IEP Forms – Referral Review and Evaluation Plan
Purpose: To guide the referral review and evaluation planning process.
Usage: This form is not required, but documentation of the review of the referral and
individual plan for evaluation is important.

Overview
This form is intended to help school teams consider
how to conduct referral reviews and plan for
individually focused evaluations.
Upon receipt of a referral to special education, the
school must determine the appropriateness of the
referral and, if appropriate, what type of evaluation
data is needed,
if any.
Instructions
• Document that the parent was informed of the
referral.
• Briefly describe the reason for referral.
• Conduct a review of existing information.
• Fo rmulate Evaluation Questions. Examples:
• What is the student’s level of reading
fluency?
• How do they process verbal information?
• What are the student’s strengths? Needs?
• What is their primary language?
• Have they received appropriate instruction in
reading and math?
• How do they function independently?
• Mark the decision of the referral review team and
take the necessary next steps.
• Involve your special education director when
rejecting a referral and writing a Prior Written
Notice.
Remember the requirement is now to conduct a
focused evaluation that is sufficient to identify the
needs of the student. We are no longer required
to
engage in a comprehensive evaluation in all
domains.
Current hearing and vision screening results and
health
histories are no longer required, unless
determined
by the Evaluation Planning team.
