This portfolio evaluation sheet is to be placed in the ‘EVALUATION’ section of your portfolio.

The portfolio package The student has an appropriate package

The labeled sections The portfolio has all of the required labeled
sections. The labels are enhanced in

1) The introductory paragraph The student has an updated, well-written,
typed paragraph introducing him/herself
and stating his/her objectives in completing
the portfolio. The introduction is enhanced
in production and design.
2) The resume The resume is included, updated and
appropriately presented. The resume has

3) Personal accomplishments A new personal accomplishment is
included and is appropriately presented.

4) Community Service Evidence of community service that has
occurred in the last calendar year is
appropriately presented. Diploma status

5) Career planning Evidence of career planning (as described
at right) is included. The section is

6) Academic work Appropriate academic entries are included.

Grade 12 Exit Interview Checklist
After the portfolio review, Grade 12 IBT students will complete an exit interview with their evaluator.
Students are required to provide evidence of the following to the evaluator:

Completion of, at a minimum, 17 IBT credits (identified by a course code that ends in an
“R” or a “T”)
Completion of 4 Senior (Grade 11 or 12) IBT credits (this is part of the 17 required IBT
credits)
Completion of 3 IBT Language credits (this is part of the 17 required IBT credits, and
can be 1 of the 4 Senior IBT credits)
