place like home,” and the<br />

majority of <strong>Hospice</strong> &<br />

Community Care patients<br />

prefer to receive hospice care at home,<br />

there may come a time when another<br />

level of care is needed to manage<br />

symptoms. For these patients and their<br />

families, compassionate care is available<br />

at <strong>Hospice</strong> & Community Care’s Essa<br />

Flory <strong>Hospice</strong> Center in Lancaster<br />

and the Bob Fryer & Family Inpatient<br />

Center in Mount Joy.<br />

Designed to feel like home, the<br />

<strong>Hospice</strong> Inpatient Centers feature large,<br />

brightly-decorated rooms, private baths<br />

and patios, and plenty of comfortable<br />

space for family and friends. Stained<br />

glass windows, rose gardens, wandering<br />

paths and fountains add to the warm,<br />

calming atmosphere.<br />

“Family members may feel stress and<br />

uncertainty when bringing a loved one<br />

to the Inpatient Centers,” says Connie<br />

Simmons, <strong>Hospice</strong> & Community<br />

Care Director of Inpatient Services.<br />

“However, many families say that they<br />

feel a sense of calm relief after seeing<br />

how beautiful the Inpatient Centers are<br />

and how kind the staff is.”<br />

Families can find warm, welcoming<br />

living rooms to gather quietly together<br />

and meditation rooms offering peace<br />

and serenity for those seeking solitude.<br />

The Inpatient Centers and fullyequipped<br />

kitchens are available 24<br />

hours a day for families and visitors to<br />

spend time together. Families are also<br />

welcome to bring beloved pets along<br />

on visits.<br />

The Inpatient Team<br />

Patients and families receive<br />

personalized care from the professional<br />

Inpatient Center staff, whose<br />

knowledgeable, assuring presence helps<br />

put families at ease.<br />

<strong>Hospice</strong> & Community Care’s<br />

physicians may be contacted 24 hours<br />

a day and nurses work closely with the

patient and family to establish a plan<br />

for managing symptoms, including<br />

pain, wound care, breathing problems<br />

and nausea. Certified hospice aides are<br />

available to assist with personal care,<br />

feeding and meal preparation.<br />

The Inpatient Center stay is short-term<br />

and patients may return home after<br />

symptoms are controlled. Inpatient<br />

Center nurses spend time helping<br />

family caregivers learn how to manage<br />

caring for their loved ones. Caregivers<br />

can learn how to give medications,<br />

help care for wounds and provide<br />

personal care.<br />

Linda Wolf, RN, Team Leader,<br />

Lancaster Inpatient Center says,<br />

“<strong>Hospice</strong> patients and families who<br />

are receiving care at home and are<br />

struggling with pain or other difficult<br />

symptoms should speak to a member<br />

of their Home <strong>Hospice</strong> Team. A shortterm<br />

stay at the Inpatient Center<br />

could provide a quick turnaround of<br />

distressing symptoms and the patient<br />

could then return home when the<br />

symptoms are more managed.”<br />

Social workers are key <strong>Hospice</strong><br />

Inpatient Center team members and<br />

are available every day to provide<br />

emotional support and assistance<br />

with care plans, accessing community<br />

resources and medical insurance<br />

questions and paperwork.<br />

The Inpatient chaplain is available<br />

to provide spiritual support and<br />

can coordinate spiritual needs with<br />

a family’s clergy. For many, the<br />

opportunity to share concerns and<br />

emotions, ask questions or just have<br />

a kind hand to squeeze is a comfort<br />

during a stressful time.<br />

Board-certified and licensed<br />

complementary therapists may also<br />

assist through the use of music therapy<br />

or massage therapy, helping to lessen<br />

anxiety, reduce pain and provide an<br />

enhanced sense of calm.<br />

<strong>Hospice</strong> volunteers play a significant<br />

role in the personalized care patients<br />

and families receive. From distributing<br />

Comfort Kits containing toiletries,<br />

snack bags and useful items for family<br />

members who spend long hours in the<br />

Inpatient Centers, to sharing books and<br />

just holding hands, the companionship<br />

of these volunteers provides an often<br />

much-needed respite for family<br />

caregivers.<br />

The advanced stage of a serious<br />

illness can be the most vulnerable and<br />

challenging time a family can face.<br />

<strong>Hospice</strong> & Community Care provides<br />

