited for category I and Cagetory II respectively.
A Bankers certificate of Rs.16.00 lacs and Rs.10.50 lacs is also to be
submitted with the application for category I & category II respectively.
The contractors must have an experience of at least three works each
above Rs.6.00 lacs during the last five years with the A.S.I. for category
upto Rs.25.00 lacs and at least three works each above Rs.3.00 lacs
during last five years with the A.S.I. for category upto Rs.15.00 lacs.
The application form may be obtained from the Office of the
Superintending Archaeologist, Archaeological Survey of India,
Dehradun Circle, “Dharohar”, Tyagi Road, Dehradun on or before 10 st
May, 2013 at the cost of Rs.500/- each (non-refundable). The forms can
be downloaded from the website (www.asidehraduncircle.in) and be
submitted along with a draft of Rs.500/-.The application form, duly
filled, along with necessary documents as detailed in the application
form must be submitted to the office of the Superintending
Archaeologist, Archaeological Survey of India, , Dehradun Circle,
“Dharohar”, Tyagi Road, Dehradun latest by 15 th May, 2013. It is
clarified that mere making an application for registration shall not
guarantee him/her to be accepted as a contractor/supplier. The
undersigned reserves the rights to accept or reject any application
without assigning any reason.

APPLICATION FOR ENROLMENT OF CONTRACTOR IN THE ARCHAEOLOGIAL SURVEY
OF INDIA

types/categories of works/supply of materials upto the cost of Rs.25.00 Lacs /
Rs.15.00 Lacs and give below a statement of my/our capabilities and
resources.
1. Modern construction works/original works including sanitary fittings and electrical installation
etc. laying of approach roads, barbed wire fencing and connected jobs.
2. Supply of building materials, manure, machinery, tools & plants.
3. Earth work including leveling of ground for horticulture and other works etc. by use of earth
moving machinery or otherwise including transport of dismantled and other materials etc.
4. Stone work including artistic work on different kinds of stone viz., Red sand stone, granite,
marble etc.
5. Drainage & public health works, water supply.
6. Water proofing, marine works.
7. Supply of glass, teak, plywood and other materials required for use in spite Museums for
gallery furniture, paints and wood polish, snowcem etc.

(6) Income tax (An Income Tax Clearance Certificate in the prescribed proforma should accompany
this application.
(7) If you are at present enlisted with any other Government Department, formation etc. give:-
i) Name of formation
ii) Category, Limit and index no.
iii) Locality or area for which enlisted.
iv) Letter No. and date of enlistment.

(8) Whether the applicant has a permanent Engineering Organization capable of dealing with large
works in all their stages and has sufficient machinery and tools etc. at their disposal (Full
details of organization and machinery and tools to be given).
(9) Whether the applicant has any officer in charge or a Guaduate Engineer with not less than ten
years experience to deal with works in all their stages. If so, give particulars.
(10) Whether the applicant is a licensed plumber or has a licensed plumber under his
employment (Attested copy of plumber’s license to be enclosed).
(11) Whether the applicant has a properly fitted workshop with sufficient large stocks of timber,
paints, polishes and other finishing equipment and employs designers capable of producing
well thought out schemes for large buildings. (Full details and location of their workshop
may be furnished.)
(12) Whether the applicant owns a good workshop; with sufficient large stocks of timber and
has management for producing good quality of furniture and arrangements for good
quality polishing and painting. (Full details and location of the workshop may be
furnished).
(13) Whether the applicant possesses a valid License and has an office with sufficient technical
personnel capable of preparing estimates and submitting tenders and bills for works done and
supervision of works. (Full details to be furnished). (Attested copy of Elect. License to be
enclosed).
(14) Whether the applicant has a valid Elect. License. (Attested copy of the License to be enclosed).
(15) Whether the applicant is already enlisted in the Archaeological Survey of India, if so, in which
class and category.
(16) Whether the applicant is a share-holder or partner of any firm enlisted in this Department.
(17) Whether the applicant or any of his partner or share holders is/are member(s) of the Indian
Parliament or any State Legislature?
(18) Whether the applicant or any of his partners or share-holders is/are dismissed Govt.
Servants(s)
(19) Has the applicant or any of his partners or share-holders been blacklisted, or removed from the
approved list of contractors, or demoted to lower class or orders passed banning/suspending
business with the applicant etc. by any Deptt. in the past.
(20) Whether the applicant has a dismissed Govt. servant under his employment.
(21) Whether the contractor has read the rules for enlistment of contractors and is prepared to
abide by them ?
(22) A list of those persons who are working with the applicant in any capacity and who are near
relatives to the officers in the Archaeological survey of India or in the Ministry should be
submitted. The applicant should also intimate the names of such persons who are employed by
him subsequently?

