(d) Persons who are or were direct relatives of yours or directly related to you by marriage,<br />

who are or were relatives of yours <strong>in</strong> the collateral l<strong>in</strong>e up to the third degree, or who are or were related<br />

to you by marriage <strong>in</strong> the collateral l<strong>in</strong>e up to the second degree.<br />

NOTE: It is not permissible, however, to provide a false answer, to simply cross out the spaces provided<br />

for the answers, or to leave them blank. Instead, fill <strong>in</strong> “no <strong>in</strong>formation provided” or “no further<br />

<strong>in</strong>formation provided” depend<strong>in</strong>g on whether you wish to leave a question completely or partially<br />

unanswered.<br />

(4) Changes <strong>in</strong> marital status, name, residence, and nationality must be provided immediately to<br />

the security officer or staff.<br />

(5) The <strong>in</strong>formation provided by you will be treated as strictly confidential.<br />

c. Your Contact Person. The security officer is available for any questions you may have. If you<br />

wish to contact the security officer, check the appropriate box <strong>in</strong> item 11.<br />

d. Return<strong>in</strong>g the Security Questionnaire. Return the filled-<strong>in</strong> AE Form <strong>604</strong>-1B <strong>in</strong> a sealed<br />

envelope directly to the security officer or the security staff.<br />

Specify your previous names as<br />

“née/né,” “former married name,” et<br />

cetera (for example, “former married<br />

name Maier”).<br />

Underl<strong>in</strong>e the name by which you are<br />

called. Use the spell<strong>in</strong>g on your birth<br />

certificate. Do not use any abbreviated<br />

form of your name.<br />

Your place of birth us<strong>in</strong>g the spell<strong>in</strong>g on<br />

your birth certificate. If the name of the<br />

town has changed (for example,<br />

because of territorial reorganization),<br />

<strong>in</strong>dicate the new name of the town with<br />

the postal code <strong>in</strong> brackets. This does<br />

not apply if your place of birth was<br />

outside of Germany. Official<br />

abbreviations may be used to <strong>in</strong>dicate<br />

the State or country.