care and support not only for patients<br />

but for the family members who care<br />

for them and share their lives, no<br />

matter where they may call “home.”<br />

<strong>Hospice</strong> and Senior Care Facilities<br />

A Model Partnership<br />

For 19 years, <strong>Hospice</strong> &<br />

Community Care has been<br />

working in close collaboration<br />

with senior care communities and<br />

nursing homes to form successful<br />

partnerships based on a common goal:<br />

to provide compassionate and skilled<br />

care of the highest quality to people<br />

who are in the final weeks to months<br />

of life.<br />

Skilled communities can offer hospice<br />

care to residents facing end of life by<br />

developing working relationships,<br />

including formal contracts, with a<br />

hospice provider. One third of patients<br />

cared for on a daily basis by <strong>Hospice</strong><br />

& Community Care live in skilled<br />

nursing locations.<br />

Through this partnership, the <strong>Hospice</strong><br />

interdisciplinary team of physicians,<br />

nurses, social workers, chaplains,<br />

complementary therapists and<br />

volunteers are identifying needs and<br />

supplementing the care provided by<br />

these skilled communities by helping<br />

provide medical, emotional and<br />

spiritual support and care to residents<br />

and their families.<br />

The quality, individualized care<br />

<strong>Hospice</strong> provides to residents of these<br />

skilled communities is no different<br />

than that provided to <strong>Hospice</strong> patients<br />

who are living at home.<br />

Residents who are eligible for Medicare<br />

can receive hospice benefits if their<br />

doctor and the hospice medical<br />

director certify that they are terminally<br />

ill and have six months or less to live<br />

if the illness runs its normal course.<br />

The <strong>Hospice</strong> Medicare Benefit allows<br />

the patient to receive care where they<br />

live. Medicare also covers short-term<br />

inpatient care in an inpatient center<br />

or hospital, if the <strong>Hospice</strong> team<br />

determines that this level of care is<br />

needed to manage symptoms. Shortterm<br />

respite care is also covered if the<br />

caregiver becomes ill or needs a rest.<br />

A comprehensive “Plan of Care”<br />

outlining needed care and services<br />

to manage symptoms is created by<br />

the <strong>Hospice</strong> team, the senior care<br />

community staff, the resident, their<br />

physician and family or caregiver.<br />

<strong>Hospice</strong> also supports the staff, both<br />

during care and as they grieve a loss.<br />

with dementia, the care and comfort<br />

of residents as conditions progress,<br />

and communicating with patients<br />

and families at end of life. <strong>Hospice</strong><br />

interdisciplinary team members also<br />

provide ongoing caregiver education to<br />

family members and caregivers.<br />

<strong>Hospice</strong>’s specialized end-of-life care<br />

and resources, including its staff of<br />

hospice and palliative care physicians,<br />

and nurse practitioners with<br />

specialized certifications and clinical<br />

support, are available 24 hours a day.<br />

<strong>Hospice</strong> offers specialized wound care<br />

services to help patients and caregivers<br />

struggling with hard-to-heal wounds.<br />

The program is headed by Marygrace<br />

Lomboy, a certified wound care nurse<br />

practitioner uniquely qualified to<br />

assist in management of non-healing<br />

wounds. Lomboy assesses wounds,<br />

recommends treatments and can<br />

educate staff and caregivers on how to<br />

manage dressings and treatments.<br />

Sara Wright, a <strong>Hospice</strong> & Community<br />

Care geriatric nurse practitioner, has<br />

been providing geriatric care for more<br />

than 20 years. Wright assists patients,<br />

families and caregivers who are coping<br />

with challenging care issues that may<br />

arise in persons with moderate to<br />

advanced dementia.<br />

While <strong>Hospice</strong> works with a large<br />

number of senior care communities,<br />

in an expanding geographic region,<br />

including all of Lancaster and York<br />

counties and parts of Adams, Berks,<br />

Chester, Dauphin and Lebanon<br />

counties, <strong>Hospice</strong> prides itself on<br />

the individualized attention the<br />

organization is able to provide.<br />

Spotlight on York<br />

Serving Patients and Families in York<br />

and Adams Counties<br />

<strong>Hospice</strong> & Community Care’s York Home <strong>Hospice</strong> Team now<br />