(23) The contractors are not entitled to tender for works in the Circle in which their near
relative are working. (Any breach of this condition by any contractor would be sufficient
to debar him from registration).(by the term near relative is meant wife, husband, parents
and grand parents, children and grand children, brothers and sisters, uncles, aunts and
cousins and their corresponding in-laws.)
(24) I/We agree to notify the Officer accepting this application and registering my/our names
on the Archaeological Survey of India list of contractors of any changes in the foregoing
particulars as they occur and to verify and confirm these particulars annually on 1 st
January.
(25) I/We agree to execute a BOND in favour of the President of India in the form and to the
amount prescribed as security against proper performance of work entrusted to me/us for
execution in accordance with the applicable conditions of contract, as a condition
precedent to my/our name being registered on the Archaeological survey of India list of
contractors.
(26) I/We understand and agree that the appropriate Authority of the Archaeological survey of
India has the right as he may decide, not to issue tender form in any particular case; also suspend,
remove or blacklist my/our submission of non-bonafide tenders or for technical or other delinquency in
regard to which the decision of the appropriate Archaeological Survey of India Authority shall be final
and conclusive.
(27) I/We certify that the above particulars are correct and should it be found that I/We have
given a false certificate of that if I/We fail to notify the fact of my/our subsequent
amalgamation with another contractor or firm, and any contract that I/We may be holding
at the time is liable to be rescinded.
(28) I/We undertake not to employ directly or indirectly any Army or Civilian Personnel
dismissed from services in my/our business.
(29) I/We note that registration as Archaeological Survey of India Contractor does not carry
with it the right to tender for all Archaeological Survey of India works.

INSTRUCTION FOR COMPLETION OF FORM NO. ASIW-80 FOR ENROLMENT AS A
CONTRACTOR
1. Categories of works
Categories of work for which enlistment is made are shown in opening part of the enrolment
from. The categories of work for which the enlistment is not desired should be deleted with a single
stroke of the pen.
2. Particulars of contractors
Photographs of the proprietor/partners and Managing Directors in the cases of Limited
Concerns should be pasted in the space provided for this purpose in the enrolment form and if this
space is insufficient the space at the bottom of page 2 may be used for this purpose.
The photographs should be of Passport size and duly attested.
The name of the person should be written/typed on his photograph.
3. Particulars of Property
(a) Certificate from a Scheduled Bank should be submitted in original in respect of
movable property. This will be retained in Government record.
(b) Statement of Immovable property should be certified by a Deputy
Commissioner/Collector or a First Class Magistrate and Should be submitted in
original. This will be retained on Government record.
4. Particulars of experience
A list of works carried out during the last three years and work under execution, if any, should
be submitted giving the following particulars in respect of each work:-
(a) Name and description of work.
(b) Contract Number and Year.
(c) Value.
(d) Department, Chief Engineer, Superintending Engineer and Executive Engineer under whom executed.
(e) Date of completion.
5. Particulars of Contractor’s Organization and Supervision Staff
In addition to particulars to be given in para 4 and 5 of the enrolment form, the following are
also to be submitted:-
(a) A statement giving the details of tools and plant and transport owned by the contractor showing the
registered numbers of the vehicles etc. with authentic evidence.
(b) Original power of attorney in favour of the signatory to the enrolment form and in favour of the
person/persons competent to enter into contracts and arbitration agreements on behalf of the
contractor. An attested copy of the power of attorney on court stamp paper should also be submitted
if the return of the original is desired.
(c) In case of Sole Proprietor concern and affidavit in original to the effect that the applicant is the Sole
Proprietor should be submitted. This will be retained in Govt. record.
(d) In case of Partnership concern—
(i) Partnership deed in original together with an attested copy of thereof on court stamp paper.
The original will be returned after verification.
(ii) Registration certificate with the Registrar of companies, in original. This will be returned after
verification, keeping an attested copy of the same.
(iii) An attested copy of the audited balance sheet and the profit and loss account of the concern
for the last two years.
(e) In the case of limited concerns:-
(i) Attested copy of the Memorandum & Article of Association.
(ii) Attested copy of the Balance Sheet and Profit and loss Account of the previous year.

