Especially chronic wounds and wounds that do not heal well and<br />
are affected by necrosis and layers often require repeated local<br />
surgery for wound débridement in addition to a sufficient therapy<br />
for the basic condition. However, these surgeries are extremely<br />
painful. Often, local anaesthesia or even surgery under anaesthetic<br />
is required. Furthermore, mechanical débridement cannot always be<br />
exactly restricted to the vital border zone; there is always the danger<br />
that intact granulation tissue is also removed.<br />
The <strong>Ultrasonic</strong> <strong>Assisted</strong> <strong>Wound</strong> <strong>Treatment</strong> (UAW) with the<br />
Sonoca ® -180 allows a new kind of treatment as an alternative to<br />
mechanical wound débridement.<br />
During UAW low-frequency ultrasound is applied together with a<br />
wound treatment solution and leads to a destruction of bacteria<br />
and a flush-out of those, an increase of the healing process and a<br />
lowering of pain.<br />

Chronische und schlecht heilende Wunden mit Nekrosen<br />
und Belägen machen neben einer suffizienten Therapie der<br />
Grunderkrankung auch immer wieder lokale Eingriffe im<br />
Sinne eines Wunddébridements notwendig. Diese sind jedoch<br />
durch eine erhebliche Schmerzhaftigkeit gekennzeichnet.<br />
Häufig wird eine lokale Anästhesie oder sogar ein Vorgehen<br />
in Narkose erforderlich. Zudem kann beim mechanischen<br />
Débridement eine exakte Abtragung nicht immer in der vitalen<br />
Grenzzone erfolgen, sondern erfaßt teilweise auch intaktes<br />
Granulationsgewebe.<br />
Die Ultraschall Assistierte Wundbehandlung (UAW) mit dem<br />
Sonoca ® -180 ermöglicht eine neue Behandlungsmethode im<br />
Gegensatz zum rein mechanischen Wunddébridement.<br />
Bei der UAW wird niederfrequenter Leistungs-Ultraschall<br />
zusammen mit einer Wundbehandlungslösung angewendet und<br />
führt dazu, dass Bakterien zerstört und herausgespült werden,<br />
die Heilung beschleunigt und der Schmerz gelindert wird.<br />

With Ultrasound <strong>Assisted</strong> <strong>Wound</strong> <strong>Treatment</strong>, the wound cavitation<br />
processes feature a high-efficient deep penetrating bactericidy.<br />
Cavitations (micro gas bubbles imploding cyclically) cause destruction<br />
of bacteria, viruses and fungi. The ultrasound pulse causes the<br />
wound treatment solution to penetrate more deeply into the fissures<br />
of the tissue, then mechanical rinsing effects.<br />
Fibrin deposits and bacteria growth are flushed out gently. The<br />
central liquid supply through the probe tip (Sonotrode) shaft has<br />
been developed for tasks where direct application to a specific area<br />
is required.<br />
Infected chronic wounds are characterised by an acidic wound environment<br />
that causes pain. The traits of aseptic wounds change as<br />
the bacteria remnants are broken down and the wound environment<br />
turns neutral, starting to become permanently free of pain. The neutral<br />
wound base tends to heal more quickly.<br />

Durch den Ultraschall bewirken Kavitationsprozesse eine<br />
Tiefendesinfektion der Wunde, ausgelöst durch die bakterizide<br />
Wirkung des Ultraschalls. Die Kavitationen (zyklisch<br />
implodierende Mikrogasblasen) führen dabei zur Zerstörung<br />
von Bakterien, Viren und Pilzen.<br />
Die Wundbehandlungslösung wird durch den Ultraschallimpuls<br />
tief in die Gewebespalten hineinkatapultiert, so dass die<br />
Kavitationseffekte tiefer in die Wunde reichen als reine Spüleffekte.<br />
Dies führt, zusammen mit der Behandlungslösung,<br />
zur schonenden Ausschwemmung von Fibrinablagerungen<br />
und Bakterienrasen. Die zentrale Flüssigkeitszufuhr durch<br />
den Sonotrodenschaft wurde hierbei gezielt für bestimmte<br />
Aufgabenstellungen entwickelt.<br />
Die infizierte chronische Wunde ist durch ein saures,<br />
Schmerzen bereitendes Wundmilieu geprägt. Mit dem Abbau<br />
der Bakterientrümmer durch die UAW ändert die keimfreie<br />
Wunde ihre Eigenschaften in ein neutrales, dauerhaft schmerzfreies<br />
Wundmilieu. Der chemisch neutrale Wundgrund zeigt<br />
eine beschleunigte Heilungstendenz.<br />

The Sonoca ® <strong>Ultrasonic</strong>-/Dissector is a compact unit and operates<br />
on 25kHz. The working frequency is automatically and permanently<br />
adjusted. All handpieces are individually computer coded: the last<br />
working parameters are stored and automatically reset when a handpiece<br />
is reconnected.<br />

The Söring system operates with high precision ceramic discs (PZT-<br />
Transducers) within the handpiece convert electrical input to mechanical<br />
output with almost 100% efficiency to generate ultrasonic vibration<br />
and create oscillation at the probe-tip (Sonotrode). No thermal<br />
energy is generated and so no cooling system is required, therefore<br />
handpieces can be continuously used without restricting the level of<br />
operating power output.<br />

in Schwingungen. Durch entsprechende Anordnung dieses<br />
Systems erzeugt das Sonoca ® Gerät Ultraschall-Schwingungen<br />
fast ohne Verlustleistung. Es kann daher auf eine Kühlung der<br />
Handstücke verzichten werden, entsprechend hoch ist die<br />
Leistungsausbeute; Dauerbetrieb ist jederzeit möglich.<br />