provides hospice care, wound care, dementia support and<br />

bereavement support to patients and families in York and<br />

Adams counties.<br />

Many of the patients we’ve cared for from the York area have<br />

received care at one of our two Inpatient Centers.<br />

Support through the Pathways Center for Grief & Loss is<br />

available to patients, families and all community members in<br />

York and Adams counties, with most programs available free<br />

of charge, due to generous community support.<br />

Pathways Center for Grief &<br />

Loss Support Programs<br />

The following support programs are being offered in York by<br />

the Pathways Center for Grief & Loss:<br />

• “Coping with the Loss of a Spouse or Companion”: Tuesdays,<br />

February 12, 19 and 26, 12:30–2:00 p.m.<br />

• “Newly Bereaved Program”: Tuesdays, May 7, 14 and 21,<br />

6:30–8:00 p.m.<br />

Chimaqua—could you serve as a buddy to accompany a<br />

grieving child? A registered nurse and a certified lifeguard<br />

are also needed to volunteer at Camp Mend A Heart. The<br />

Pathways Center for Grief & Loss provides two wonderful<br />

opportunities for children impacted by the loss of a loved one:<br />

• Camp Mend A Heart, a one-day camp for children and<br />

teens ages 6–18 who are grieving—Saturday, June 1, 9:00<br />

a.m. to 7:30 p.m., at York YWCA’s Camp Cann-Edi-On.<br />

• Camp Chimaqua, an overnight weekend camp for<br />

grieving children ages 6–12—June 7–9, at Camp Donegal<br />

in York County.<br />

The focus of both camps is to help children learn about loss,<br />

develop coping skills, build self-confidence and learn how<br />

to trust and love again. A trained adult buddy is paired with<br />

each child camper to provide constant companionship,<br />

support and supervision. It’s a fun and meaningful event—for<br />

both buddies and campers. Please call Sonya Hershey-Velasco<br />

at (717) 391-2413 to request a volunteer buddy application.<br />

November 30 Extraordinary Give Success<br />

thank you! We were overwhelmed by the generosity and participation of the<br />

community during the Lancaster County Foundation’s “Extraordinary Give”<br />

campaign in November. Generous support for <strong>Hospice</strong> & Community Care<br />

resulted in:<br />

• $20,162 raised<br />

• Plus an additional $5,760 in prizes and matching funds<br />

…all to provide care and support for <strong>Hospice</strong> patients and families.<br />

Over the years, many in our community have expressed a desire to contribute to <strong>Hospice</strong> in a way that also<br />

commemorates a loved one, family member or friend.<br />

The Path of Remembrance, which wanders through the rose garden and peaceful grounds of the<br />

Bob Fryer & Family <strong>Hospice</strong> Center in Mount Joy, fulfills this wish and provides a beautiful<br />

setting for peace and reflection.<br />

By purchasing an engraved brick, you create a lasting memory that honors a loved one.<br />

Proceeds from the Path of Remembrance also help support a variety of essential patient<br />

and family services, including hospice care and bereavement support for patients and<br />

families who are unable to pay for care.<br />

Bricks may be purchased at any time throughout the year, and will be engraved<br />

and added to the Path once a year. <strong>Hospice</strong> & Community Care will host a<br />

Dedication Ceremony to celebrate new bricks being added to the Path on:<br />

President’s Letter<br />

in 2012, <strong>Hospice</strong> & Community Care responded<br />

to a growing need for our care and services from<br />

surrounding counties. We changed our name,<br />

collaborated with WellSpan Health, opened a<br />

new office and established a dedicated team to serve<br />

patients and families in York and Adams counties.<br />

We are already seeing a number of families turning to<br />

<strong>Hospice</strong> for care and support not only at home but<br />

in our <strong>Hospice</strong> Inpatient Centers. In <strong>2013</strong>, we will<br />

continue to see expanded programs and services in this<br />

area, including bereavement support for adults, teens<br />

and children.<br />

This issue takes a focused look at our Inpatient Centers, to help assure families<br />

of the care their loved ones can receive in a beautiful, home-like atmosphere and<br />

how our valued relationships with senior care communities help provide the best<br />

end-of-life care for their residents.<br />

As always, we are grateful for the ongoing support of our community which<br />

makes this care possible for so many. This dedication was reflected during<br />

the Extraordinary Give, a first-ever 24-hour online giving marathon held in<br />

November by the Lancaster County Community Foundation. Your response was<br />

overwhelming and we thank you.<br />
