F-1s/J-1s Fellowship/Scholarship<br />
The following forms should be completed:<br />
1) Alien Certificate and if a J-1 visa holder also attach the DS-2019<br />
2) If recipient is not from a tax treaty country no other forms are<br />
needed<br />
3) If recipient is from a tax treaty country complete a Form W-8BEN<br />
4) If recipient is from a tax treaty country and ineligible for a SSN they<br />
need to complete a Form W-7 (ITIN application)<br />
These forms can be found on the payroll website under ‘<strong>Foreign</strong><br />
Students, Faculty & Staff’, Required Tax Forms, under Student on<br />
Fellowship/Scholarship