(iii) Attested copy of the Resolution passed by the Board of Directors appointing the Managing
Director(s) with the power delegated to him on behalf of the firm.
(f) In case enlistment is requested for manufacture of furniture etc. the following additional particulars
should be furnished:-
(i) Whether the applicant owns a workshop and/or show rooms for the furniture, if so complete
address of workshop/show room be given.
(ii) Whether the manufacture of furniture is applicant’s normal trade.
(iii) List of furniture works executed by the applicant giving particulars as in para 4(a) to (e) above.
(g) Names of permanent qualified technical staff with their technical qualification employed by the
contractor.
6. Income Tax Clearance and other Certificate
(a) An Income Tax Clearance Certificate on the form given in the enrolment form is to be
submitted in original. Without this the request for enlistment will not be considered.
(b) Besides the above the Central Sales Tax and Local Sales Tax Certificates should also be
furnished.
7. Relations in the Archaeological Survey of India, In the Ministry of Education and
Culture.
A list of persons who are working with the contractor applicant and who are near relatives of Officer
and staff in the Archaeological Survey of India or any officer of the Ministry of Education & Culture
should be submitted with the Enrollment form the full name, present official address, residential
address and the appointment held.
OR
The following certificates should be submitted duly signed by the Proprietor/all the partners in
case of partnership concern/Managing Directors in the case of limited concern.

Hereby certify that *I/We and none of the persons with us are near relatives of any Officer or staff in
the Archaeological Survey of India or any Gazetted Officer of the Ministry of Education & Culture.
*Delete whichever is inapplicable.
8. A list of persons related to the contractors or the Directors/partners/owners of firms serving in
the Archaeological Survey of India or the Ministry of Education & Culture has to be attached, or the
following Certificate may be submitted by the Contractor/firm:-
In the event of the enlistment of *my/our name as approved contractor or the Archaeological
Survey of India we undertake to notify the name(s) of any such persons employed by us subsequently.
*I/We also understand that we will not be entitled to tender for any works which our near relatives will
be deal with or with which our relatives are connected in any way and any break of this condition may
be considered sufficient for the removal of *my/our name from the list of approved contractor.
Note:
-1 By the term “near relative” is meant wife/husband, parents, children, brothers, sisters,
brother/sister-in-law, sons/daughter-in-law, uncle, first cousins and their corresponding in-law.
-2 Certificate/Credential etc.
All certificate/credential etc. should be submitted in original together with two attested copies
of each thereof if the originals are desired to be returned. Copies should be attested by a First Class
Magistrate or a Gazetted Officer with his court/office stamp attested thereon.

List of documents/information required to be furnished by the Indian firms:-
1. Memorandum and articles of Association/Partnership Deed/Affidavit.
2. Current Income Tax clearance certificate and PAN No.
3. Annual Profit and Loss Account and balance Sheet.
4. Power of Attorney (wherever necessary)
5. Name of Bankers.
6. Latest entry in Form “A” from the Register of the Registrar of Firms in the case of
Partnership concerns.
7. Local Sales-tax certificate
8. Central Sales-Tax Certificate.
9. Photo copies of registration with C.P.W.D. or any other departments like Railways and
M.E.S. etc., duly attested.
10. A declaration to the effect as to whether the Contractor/firm was ever black-listed by any
department of the Government of India, and if so, the reasons and duration of blacklisting
have be to specified.
11. Performance statement.
12. Photographs of Partners/Owners of the firms.
13. List of persons holding powers of attorney, together with their specimen signatures and
photographs duly attested by a Gazetted officer.
14. 14. Certificate of experience.
15. List of supervising staff with their qualifications.
16. Names of Govt. Departments in which registered as Contractors, together with attested
copies of registration certificates.
17. List of persons, who are related to the partners/owners of the firms, working in the
Archaeological Survey of India.
18. Copy of Licence for holding business.
19. Copy of Electric Licence held by the firm.
