IEP Forms – Special Education Referral
Purpose: To provide a referral that is specific to special education and
differentiates between other referrals in a district (like RtI, ELL, etc.)
Usage: When a parent or a school staff member refers a student.

Who Can Refer for Special Education?
Only an agent of the school or a parent may initiate a
referral to special education. Other interested parties
must work in collaboration with either the parent or
the school.
• This form may change!
• Identify the specific reason for referral so the
Evaluation Planning team understands the scope of
the concern.
Evaluation Planning
• The receipt of a referral to special education requires
a review of existing data (cumulative file, assessment
information, previous evaluation data, teacher
records, child study information, etc.)
• This information forms a basis for determining the
appropriateness of the referral and planning the
evaluation.
• This review and planning does not require a formal
meeting, but should involve critical and relevant
personnel.
• Each school may have a different process by which
this takes place.
Do We Have to Accept a Referral?
Once a referral is received, the team must either:
• Accept the referral and conduct any additional
evaluation, if necessary.
• Reject the referral detailing the decision on the
Prior Written Notice form.
