However, dental status <strong>in</strong> adulthood, whilst stat<strong>is</strong>tically significant, appeared to be of<br />

m<strong>in</strong>or importance to overall self-esteem <strong>in</strong> a model that <strong>in</strong>cluded other psychological<br />

variables (low quality evidence).<br />

In addition Birkeland et al. found that 14 some patients <strong>with</strong> a great need for orthodontic<br />

treatment do not express orthodontic concern, whereas others <strong>with</strong> near ideal<br />

occlusion express concern. Th<strong>is</strong> seems <strong>in</strong> contrast <strong>with</strong> another conclusion of the same<br />

study, that there was also found an association between orthodontic concern and<br />

orthodontic treatment need assessed by IOTN. However th<strong>is</strong> confirms that there <strong>is</strong> a<br />

difference <strong>in</strong> op<strong>in</strong>ions between laypersons and orthodontic practitioners.<br />

A possible explanation of some d<strong>is</strong>crepancies between the perception of laypersons and<br />

OBSTRUCTIVE SLEEP-APNEA SYNDROME<br />

Although orthodontic treatment can be <strong>in</strong>dicated <strong>in</strong> patients <strong>with</strong> Obstructive Sleep<br />

Apnea Syndrome (OSAS), the topic <strong>is</strong> not considered relevant <strong>in</strong> the present age group,<br />

as the most common treatment for OSAS <strong>in</strong> children <strong>is</strong> adenotonsillectomy and the use<br />

of orthodontic appliances for OSAS <strong>in</strong> children, e.g. <strong>with</strong> craniofacial malformations, <strong>is</strong><br />

very rare.<br />

WHAT ARE, IN THE OPINION OF DENTAL PRACTITIONERS AND<br />

ORTHODONTIC PRACTITIONERS, THE BENEFITS OF ORTHODONTIC<br />

TREATMENT?<br />

As concluded from the literature review <strong>in</strong> the SBU-study 2 , untreated deviations from a<br />

normal occlusion have been l<strong>in</strong>ked by dental health professionals to deterioration <strong>in</strong><br />

oral health, oral function or both.<br />

General dental practitioners and orthodontic practitioners rated psychosocial ga<strong>in</strong> from<br />

orthodontic treatment higher than its dental health ga<strong>in</strong>, however both groups still felt<br />

that orthodontic treatment reduces susceptibility to dental d<strong>is</strong>ease 15 .<br />

ADVERSE EFFECTS OF UNTREATED MALOCCLUSIONS<br />

• Accord<strong>in</strong>g to consensus cl<strong>in</strong>ical expert op<strong>in</strong>ion, orthodontic treatment <strong>is</strong> an<br />

important aspect of the multid<strong>is</strong>cipl<strong>in</strong>ary treatment plann<strong>in</strong>g of cleft lip<br />

and/of palate patients.<br />

• Low quality of evidence was found that malocclusion <strong>is</strong> not associated <strong>with</strong><br />

<strong>in</strong>creased prevalence of caries.<br />

• Moderate quality of evidence was found on the correlation between a large<br />

overjet <strong>with</strong> <strong>in</strong>complete lipseal and the r<strong>is</strong>k on trauma of the upper front<br />

teeth.<br />

• Low quality of evidence was found for the correlation between <strong>in</strong>correctly<br />

positioned maxillary can<strong>in</strong>es before their eruption and the r<strong>is</strong>k on damage on<br />

the roots of adjacent teeth.<br />

• <strong>The</strong>re <strong>is</strong> conflict<strong>in</strong>g evidence on the relationship between moderate<br />

malocclusions and negative self-esteem among children and teenagers of 11-<br />

14 years old (Low quality of evidence).<br />

• Low quality of evidence was found on the fact that adults <strong>with</strong> untreated<br />

malocclusions express more d<strong>is</strong>sat<strong>is</strong>faction <strong>with</strong> their appearance of their<br />

bite than adults <strong>with</strong>out malocclusions.<br />

• However, dental status <strong>in</strong> adulthood, whilst stat<strong>is</strong>tically significant, appeared<br />

to be of m<strong>in</strong>or importance to overall self-esteem <strong>in</strong> one study model that<br />

<strong>in</strong>cluded other psychological variables (Low quality evidence).<br />

• <strong>The</strong>re <strong>is</strong> no conclud<strong>in</strong>g evidence on the relation between untreated<br />

malocclusion and periodontal problems, chew<strong>in</strong>g or speech problems, and<br />

temporomandibular d<strong>is</strong>orders.
